After a short survey on the history of the therapy of
liver cirrhosis
the importance of the prevention of
alcoholism
is emphasized. A "liver diet" is regarded as unnecessary, prednisolone is recommended for the treatment of the active
cirrhosis
. After a short description of the principles of the therapy of biliary cirrhoses, of haemochromatosis and of Wilson's hepatocerebral degeneration the use of restriction of salt, saluretics and aldosterone antagonists in the treatment of ascites is discussed in detail. After description of the conservative therapy of the haemorrhage from varices with the compression sound, intraarterial octapressin infusion and combat against consumption coagulopathy finally the prophylaxis of the hepatoportal encephalopathy with reduction of the protein intake and the restriction of the formation of toxic products of protein metabolism in the intestine by application of neomycin or lactulose, respectively, is described.

Psychometric tests were performed in 41 patients with
cirrhosis of the liver
and suspected hepatic encephalopathy and compared with EEG-examinations and clinical investigations. Marked intellectual impairment was noted frequently even when the clinical investigation was normal. This difference was mainly due to the preserved verbal ability of the patient. The etiology of the
cirrhosis
did not influence the test results. Male cirrhotic patients with and without
alcoholism
showed significantly more intellectual impairment then alcoholics without
cirrhosis
. Patients Patients with constructed porto-systemic shunts showed only slightly reduced intellectual ability compared to those without shunts.

Data on 40 patients with Staphylococcus aureus endocarditis treated with appropriate antibiotics in adequate dosage at the University of Cincinnati Medical Center hospitals between January 1961 and June 1975 were analyzed. The overall mortality was 40 per cent. The mortality was 11.1 per cent in patients under 50 years old and 63.6 per cent in patients over 50 years old (p less than 0.01). Seven patients were narcotic addicts who had no underlying disease and were under 50 years old; all survived. For patients without underlying diseases, the mortality was 0 per cent in those under 50 years old and 75 per cent in those over 50 years old. Patients who died had a greater number of major underlying diseases (pre-existing cardiac disease, diabetes mellitus,
alcoholism
and/or
cirrhosis
) than the survivors. Patients over 50 years old had significantly more major underlying diseases than patients under 50 years old (p less than 0.001). Among patients over 50 years old, those who died had more complications than the survivors while the number of underlying diseases were comparable. A group of patients treated with gentamicin during the first two to three weeks of therapy in addition to a penicillin was compared to a similar group treated with a single antibiotic. The mortality of both groups was 40 per cent.

29 348 in patients were treated from 1945-1975 on a gastroenterological ward; 1 892 (6.47%) of these patients had
cirrhosis
and 183 (0.63%) carcinoma of the liver. In the groups of patients who died, resp. came to autopsy percentages were considerably higher: 17% resp. 25.6% had
cirrhosis
, 4.9% resp. 13% had primary carcinoma of the liver. Analysis of these data shows, that the incidence of both diseases is increasing. This seems to be due in the first line to increasing
alcoholism
, and only to a lesser degree, at least in Middle Europe, to viral hepatitis. Taking into account other data published in Austria concerning absolute and relative incidence of primary carcinoma of the liver it can be stated, that this malignancy ranges among the 10 most frequent tumor diseases already.

The aim of this study has been to explore and compare the mortality of 100 female and 100 male alcoholics, admitted to a department of alcoholic diseases in 1963-69. The patients were early cases and mortality was studied during an observation period of 6-12 years. A total of 18 women and 16 men died. As compared with the general population, mortality was 5.6 and 3.0 times higher than expected for the women and men, respectively. Among the women a significant excess mortality was found for accidents, suicides, diseases of the respiratory system, and especially
cirrhosis of the liver
. Mortality among the men was significantly higher than expected due to suicides, diseases of the circulatory system, neoplasms,
chronic alcoholism
, and acute alcohol poisoning. The excess mortality from suicides found for both sexes was highest in the female group. Despite the hitherto rather small number of deaths in the two groups, the high frequency of
cirrhosis of the liver
among the women is striking.

Five hundred patients with the isomorphic pattern of the isoenzymes of serum lactate dehydrogenase (LDH) were surveyed. The isomorphic pattern of LDH isoenzymes is defined as a significant increase of total LDH with normal or low percentage of individual fractions, but with the LDH1:2 ratio less than unity. Diagnoses were, in descending order of frequency, cardiorespiratory diseases, malignancy, fracture, diseases of the central nervous system, infection/inflammation,
hepatic cirrhosis
and/or
alcoholism
, trauma without fracture, infectious mononucleosis, hypothyroidism, uremia, necrosis, pseudomononucleosis, viremia and intestinal obstruction. Incidence of increased serum activity in individuals without evidence of disease or drug explanation was 3 percent. Low PaO2 was observed in 88 percent of the 67 patients in whom it was measured.

Acute alcoholic hepatitis is an anatomical (fatty liver with sclerosing hyaline necrosis) and a clinical (hepatomegaly with a variety of symptoms of hepatic failure) entity arising out of
chronic alcoholism
, and of a typically 'pre-cirrhotic' state. Although fatal in 25% of acute cases due to failure of homeostasis, it often leaves a centrilobular scarring necrosis which in more than 60% of cases progresses to nodular
cirrhosis
. Continued
alcoholism
worsens the prognosis. Alcoholic hepatitis may be confused with acute abdominal catastrophes or with a hepatoma. The characteristic Mallory bodies found on liver biopsy are found rarely in non-alcoholic hepatitis. There is no effective treatment for this disease except reduction of alcohol intake; indeed, the disease may become self-perpetuating.

"Cures" embrace by definition a broad spectrum starting from taking waters in health resorts to hospital treatment in modern rehabilitation centers. The effectiveness of traditional cure procedures is discussed. Effectiveness of drinking cures, baths and mud packs in liver disease has not yet been proven. Controlled trials are necessary. Clinical treatment is indicated in alcoholic liver damage, viral hepatitis with a protracted course, chronic aggressive hepatitis and compensated
cirrhosis of the liver
; such treatment, however, is questionable in fatty liver and in chronic persistent hepatitis. Data concerning the effectiveness of treatment of chronic liver diseases are given. The following conclusions are drawn: patients with liver disease ought to be hospitalized when undergoing cures, indications have to be precised, collaboration of patients has to be stimulated, hospital discipline has to be tight, therapy of
alcoholism
has to include several psychosocial aspects, treatment after leaving hospital has to be improved.

Intestinal 47-calcium absorption has been studied in 34 chronic alcoholics, 17 of whom were cirrhotics (group A) and 17 non-cirrhotics (group B). These patients were compared with 44 normal subjects (group C). In group C, the 47Ca 2 h plasma % of the dose showed a significant negative correlation to the total body weight (p less than 0.001) and a positive correlation with the serum albumin (p less than 0.05). The mean intestinal absorption of 47Ca expressed as 2 h plasma % of the dose multiplied by total body weight was 131 +/- SD 52 in group A and 136 +/- SD 71 in group B. These two means are significantly low (p less than 0.001 and p less than 0.002 respectively) in relation to that found in group C (168 +/- SD 32). These results suggest that intestinal calcium absorption is diminished in
chronic alcoholism
even in the absence of
hepatic cirrhosis
.

The question wheter in Switzerland, too, there exists an association between
alcoholism
and
cirrhosis of the liver
was examind by the use of Federal Statistics. E.M. Jellinek devised an internationally applicable formula which shows a linear relationship between these two conditions. Three hypotheses were tested to validate the assumption that there are no alternate causes of
cirrhosis of the liver
. The results show: a) that in Switzerland as elsewhere, a direct relationship exists only between
alcoholism
and
cirrhosis of the liver
; b) that, in consequence, other causes of the condition have only small significance; c) that mortality from
cirrhosis
shows a distinct sex difference, with a surprisingly great increase amongst women in recent years.
