The activities of several glycosidases (beta-galactosidase, beta-glucosidase, N-acetyl-beta-glucosaminidase) were demonstrated in human bile. The enzyme activities are increased about 100 times after exclusion of bile salts and other small molecular compounds by Sephadex G-50 gel filtration. The use of 4-methylumbelliferyl derivatives as substrates was useful as measurement of the bile enzyme activities are not altered in the presence of bile pigments. Enzyme characteristics of bile glycosidases were determined: pH optimum and isoelectric point. The bile glycosidase activities were also measured in various hepatobiliary disorders (cholelithiasis,

of gallbladder, acute hepatitis,
liver cirrhosis
and fatty liver). The glycosidase activities in bile from patients with liver diseases, as well as with cholelithiasis, were generally decreased. Isoelectric focusing patterns of biliary glycosidases were similar for specimens from patients with hepatobiliary disorders as compared to normal.

Serum gamma-glutamyl transpeptidase (gamma-GT) level was estimated in 132 patients with different liver diseases (chronic persistent and chronic active hepatitis, postnecrotic
cirrhosis
, chronic alcholic hepatitis and alcoholic cirrhosis, cholestasis syndrome, fatty liver, Gilbert disease) and
malignancies
with and without liver involvement. The gamma-GT levels were compared with the values for serum bilirubin, transaminases (GOT, GPT) and alkaline phosphatase in the same patients. gamma-GT values were normal in chronic persistent hepatitis and increased in chronic active hepatitis. Very high activities were measured in chronic alcoholic cirrhosis in contrast to postnecrotic
cirrhosis
. gamma-GT proved to be more sensitive than alkaline phosphate as an index of cholestasis and liver involvement in
malignancies
. It is suggested that gamma-GT activity offers valuable aid in differential diagnostics of liver-diseases. gamma-GT being an inducible enzyme, its activity may be raised by enzyme inducing drugs also in subjects without liver disease.

Ascitic fluid samples from 19 patients with ovarian carcinoma, 3 with a benign ovarian tumor, and 5 with
cirrhosis of the liver
were examined for their content of coagulation factors and components of the fibrinolytic system. The concentration of trypsin inhibitors in the ascitic fluid was significantly higher in the presence of carcinoma. Large amounts of FDP were found in the ascitic fluid in all patients with malignant tumors, but not in the other two groups. Determination of FDP may therefore make it possible to differentiate between malignant and nonmalignant ascitic fluid.

, 67Ga was concentrated in the defect or defects visible on the 99mTc-sulfur colloid scan, but in the remaining 6 cases (33%), the 2 scans were identical and the defects may have been attributed wrongly to

. Alpha-fetoprotein (AFP) was detected by immunodiffusion in the serum of 26 patients. Twenty-one of these showed selective uptake of 67Ga by the tumor as compared with 6 out of 12 patients in whom this protein could not be detected. We were therefore unable to confirm a previous finding of a greater uptake of the of the radionuclide in AFP-negative primary liver cancer.

"e" is a serum antigen associated with type-B hepatitis. It is found only in hepatitis B surface antigen (HBsAg) positive sera, but is antigenically distinct from HBsAg. e antigen was not detected in the serum of any of 99 cases of acute type-B hepatitis who recovered normally. Its antibody, anti-e, was found in 14 (14%). The antibody usually appeared before clearance of HBsAg and before appearance of HBsAb. Serum e was not detected in any of 29 symptom-free carriers of HBsAg, but 21 (73%) showed anti-e. Serum e was found in chronic active hepatitis (44%) and chronic persistent hepatitis (31%). The antibody, however, was detected in only 2 of 79 patients with chronic active hepatitis but in 7 (44%) of chronic persistent hepatitis. Serum e was not found in 5 patients with primary liver-cell carcinoma or 5 with inactive HBsAg-positive
cirrhosis
. The antibody was, however, found in all 5 of those with inactive
cirrhosis
and in 4 of the 5 with primary
cancer
. These results suggest that the presence of e antigen is associated with active and usually continuing liver disease. Anti-e, however, is associated with inactive liver disease and asymptomatic carriage of HBsAg, and its presence must be regarded as a valuable sign in predicting those who will escape progressive chronic liver disease.

Intracytoplasmic hyaline globules (IHG) were found in 16 out of 105 hepatocellular carcinomas (15.2%). In all cases, there was a male preponderance, most of the patients were Black, and the mean age was in the fifth decade. IHG were mostly noted in the better-differentiated tumors. Bile production was present in 12% of all tumors. Over 80% of tumors with globules showed necrosis as compared with 40% of the tumors without globules.
Cirrhosis
was present in almost half the cases, with liver cell dysplasia in 15%. These globules are possibly alpha-fetoprotein. Some of them may be giant lysosomes. Their role in carcinogenesis and prognosis is uncertain.

The plasma level of alpha1-fetoprotein in 35 hepatic patients with a "cold" area showed by liver scanning has been detected by means of the radioimmunoassay technique. High levels (more than 320 ng/ml) of AFP were found in 4 cases of primary carcinoma of the liver; low concentration of AFP was found in 1 case of hepatoma. In 4 cases of liver metastasis the plasma levels of AFP were very low; the highest concentration (10 ng/ml) was found in a patient with a
cancer
of the colon. Low levels of AFP were found in all the cases (26) of
hepatic cirrhosis

The existence of Alpha-Fetoprotein was investigated in sera of 544 patients with laparoscopically and/or histologically proved chronic liver diseases and
cancer
of the liver. The applied methods were the double-diffusion technique (micro-Ouchterlony), the counter immunelectrophoresis and the latex agglutination test. In 15 patients with primary
cancer
of the liver there was a positive result for Alpha-Fetoprotein with the first two methods, 73,3 and 80,67%, respectively. In other liver diseases (
liver cirrhosis
, chronic hepatitis, malignant tumors with liver metastases) there were only a few positive results (4,17% in patients with liver metastases of unknown origin and less than 1% for the other disease groups). In this group the latex test showed a higher frequency of positive tests for Alpha-Fetoprotein, however with a concomitant rise of positive Waaler-Rose test in 80-93%. In the case of a positive rheumatic factor the latex test will therefore demand a cautious judgement because the latex particle coated with rabbit-anti-Alpha-Fetoprotein-globulin could be agglutinated by anti-rabbit-globulin existing in human serum. A correlation between the existence of Alpha-Fetoprotein and clinical parameters or laboratory datas does not exist. Increases of Alpha-Fetoprotein shown by counter immunelectrophoresis exist in primary cancers of the liver and embryonic tumors, sometimes and only temporarely in acute viral hepatitis, in some sorts of
liver cirrhosis
of indian children and in the case of pathologic gestation. If these last-named diseases are excluded a positive test for Alpha-Fetoprotein by precipitation methods is highly suggestive for primary
cancer
of the liver. Finally, the determination of this substance has a certain value for the therapeutic control of Alpha-Fetoprotein positive malignant tumors.

The clinical features of 57 autopsied cases of intrahepatic bile duct carcinoma including 28 cases of the peripheral type (cholangiocarcinoma in the narrow sense) and 29 cases of the hilar type are described in comparison with those of hepatocellular carcinoma, with a review of the literature on the clinicopathological aspects of intrahepatic bile duct carcinoma. As compared with hepatocellular carcinoma, the average age of the patients was older; the male predominance was not obvious, chronic parenchymal liver disease was infrequent in the past history, association of primary
cirrhosis
was seldom, cholestatic features were frequently the early signs and more pronounced during the course, the liver was enlarged to a lesser extent, ascites was less common, signs of portal hypertension were absent or minimal, and extrahepatic metastases were less frequent. In many respects, the hilar type resembled extrahepatic bile duct carcinoma, and the peripheral type was somewhat between it and hepatocellular carcinoma. Although the overall survival was not much different from that for hepatocellular carcinoma, early diagnosis is emphasized; this would make surgical management possible. Differential diagnosis from hepatocellular carcinoma may be possible in the majority with direct cholangiography, liver scan, celiac angiography, determination of alpha-fetoprotein and hepatitis B antigen, and blood chemistry such as SGOT, SLDH, serum bilirubin and alkaline phosphatase. Illustrative cases are given including one patient with a hilar carcinoma who survived for more than 2 years after transhepatic biliary drainage.

Certain characteristics of 328 Mozambican male Shangaans with primary hepatocellular
cancer
(PHC) have been compared with those of 163 Shangaan men with hepatomegaly from causes other than PHC and with those of 122 Black Southern African men with the same tumour but who belonged to tribes other than the Shangaan. Shangaans with PHC were significantly younger than non-Shangaans with the tumour (mean age 33,4 cf. 40,0 years; Pless than 0,001). They also had a significantly higher positivity rate of alpha-fetoprotein by immunodiffusion (71,4%) than the non-Shangaans (16%), although in other respects the tumours appeared to be similar.
Cirrhosis
of the non-tumorous part of the liver was present at necropsy in 62% of the Shangaans and in 66% of the non-Shangaans. The hepatitis B (surface) antigen (HBsAg) was detected in the serum of 60% of the Shangaans with PHC compared with only 9% of the controls. The antigen was present in 53,4% of the non-Shangaans with PHC (the difference between this fifure and that in the Shangaans was not significant). HBsAg was detected in the serum of 64% of the Shangaans with PHC and
cirrhosis
, but also in 74% of those with the tumour without associated
cirrhosis
. The possible role of the hepatitis B verus in the aetiology of PHC is considered in the light both of these findings and of the possibility that the frequency with which the tumour is associated with
cirrhosis
may be decreasing in Shangaans. Some of the dietary habits of the Shangaans with PHC were compared with those of the controls. Virtually all the patients with PHC, but also almost all the controls, ate groundnuts in large quantities from an early age, as well as cashew nuts in smaller amounts. Cycad pips, mopani leaves and pods, mopani worms and locusts were not eaten by significantly more of the Shangaans with PHC than the controls. The limitations of this type of dietary analysis are discussed.
