The effect of the activity present in the globulin fraction of the serum of patients with acute leukaemias and thrombocytopenia on the isologous platelets was investigated. This activity caused, due to increased availability of platelet factor 3, an acceleration of the endogenous clotting pathway with resulting shortening of the clotting time in the test of platelet factor 3 availability. This test was done in 44 patients with acute leukaemia and 19 with thrombocytopenia demonstrating a high-grade shortening of the clotting time as compared with that in a group of 30 healthy controls. In some cases of complete remission of acute leukaemia an effect of this activity on the autologous platelets was demonstrated. The antiplatelet activity was inhibited by rabbit serum against human IgG and Fc fragment of the heavy chain in IgG. These results suggest that the activity present in the globulin fraction of the serum may be one of the causes of thrombocytopenia in acute leukaemia.

In a patient with chronic lymphatic leukaemia severe autoimmunohaemolytic anaemia (NAIH) was observed with coexistence of cold incomplete IgM, warm incomplete IgG and warm haemolysins active against papainized erythrocytes in low pH medium. The serologic and immunochemical characteristics of autoantibodies are presented and the mechanism of their action in vitro is tentatively explained.

In 82 patients with non-Hodgkin lymphoma (NHC) the DNA synthesis by mononuclear cells from the peripheral blood was assessed by means of the index of mitoses (IM) or by pulse labelling of cells with 3H-TdR. In chronic lymphatic leukaemia (47 cases), hairy-cell leukemia (1 case), plasma-cell leukaemia (1 case) no synthesis of DNA was found in mononuclear cells. On the other hand, it was raised in most cases of lymphoplasmocytoma, centrocytoma, centroblasto-centrocytoma, centroblastoma and in lymphoblastic leukaemia or lymphoma.

After treatment of acute leukaemia in adults with cytosine arabinoside and thioguanine complete and partial remission was achieved in 50% of cases. In view of short-lasting remissions achieved maintenance treatment with other cytostatic agents was necessary. The authors called attention to the fact that in the cases with remission the counts of granulocytes and platelets increased early during cytostatic treatment and the number of blast cells fell rapidly. In cases without response to this treatment these early signs of normalization of peripheral blood morphotic elements were not observed.

In 25 cases cytochemical types of acute leukemia were determined using the classification of Loeffler. Three cytochemical methods used were: p.a.S.-glycogen content, pox-peroxidase activity, and esterase activity determinatione. At the same time selective identification of lysosomes in perypheral blood blast cells was done using vital euchrisine staining and fluorescence microscopy. A correlation was observed between the cytochemical types of acute leukaemia and the lysosomal pattern. The fluorescence method of the identification of lysosomes is suggested for diagnosis of acute leukaemia.

In 6 patients with chronic lymphatic leukaemia receiving subcutaneously purified calf thymus extract a greater frequency of the following processes was observed: 1) increase in the number of smooth-surface lymphocytes (T cells) in scanning electron microscopy, 2) increased number of lymphocytes forming rosettes with uncoated sheep erythrocytes, 3) increased number of lymphocytes showing high activity of acid phosphatase in cytochemical investigations.

It was found that supernatant from a PHA-stimulated 3-day-old culture of peripheral blood lymphocytes of healthy people had the ability of stimulating blastic transformation of peripheral blood lymphocytes of other healthy people. The same amount of supernatant caused a much lower blastic transformation of peripheral blood lymphocytes from patients with chronic lymphatic leukaemia. The tested supernatant caused no increase in the degree of blastic transformation of PHA-stimulated lymphocytes from patients with chronic lymphatic leukaemia.

The authors analysed 58 consecutive cases of acute leukaemia treated during 3.5 years. The material included 35 patients with myeloblastic leukaemia, 2 with pyomyelocytic leukaemia, 3 with monocytic leukaemia, 2 with erythroleukaemia, 7 with lymphoblastic leukaemia, and 8 with myeloblastic exacerbations of chronic myelocytic leukaemia. Most patients were treated by the VAMP schedule, in some cases L asparaginase, in other rubidomycin and cytosine arabinoside were used for obtaining remissions. Fourteen patients died within 14 days after admission to hospital. In 15 cases partial remissions, rather short-lasting, were achieved. In 4 cases the remissions were complete and in 3 of them the survival exceeds now 3 years. All patients in whom long-lasting remissions were obtained belonged to the youngest age group and 3 had lymphoblastic leukaemia. The authors suggest the need of rapid institution of treatment and systematic continuation.

In 18 patients with acute leukaemia complete remission was obtained by chemotherapy and it was tried then to prolong remissions repeating at weekly intervals BCG immunization by the method of searification. In one patient no response was obtained, in 5 cases leukaemic exacerbations occurred after withdrawal of cytostatic agent. Twelve patients were maintained in complete remission during BCG vaccinations for 3.5 to 37 months. Presently only 2 patients remain in remission from 14 to 37 months with repeated BCG vaccinations only. In the remaining cases exacerbations developed after 3 to 17 months of vaccinations. The same cytostatic agents were administered to which the patients had responded previously favourably before starting vaccinations and new remissions were obtained again lasting from 3 to 24 months. Six patients died 4 to 19 months after withdrawal of BCG vaccinations. Autopsy confirmed the diagnosis of acute leukaemia.

In a group of 172 cases of polycythemia vera treated with radioactive phosphorus acute granulocytic leukemia developed in 3 and chronic granulocytic leukemia in 6 cases. The author believes that development of acute granulocytic leukemia during this treatment may be considered with some probability as a result of the leukemia-inducing action of ionizing radiation. Transition of polycythemia vera into chronic granulocytic leukemia seems to be a natural outcome of this complex myeloproliferative syndrome in patients with survival prolonged by treatment with 32P.
