enterocolitica (Y. e.) may cause generalized infection in patients with malignant and other serious diseases or immunodeficient subjects. In certain conditions, elevated serum and tissue iron concentrations represent an additional risk factor for systemic infection with this opportunistic bacterium. In our patient, Y. e. septicemia developed during
liver cirrhosis
decompensation. Clinical signs of infection were alleviated by appropriate antibiotic therapy (gentamycin, cefuroxime), but as septicemia had been present for several days prior to therapy, it aggravated the patient's general condition, which entailed the development of hepatorenal syndrome and eventually lethal outcome.

It is well-known that
cirrhosis
is a predisposing factor to
Yersinia
septicemia. This study includes 73 cirrhotics and shows a high number of positive serologic tests (47/73 : 64.4%). However, there is no correlation with clinical features or bacteriological findings. The most frequent serotypes, i.e. pseudotuberculosis IV and enterocolitica 0:9, differ from those which are usually found in
Yersinia
septicemias. Iron overload in
cirrhosis
, increased intestinal load of gram-negative bacilli and possible latent bacteremia may partly explain these results. However, the role probably played by as yet poorly known cross-reactions between
Yersinia
and other pathogens (Shigella, E. Coli...) must be underscored. The authors conclude that slightly positive, stable, serodiagnostic tests have little meaning in cirrhotics.

Yersinia
enterocolitica, a gram negative aerobic non-haemolytic bacillus, has been identified as a cause of meningitis only once before and the neuropathological features of Y. enterocolitica meningitis have not been reported in the literature. We describe the pathological features of a case of acute fatal meningitis caused by Y. enterocolitica, serotype 0:18, in a 47-year-old male alcoholic with
cirrhosis
. The organism presumably reached the meninges via the blood stream from the abdominal cavity. Intravascular fibrin thrombi, both recent and organising, were associated with multiple foci of necrosis in the brain. In spite of vigorous antibiotic therapy for three weeks the organism was cultured from the brain at autopsy. Among the various infections caused by Y. enterocolitica, meningitis appears to be the most difficult to eradicate.

hepatic cirrhosis
who developed suppurative arthritis of the shoulder caused by Y enterocolitica is presented. There was no evidence of gastrointestinal infection or septicaemia. The severity of the illness was associated with various factors, including the underlying disease, the type of occupation, and the difficulties encountered in defining the optimal therapeutic regimen.

We report three cases of multiple liver abscesses due to
Yersinia
enterocolitica that led to previously unknown diagnoses of primary hemochromatosis. Y. enterocolitica is an iron-dependent bacterium that relies entirely on exogenous iron for growth. A review of the literature with use of MEDLINE (National Library of Medicine, Bethesda, MD) disclosed 35 cases of Y. enterocolitica liver abscesses; 21 (60%) of these cases were associated with hemochromatosis. In 11 of the remaining 14 cases, two common manifestations of hemochromatosis, diabetes mellitus and
cirrhosis of the liver
, also were present; these findings were significant. Finally, we emphasize that when iron overload cannot be documented at the time of diagnosis of the liver abscess, long-term follow-up for determination of increasing iron stores is mandatory. With this approach, most manifestations of hemochromatosis in asymptomatic patients can be prevented.

pseudotuberculosis is a rare cause of disease in humans, the most common manifestation being mesenteric lymphadenitis accompanied by abdominal pain and fever. A septicemic form of
Yersinia
pseudotuberculosis infection has been reported only rarely. It is usually seen in patients with underlying disorders such as diabetes,
hepatic cirrhosis
or iron overload. Fifty-four cases of septicemic infection were found in the literature. The earlier published cases are reviewed, and four cases occurring in Finland during the period February to June 1992 are reported.
