middle lobe syndrome
were examined with fibrobronchoscope. The total rate of positive finding was 92.8% (219/236). Inflammation (167 cases), lung cancer (47 cases), tuberculosis (17 cases) accounted for most of the cases. The diagnostic rate for the three above-mentioned diseases were 97.6%, 87.2% and 58.8% respectively. Fibrobronchoscopy was also of significant diagnostic value in determining other uncommon cases such as broncholithiasis, lipoid pneumonia, lung infiltration in
leukemia
, etc. The etiology, chest X-ray (including CT) presentations of this
middle lobe syndrome
and the diagnostic value of fibrobronchoscopy were discussed. Our experience suggests that this procedure is both safe and effective in the diagnosis of patients with

leukemia
cell lines. The human and murine NF45 proteins differ only by substitution of valine by isoleucine at amino acid 142. Fluorescence in situ hybridization localized the human NF45 gene to chromosome 1q21.3, and mouse NF45 gene to chromosome 3F1. Promoter analysis of 2.5 kB of the murine NF45 gene reveals that significant activation is conferred by factors, possible including NF-Y, that bind to the CCAAT-box sequence. The function of human NF45 in regulating IL-2 gene expression was characterized in Jurkat T-cells stably transfected with plasmids directing expression of NF45 cDNA in sense or antisense orientations. NF45 sense expression increased IL-2 luciferase reporter gene activity 120-fold, and IL-2 protein expression 2-fold compared to control cells. NF45 is a highly conserved, regulated transcriptional activator, and one target gene is IL-2.

Safety assessment and designing of safer ionic liquids (ILs) are among the priorities of the chemists and toxicologists today. Computational approaches have been considered as appropriate methods for prior safety assessment of chemicals and tools to aid in structural designing. The present study is an attempt to investigate the chemical attributes of a wide variety of ILs towards their cytotoxicity in
leukemia
rat cell line IPC-81 through the development of nonlinear quantitative structure-activity relationship (QSAR) models in the light of the OECD principles for QSAR development. Here, the cascade correlation network (CCN), probabilistic neural network (PNN), and generalized regression neural networks (GRNN) QSAR models were established for the discrimination of ILs in four categories of cytotoxicity and their end-point prediction using few simple descriptors. The diversity and nonlinearity of the considered dataset were evaluated through computing the Euclidean distance and
Brock
-Dechert-Scheinkman statistics. The constructed QSAR models were validated with external test data. The predictive power of these models was established through a variety of stringent parameters recommended in QSAR literature. The classification QSARs rendered the accuracy of >86%, and the regression models yielded correlation (R(2)) of >0.90 in test data. The developed QSAR models exhibited high statistical confidence and identified the structural elements of the ILs responsible for their cytotoxicity and, hence, could be useful tools in structural designing of safer and green ILs.

Cases of adolescents in organ failure who refuse solid organ transplant are not common, but several have been discussed in the media in the United States and the United Kingdom. Using the framework developed by Buchanan and
Brock
for surrogate decision-making, I examine what role the adolescent should morally play when deciding about therapy for life-threatening conditions. I argue that the greater the efficacy of treatment, the less voice the adolescent (and the parent) should have. I then consider how refusals of highly effective transplant cases are similar to and different from refusals of other lifesaving therapies (eg, chemotherapy for
leukemia
), which is more commonly discussed in the media and medical literature. I examine whether organ scarcity and the need for lifelong immunosuppression justify differences in whether the state intervenes when an adolescent and his or her parents refuse a transplant. I argue that the state, as parens patriae, has an obligation to provide the social supports needed for a successful transplant and follow-up treatment plan, although family refusals may be permissible when the transplant is experimental or of low efficacy because of comorbidities or other factors. I conclude by discussing the need to limit media coverage of pediatric treatment refusals.
