The authors have used an original method of direct radioimmunoassay of plasma CEA for testing plasma of 1704 patients affetected by various neoplastic and non-neoplastic diseases. The percentage of positive results in blood from patients with
adenocarcinoma
of the gastroenteric tract was 66.8%. The positivity in other non-neoplastic diseases was 2.7%, except for
liver cirrhosis
and other chronic hepatopathies, which showed 29.6% of positive reactions. The test should be clinically useful in the differential diagnosis of gastro-enteric carcinoma.

16 patients (14 males, 2 females, mean age: 59.2 years) underwent sonographic-guided ethanol injections as treatment for 23 hepatocellular carcinomas (HCC) complicating
cirrhosis
. All lesions were pathologically proven by sonographic-guided cytology. Tumor sizes ranged from 9 to 66 mm. Sterile 96% alcohol was injected with a 17.7 cm-long 22 gauge spinal needle at one week intervals. At each session, 8-50 ml was injected depending on the diameter of the tumor. We regarded as a "success" the negativation of the cytologies one, two and three months after the end of the treatment associated with normalization of alpha-fetoprotein levels and typical echographic and tomodensitometric changes. No serious complication was associated with the procedure. In the "Child A" group1, 6 of 7 tumors have been successfully treated, the largest measuring 66 mm. The seventh lesion is currently being treated. In the "Child B" group1 3 of 6 lesions have been successfully treated. No success has been obtained in the "Child C" group. Volumes of alcohol greater than previously reported may be useful for lesions larger than 40 mm. Percutaneous alcohol injections can be considered as an alternative to surgery even for lesions larger than 50 mm. Among 4 patients presenting with 11 liver metastases of colic and gastric

The positive rate in the HCC with a diameter of 10 cm or more was 74.1%, which was a significantly higher rate compared with 20.0% in the HCC with diameters less than 10cm. , 1 case of renal cell carcinoma, and 1 case of prostatic adenocarcinoma. The organ most commonly involved was large bowel (4 cases). Epithelial origin neoplasms comprised the vast majority (84.6%). Of the 13 cases, 2 associated malignancies existed metachronously, 4 and 5 years before HCC. The others were found at the same time as HCC. The clinical and pathological observations included age, sex, serum alpha-fetoprotein (AFP), serum hepatitis B surface antigen (HBsAg),

, gross and histologic appearance. The above presentations were similar in cases with and without second malignancy. We failed to find any factor that was possibly related to the etiology of the second neoplasm. Much more such cases are needed for further evaluation.

The asialocarbohydrate antigen YH206 is expressed on
adenocarcinoma
-associated mucin molecules which lack epitopes of CA19-9 and DU-PAN-2. To further characterize this molecule, the monoclonal antibody BM2 against the affinity-purified antigen YH206 was established. It was demonstrated by an inhibition test that antigen BM2 was an X-hapten-like structure, one of the representative oncodevelopmental antigens. Although the sensitivity of antigen BM2 in sera of stomach and pancreas cancer patients did not appear to be superior to that of antigen YH206, both antigens were complementary to each other resulting in the improvement of sensitivity. Interestingly, double-determinant enzyme immunoassays showed that antigen BM2 and YH206, both having a cryptic nature for neuraminidase, were co-expressed on the same mucin molecule in sera of patients with stomach cancer or

. These data suggest that mucin molecules in serum might be classified into several groups based on the distribution of tumor-associated epitopes.

From April 1978 to 1st October 1990, 19 patients underwent liver transplantation for primary or secondary cancer of the liver. Eleven patients were transplanted for hepatocellular carcinoma secondary to

, generally alcoholic (9 cases), hepatitis B (1 case) or secondary to haemochromatosis (1 cas). Three patients developed hepatocellular carcinoma in a normal liver, including one fibrolamellar cancer and three a proximal bile duct cancer. Lastly, two patients received a graft for secondary cancer from a colonic

and a carcinoid tumour of the right colon. The operative mortality was nil for the transplantations for cancer in a normal liver, but there were 4 deaths out of the 11 cases of cancer secondary to

. The actuarial survival of the overall series was 55% at 1 year and 31% at 2 years. The poorest survival was observed for cancers in a normal liver, with the exception of the fibrolamellar cancer in which recurrence was delayed. The longest survival was observed for cancers secondary to

Since Koprowski and coworkers discovered the CA 19-9 antigen 10 yr ago, it has become the most useful blood test in the diagnosis and management of patients with cancer of the pancreas. With an upper limit of normal of 37 U/ml, the assay's overall sensitivity is approximately 80% and its specificity is 90%. If higher cutoffs are used, the specificity rises so that, at levels greater than 1000 U/ml, the marker's specificity approaches 100%. Acute cholangitis and

The CA 19-9 is currently the "gold" standard marker for pancreatic cancer, against which other assays in this field will be judged.

We have developed a quantitative immunoassisted enzyme assay to screen for creatine kinase isoenzymes BB (CK-BB) and mitochondrial (CK-m) using commercial antibodies and reagents. Presence of CK-m activity was subsequently confirmed by electrophoretic separation of samples with elevated values. A prospective clinical trial was undertaken in 117 subjects: Normal (30),
cirrhosis
patients (30), myocardial infarction patients (30), and untreated oncology patients with metastatic malignancy (27). In 12 patients with malignancy CK-m activities were elevated; all had adenocarcinomas. No significant activity was detected in patients with other malignancies. CK-m positive tumour patients had a significantly higher mortality rate, and in two instances death was preceded by a sudden rise in CK-m activity. We suggest CK-m is a marker of
adenocarcinoma
and its presence in serum signifies increased mortality.

Two mouse monoclonal antibodies (MoAbs), KM-93 raised against human lung
adenocarcinoma
and KM-231 raised against human gastric cancer, were useful in serum diagnosis of several human cancer. KM-93 and KM-231 recognize sialyl Lex epitope and sialyl Lea epitope, respectively, expressed on glycoprotein and glycolipid. We established a new "cocktail" sandwich enzyme-linked immunosorbent assay system using the two MoAbs and the advantage of this assay system, which can simultaneously detect sialyl Lex and sialyl Lea antigens, is assessed in the present study. The new assay system is composed of a mixture of KM-93 and KM-231 as 1st antibodies and a mixture of biotinylated two MoAbs as 2nd antibodies. We evaluated the concentration of MoAbs and optimized it to gain high cancer-positivity. This assay system covered sialyl Lex positive and/or sialyl Lea-positive sera and gave a high rate of positive results in lung
