A method based on the intravenous 133Xe injection technique has been used for measurement of bone blood flow in man. The measurements were made from the greater trochanteric region of the femur of eight healthy subjects and three patients with bone marrow or a bone disease in which bone blood flow is known to be increased. The half-times of the fast and the slow compartments of the externally recorded two-exponential bone washout curves were 4.05 +/- 0.88 min and 45.4 +/- 7.4 min (mean +/- 1 sd) in the healthy subjects, 1.46 min and 20.1 min in the patient with chronic myeloid leukemia, 2.50 min and 22.9 min in metastic bone disease and 1.93 min and 18.1 min in the patient with osteosarcoma, respectively. The corresponding flow values were 11.5 +/- 1.4 ml/100 g/min (mean +/- 1 sd) in healthy subjects and 59.8, 28.3 and 34.0 ml/100 g/min in patients with bone disorders. The precision of the method estimated from the duplicate measurements in eight healthy persons is; for the fast compartment, 6.8%; and for the slow one, 3.2%. Because of the rapid washout of xenon and the very low radiation dose the measurements are easily repeatable.

Systemic mast cells disease (SMCD) is an uncommon disorder that constitutes approximately 10% of all mastocytoses. Diagnosis requires a substantial degree of clinical suspicion, which may not be present if characteristic skin lesions of urticaria pigmentosa are not observed. Lack of well-defined histopathologic features for the disease have delayed or prevented the diagnosis of SMCD. An initial diagnosis of "myeloproliferative disorder," chronic granulocytic leukemia, or myelofibrosis is frequently made. Study of the clinical and pathologic features of 26 cases of SMCD indicated that affected patients are generally middle aged and may have had urticaria pigmentosa for many years. Gastrointestinal symptoms are common, and splenomegaly and hepatomegaly along with radiographic evidence of generalized bone disease are usually noted. Hematologic factors are highly variable. Characteristic histopathologic features of SMCD are described for bone marrow, lymph nodes, liver, and spleen. The authors consider tissue fixation and staining methods to help identify mast cell lesions.

We have developed a reverse transcriptase-polymerase chain reaction (RT-PCR) assay to identify breast carcinoma cells in bone marrow aspirates with high sensitivity and specificity. This assay relies on the detection of cytokeratin 19 (K19) RNA by nested primer PCR followed by annealing to a (32P)-labeled internal sequence probe and autoradiography. In reconstitution experiments, this assay is capable of detecting 10 fg of admixed mammary tumor RNA in 1 microgram of normal marrow RNA (a dilution of 1:10(7)). Thirty of 30 primary breast tumor specimens, 19 of 19 cytologically positive bone marrow aspirate specimens, and three of 11 aspirate negative/biopsy positive specimens showed detectable K19 transcript. This assay shows high specificity, with 50 of 52 negative control aspirates showing no detectable amplification product. False-positive amplification was noted in two of 18 aspirates obtained from patients with active chronic myelogenous leukemia. Of stage II and III postsurgical breast carcinoma patients with histologically negative bone marrows and no radiographic bone disease, 14 of 30 were K19 positive by PCR. RT-PCR analysis of K19 transcript is a highly sensitive and specific method of detecting and monitoring low-level metastatic disease in patients with primary carcinoma of the breast. The presence of K19 RNA in histologically negative bone marrows suggests that this assay may prove a powerful monitor for patients undergoing curative therapy as well as a novel prognostic indicator.

This short report describes three patients with chronic myeloid leukemia and isolated extramedullary bone disease with isolated lytic bone lesions. All these patients were in chronic phase when skeletal blast crisis took place. All the patients developed a full-blown blastic crisis within 3 months, with a rapidly fatal outcome, despite several intensive therapies.
