There have been many reports in the literature on keratohypopyon in acute leukemia. We report a case of a 40-year-old-man with chronic myeloid leukemia who developed bilateral, recurring kerato-hypopyon, the first manifestation of which appeared during intrathecal chemotherapeutic treatment just after an episode of leukemic meningiosis. There was no involvement of the CNS at the time of the relapse of kerato-hypopyon.

A 56-year-old woman presented with CML in lymphoid blast crisis. Complete remission with no suggestion of CML was achieved with chemotherapy. Twenty-six months later, an isolated ocular anterior chamber relapse of acute leukemia presenting as uveitis (ocular masquerade syndrome) was diagnosed and treated with radiotherapy. Ten months later, a second ocular relapse in the same location was diagnosed and treated with radiotherapy. Two months later, a systemic relapse into second chronic phase was diagnosed and is currently being treated with alpha interferon. Only one previous case of leukemic hypopyon in CML has been reported. This is also the first case of isolated ocular blast crisis relapse in CML to be described.

The authors report a case of anterior uveitis with hypopyon that was refractory to topical corticosteroid therapy. A detailed history revealed that the patient had chronic myeloid leukemia. Cytology in the aqueous humor was consistent with relapse of chronic myeloid leukemia.

Anterior chamber leukaemic hypopyon is a rare occurrence in chronic myeloid leukaemia. We discuss two cases marked by rapid exudation inside the anterior chamber, which were subsequently diagnosed as chronic myeloid leukaemia. The hypopyon in both the cases resolved on induction of chemotherapy.
