Bone scintigraphy was performed in a 69-year-old male patient with adult T-cell leukemia suffering from right lower limb pain. Numerous sites of increased uptake were seen in the skull, left clavicle, bilateral humeri, bilateral radii and right femur and tibia. Bone radiographs showed multiple osteolytic lesions, most of which corresponded to the abnormal deposits on the bone scans with 740 MBq of 99mTc-hydroxymethylene diphosphonate. This pattern is rarely reported, but bone involvement of adult T-cell leukemia is not uncommon. Bone involvement was remarkable on the appendicular skeleton when compared with common metastatic bone tumors. Bone scintigraphy may be useful in detecting bone involvement in adult T-cell leukemia.

The case is presented of a 5 year old boy who attended after an inversion injury. He had persistent pain despite treatment and was subsequently diagnosed with acute myelogenous leukaemia. It is well recognised that acute leukaemia commonly presents as non-traumatic limb pain. This case highlights the need to consider acute leukaemia in the differential diagnosis of any persisting bone or joint pain, even in the context of recent injury.

Common presenting symptoms of acute lymphoblastic leukemia in children are well known and include pallor, fatigue, and loss of appetite. Limb pain is sometimes described and can be misleading. We describe two recent cases seen in our emergency department, where vertebral fractures, a much rarer finding, were the only presenting symptoms that led to the diagnosis. One case had been thoroughly evaluated only 5 weeks prior to the diagnosis and included magnetic resonance imaging. The second patient was rapidly referred to our center with a history of acute lumbar pain. Emergency physicians caring for children must be aware of this rare type of presentation of leukemia.

Although magnetic resonance imaging (MRI) is not performed routinely in the diagnostic work-up of children with leukemia, patients may present with a normal peripheral blood cell count and osteological manifestations. For such cases, MRI may be requested in the early workup for a diagnosis. This situation illustrates that a delay in appropriate diagnosis may occur, with the classic features of the disease being uniformly absent. We present a child illustrating the salient features of acute leukemia upon MRI with limb pain and an initial normal peripheral blood cell count.
