A double-antibody immunoprecipitation method was developed for detecting antibody to liver-specific membrane lipoprotein (anti-LSP) in sera of patients with various liver diseases and primary nonhepatic autoimmune diseases. Liver-specific membrane lipoprotein prepared from normal rat livers was labeled with 125I (chloramine-T) and monospecific antibody raised in rabbits. Cross-reactivity and absorption studies demonstrated that the assay used was highly specific. The frequency and titer of anti-LSP were similar for HBsAg-positive and -negative patients with both acute and chronic liver diseases. Patients with chronic active hepatitis had the highest frequenzy (25 of 44 cases, 57%) when compared with those with chronic persistent hepatitis (5 of 23 cases, 22%) and nonalcoholic cirrhosis (8 of 21 cases, 38%). Of the anti-LSP positive cases, the mean titer in patients with chronic active hepatitis tended to be the highest. In patients recovered from acute viral hepatitis, anti-LSP was transiently positive (7 of 20 cases, 35%) in the acute phase. In those who progressed to chronic hepatitis, a late rise as well as an early rise occurred in 6 of 10 patients before the diagnosis was made. Two of 6 patients with primary biliary cirrhosis had anti-LSP, but none of 41 patients with other nonviral liver diseases and none of 60 patients with primary nonhepatic autoimmune diseases. These data indicate that an autoimmune reaction directed against LSP can be initiated during the acute phase of viral hepatitis and it may persist in chronic hepatitis in both HBsAg-positive and -negative cases.

The results of liver scans performed with 99mTc-sulphur colloid in 169 patients suffering from diffuse liver diseases and in 48 normal controls were evaluated. The patients with reactive hepatitis, acute hepatitis, chronic persistent hepatitis, fatty liver and fibrosis of the liver show only minimal deviations from the scintigraphic pattern. On the contrary, highly increased colloid uptake in the spleen is found in cases of chronic aggressive hepatitis, whilst the intrahepatic distribution of the colloid is approximately normal. In cases of liver cirrhosis, increased colloid uptake is found in the left lobe of the liver as well as in the spleen and in the bone marrow. Either normal findings or cirrhosis-like changes of the colloid distribution are observed in patients with alcoholic hepatitis.

A histopathological study was carried out on 27 patients with chronic inflammatory liver disease and clinical and/or biochemical evidence of cholestasis who had either mitochondrial antibodies against mitochondrial antigen fractions of 1.19 density ("PBC antigen"; 14 cases) or of 1.13 density ("CAH-PBC mixed-type antigen"; 13 cases). For comparison, the liver biopsies of 17 patients with chronic-aggressive hepatitis (CAH) and antinuclear and/or anti-smooth muscle antibodies but without cholestasis and mitochondrial antibodies, were evaluated. The 14 patients with mitochondrial antibodies against the PBC antigen showed the typical histological features of primary biliary cirrhosis (PBC). The 13 patients with mitochondrial antibodies against the CAH-PBC mixed-type antigen had heterogenous liver alterations. In 11 cases highly active CAH and/or active postnecrotic cirrhosis (AC) were found both with augmented ductular proliferation. Some of these cases showed distinct criteria of PBC as early bile duct lesions or absence of regular bile ducts. The liver histology of one case corresponded to classical PBC; another case to chronic persistent hepatitis. The CAH-patients without cholestasis and mitochondrial antibodies only occasionally showed bile duct proliferation. In conclusion, a high correlation was found between mitochondrial antibodies against the CAH-PBC mixed-type antigen and highly active CAH or early AC with augmented ductular proliferation. This represents an overlapping of CAH and PBC. In contrast, the cases with antibodies reacting to the PBC antigen showed the slowly progressive liver changes of typical PBC.

Lipoprotein electrophoresis with measurement of serum lipids was performed on 115 patients with various forms of liver disease. There was a reduction in alpha-lipoproteins and an increase in beta-lipoproteins, as well as a reduced separability of pre-beta and beta fractions in those with acute viral hepatitis. All these changes regressed completely with healing. Similar changes were shown also in chronic liver disease and were most marked in acute liver failure, but also marked in decompensated liver cirrhosis and chronic progressive hepatitis, while less marked in chronic persistent hepatitis and compensated liver cirrhosis. In patients with fatty livers there were no characteristic findings other than a slight increase in pre-beta lipoproteins. On the other hand, the lipoprotein pattern was markedly changed in cases with tumour in the region of the gallbladder, but similar changes were noted also with tumours at other sites. They are, therfore, unlikely to be liver-specific.

A routine search for Australia antigen in 29,936 blood donors at the Orleans Hospital Blood Transfusion centre led to the discovery of 105 apparently healthy carriers. Liver function tests were carried out in 80 of the latter and revealed abnormalities in 34 of them. Out of 18 patients who had no other explanation such as alcohol or drugs and who had abnormal tests six months later, 11 accepted liver biopsy. Histology revealed 4 cases of post-hepatic cirrhosis, 2 cases of chronic aggressive hepatitis, 2 cases of chronic persistent hepatitis and 3 livers with non-specific changes.

The morphologic type of cirrhosis that is followed most frequently by hepatocellular carcinoma is posthepatitic cirrhosis. Furthermore, HB AG is detected in a high rate among cases with hepatocellular carcinoma suggesting the intimate causal relationship between hepatitis b virus and hepatocellular carcinoma. It has been considered that hepatocellular carcinoma might develop during destruction and regeneration of fully developed liver cirrhosis. However, hepatocellular carcinoma is combined with not only liver cirrhosis but also with mild liver fibrosis. An attempt was made to determine HBs Ag in the liver tissue of liver fibrosis with hepatocellular carcinoma. HBs Ag was found in non-cancerous liver tissue of 40 percent of those cases. Therefore, it may be concluded that, at least some of those fibrosis is caused by chronic viral hepatitis and hepatocellular carcinoma may develop not only on posthepatitic cirrhosis but also on chronic persistent hepatitis. This evidence also suggests the carcinogenicity of hepatitis B virus.

Absolute number of T and B lymphocytes were measured in the peripheral blood of 25 patients with biopsy proven HBsAg-positive Chronic Active Hepatitis (CAH), in 9 with Chronic Persistent Hepatitis (CPH), and in 25 HBsAg-negative healthy controls. In patients with CAH and cirrhosis total T lymphocytes were decreased; no difference were found between patients with CPH and normal controls. This data emphasize the importance of monitoring T-cells population in patients with HBsAg-positive CAH.

The characterisation of lymphocytes from liver biopsies indicates that 'activated' T lymphocytes are present in the liver in alcohol induced hepatitis, chronic active hepatitis (HBS+ve and -ve), and in primary biliary cirrhosis but not in inactive cirrhosis, chronic persistent hepatitis, extrahepatic and drug induced cholestasis. A greater percentage of lymphocytes bear Fc-receptors in chronic active hepatitis than in alcohol induced hepatitis or cholestatic liver disease. The concentration of 'activated' T cells in the peripheral blood in all groups studied was within the normal range, suggesting that the 'activated' T cells found in the liver were reacting to either native or foreign antigens within the liver. The data on Fc-receptor bearing cells are consistent with the involvement of antibody assisted K cell mediated cytotoxicity in chronic active hepatitis.

The classification, clinical course, etiology and treatment of chronic hepatitis are discussed. The clinical manifestations of chronic hepatitis are of limited diagnostic use. Diagnosis must be made by liver biopsy. The disease is classified as chronic persistent or chronic active hepatitis. The prognosis for chronic persistent hepatitis is excellent, and no treatment is required. Chronic active hepatitis may progress to cirrhosis and is associated with a poor prognosis if untreated. Recognized causes of chronic active hepatitis are hepatitis-B virus infection, post-transfusion hepatitis not associated with hepatitis-B virus, and certain drugs. For drug-induced hepatitis, discontinuation of the medication is indicated. For other types of chronic active hepatitis the recommended treatment is prednisone 10 mg and azathioprine 50 mg daily.

The AA., after the literature revision concerning the autoantibodies in the autoimmune diseases, have examined the cases of acute and chronic hepatitis happened in the period 1972-1976. After a short observation of used methodologies the AA. have connected the presence of autoantibodies (FN, SM, AM) with the rate of immunoglobulins in single groups of liver diseases, divided in acute hepatitis, chronic persistent hepatitis, chronic active hepatitis, liver cirrhosis (cryptogenetic and alcoholic). The results are that while the immunoglobulins fractions increase, although in different manner, in every pattern of liver disease studied, instead, there are no typical changes of single immunoglobulins rate in the groups with autoantibodies. Statistically it is not possible to assert that single antibodies belong to immunoglobulins determinate class. Finally it had been impossible to demonstrate sex and age influence on the immunoglobulins increase in the groups of liver disease with autoantibodies.
