We encountered a patient with
chronic myelogenous leukemia
in basophilic crisis accompanied with histamine excess symptoms including bronchial asthma and gastric ulcer. The concentrations of histamine and
histidine decarboxylase
in leukemic cells containing granules typical for basophils were similar to those in mature basophils. His histamine excess symptoms rapidly disappeared concomitant with the reduction of blast cells after chemotherapy. We speculate that his histamine excess symptoms were induced by the leukemic cells.

Increasing number of data suggests that locally produced histamine is involved in regulation of hematopoiesis. In this study the granulocyte/macrophage (CFU-GM) colony formation by normal murine or human bone marrow cells, leukaemic colony formation (CFU-L) by a murine leukemia cell line (WEHI 3B), and colony formation by bone marrow cells from patients with
chronic myeloid leukemia
(
CML
) have been examined. We detected mRNA and protein expression of
histidine decarboxylase
(
HDC
), the only enzyme responsible for histamine synthesis both in normal bone marrow progenitor cells and in leukaemic progenitors. The significance of in situ generated histamine was shown on colony formation by inhibitory action of alphaFMH (blocking
HDC
activity, i.e. de novo histamine formation) and by N,N-diethyl-2-[4-(phenylmethyl)phenoxy]-ethanamine-HCl (DPPE) disturbing the interference of histamine with intracellular binding sites. These data provide further confirmation of the role of histamine in development and colony formation of bone marrow derived cells.

In myeloproliferative disorders (MPDs), basophils typically increase in number in the bone marrow (BM) and blood. In
chronic myeloid leukemia
(
CML
), basophilia is a diagnostic and prognostic variable. However, no reliable approach for routine detection and enumeration of basophils in BM sections is available. We applied the antibasogranulin antibody BB1 on paraffin-embedded BM sections in 21 control samples (normal BM), 45 patients with
CML
, 9 with chronic idiopathic myelofibrosis, 11 with polycythemia vera, 19 with essential thrombocythemia, and 7 with indolent systemic mastocytosis. As assessed by immunostaining of serial BM sections, BB1+ cells coexpressed myeloperoxidase,
histidine decarboxylase
, and leukosialin but did not express B- or T-cell-restricted antigens. BB1+ BM cells were found to be highly elevated in patients with
CML
compared with normal BM or other MPDs, with maximum counts found in accelerated phase CML (median, 160 cells/mm(2)). In summary, BB1 (basogranulin) is a new immunohistochemical basophil marker that should allow quantification of basophils in
CML
at diagnosis and during therapy.

Basophil numbers are typically elevated in
chronic myeloid leukemia

CML
) and increase during disease progression. Histamine is an essential mediator and marker of basophils and is highly up-regulated in
CML
. We examined the biochemical basis of histamine synthesis in
CML
cells. The
CML
-specific oncoprotein BCR/ABL was found to promote expression of
histidine decarboxylase
(
HDC
) and synthesis of histamine in Ba/F3 cells. Moreover, the BCR/ABL tyrosine kinase inhibitors imatinib (STI571) and nilotinib (AMN107) decreased histamine levels and
HDC
mRNA expression in BCR/ABL-transformed Ba/F3 cells, in the
CML
-derived basophil cell line KU812, and in primary
CML
cells. Synthesis of histamine was found to be restricted to the basophil compartment of the
CML
clone and to depend on signaling through the PI3-kinase pathway.
CML
cells also expressed histamine receptors (HRs), including HR-1, HR-2, HR-4, and histamine-binding CYP450 isoenzymes which also serve as targets of HR antagonists. The HR-1 antagonists loratadine and terfenadine, which bind to CYP450, were found to counteract proliferation of
CML
cells, whereas no growth inhibition was observed with the HR-1 antagonist fexofenadine which is not targeted or metabolized by CYP450. Moreover, DPPE, an inhibitor of histamine-binding CYP450 isoenzymes, produced growth inhibition in
CML
cells. Together, these data show that BCR/ABL promotes histamine production in
CML
cells and that certain HR-targeting drugs exert antileukemic effects on
CML
cells.
