The occurrence of
liver cirrhosis
and
PHC
cases were analyzed from the autopsy material of three hospitals. In the course of this work 13,369 autopsy reports were examined. In 339 cases
liver cirrhosis
was established, the frequency of association with
PHC
was 19.4% (66 cases). Out of 66
PHC
cases 17 (25.6%) were associated with a secondary malignant tumor. This number is 10 times greater than that of other tumors of epithelial origin. It also exceeds the enlarged number of linked secondary tumors as shown in the case of certain lymphoproliferative diseases.

Forty-nine liver disease patients (7 chronic persistent hepatitis, CPH; 10 chronic active hepatitis, CAH; 13
liver cirrhosis
, LC; 9 primary hepatocellular carcinoma,

, without LC; and 10

with associated LC) and 20 controls were assessed for their serum alpha-L-fucosidase (ALF) and alpha-fetoprotein (AFP) levels and several routine liver injury parameters. Tumor diameter in those with hepatic cancer was assessed by angio-CT. Only ALF and AFP were significantly greater in patients with
PHC
and
PHC
+ LC patients as compared to patients with LC alone. At an accepted cutoff level of 500 ng/ml, the AFP level provided 43% false negative tests. On the other hand, an ALF level exceeding 740 mumol/hr/ml provided a sensitivity of 84% with a specificity of 94%. No relationship between the ALF level and Child's criteria or with any liver injury parameter was evident. Considering all individual values, the ALF, rather than the AFP, correlated with tumor size. This finding suggests the ALF level may be of value in the early detection of
PHC
as well as in the follow-up of patients treated for
PHC

Early diagnosis of
PHC
development in
cirrhosis
is sometimes difficult through common tests excluding invasive diagnostic procedures; liver biopsy as a routine periodical control during the course of the disease is not advisable and AFP monitoring as a diagnostic test is preferable. The present study shows the results of a screening for AFP levels in a series of 113 cirrhotic patients aged over 50. 11.5% of them presented increased levels of serum AFP, indicating development of
PHC
. AFP elevated values resulted in 76.5% of cases associated with a previous HBV infection, and the risk of
PHC
development resulted sixfold greater in anti-HBc positive male cirrhotic patients. In patients with elevated AFP levels the prevalence of complications of
cirrhosis
resulted up to tenfold greater than in AFP negative patients.

less than or equal to 3 cm in diameter. (2) Solitary tumors were found in 78.9% (15/19). (3) Sixteen out of nineteen (84.2%) cases had peritumor-fibrous encapsulation, and 13 cases had fibrolamellar structures in cancerous foci. (4) 14 out of 19(73.7%) cases were found to have lympholeukocyte infiltration in cancerous foci or encapsulation. (5)

The HB antigen of the hepatitis B (HB) virus, studied by counter immunoelectrophoresis shows a prevalence of 8.7% in 1,860 rural Malians and 11.3% in 764 blood donors from Bamako. Amongst 1,350 hospitalised patients, no correlation could be established between the HBs antigen chronic carriers state and other infectious diseases, malnutrition or genetic deficiencies. On the other hand, the prevalence of HBs antigen is particularly high in hepatic infections: acute and chronic hepatitis (53.5%),
