We evaluated spinal and femoral bone mass and density utilizing dual-energy x-ray absorptiometry (DEXA) in rats in which severe hyperparathyroidism was produced by the expression of the gene for human PTH-(1-84) (hPTH). This gene was incorporated into a retroviral vector that was transfected into fibroblasts which were subsequently injected into their peritoneal cavities. Further, we examined the effect of the administration of pamidronate on bone mass and density in the presence of extremely high concentrations of hPTH. Three groups of rats were studied. Groups 1 and 2 receive the hPTH-secreting fibroblasts; group 2 subsequently received pamidronate (2.5 mg/kg IV) 18 and 27 days after receiving the fibroblasts. These animals developed levels of hPTH greater than 1.0 microgram/liter and became hypercalcemia within 20 days. These animals became
lethargic
and were significantly lower in weight than age-matched controls (group 3, p less than 0.05). After accounting for the animal weight there was a further significant decrease in bone mineral content and density (BMC and BMD) on day 29 attributable to hPTH-mediated bone loss. Treatment with pamidronate resulted in a higher BMC of the lumbar spine than in the untreated animals, with elevated concentrations of hPTH. The BMD was significantly higher at both the lumbar spine and femur in the pamidronate-treated animals (p less than 0.05). The CV of paired measurements of BMD was 2.7% at the spine and 1.5% of a femur, respectively. The BMC of the lumbar spine and femur was closely correlated with the ashed weight of the same bones (r = 0.92 and 0.85, respectively).

A 18-year-old boy was admitted to hospital in an unconsciousness state as a result of taking a large dose of several psychotropic drugs simultaneously in an attempt to commit suicide. Blood studies revealed hypoxia (55.7 mmHg) and hypercapnia (59.7 mmHg). Hypoxia (74.3 mmHg) and hypercapnia (46.7 mmHg) were still present on the fourth day after admission, and the patient was becoming
lethargic
. Reduced vascular markings in the right upper lung field on chest roentgenogram in spite of hypercapnia suggested that the persistent hypoxia was the result of a pulmonary embolism. This diagnosis was supported by a perfusion defect on 99mTc-MAA scintigram and arterial obstruction in right pulmonary angiogram. Hypercapnia is an unusual finding in pulmonary embolism, and in this case was considered due to depression of respiration by psychotropic drugs.

The medical records of 7 hypercalcemic cats with primary hyperparathyroidism were evaluated. Mean age was 12.9 years, with ages ranging from 8 to 15 years; 5 were female; 5 were Siamese, and 2 were of mixed breed. The most common clinical signs detected by owners were anorexia and
lethargy
. A cervical mass was palpable in 4 cats. Serum calcium concentrations were 11.1 to 22.8 mg/dl, with a mean of 15.8 mg/dl calculated from each cat's highest preoperative value. The serum phosphorus concentration was low in 2 cats, within reference limits in 4, and slightly high in 1 cat. The BUN concentration was greater than 60 mg/dl in 2 cats, 31 to 35 mg/dl in 2 cats, and less than 30 mg/dl in 3 cats. Abnormalities were detected in serum alanine transaminase, aspartate transaminase, and alkaline phosphatase activities from 2 or 3 cats. Parathormone (PTH) concentrations were measured in 2 cats before and after surgery. The preoperative PTH concentration was within reference limits in 1 cat and was high in 1 cat. The PTH concentrations were lower after surgery in both cats tested. A solitary parathyroid adenoma was surgically removed from 5 cats, bilateral parathyroid cystadenomas were surgically resected in 1 cat, and a parathyroid carcinoma was diagnosed at necropsy in 1 cat. None of the cats had clinical problems with hypocalcemia after surgery, although 2 cats developed hypocalcemia without tetany, one of which was controlled with oral administration of dihydrotachysterol and the other with oral administration of 1,25 dihydroxyvitamin D. PMID:Primary hyperparathyroidism in cats: seven cases (1984-1989). 181 72
We performed a 5-year review of 40 patients less than or equal to 30 days of age with viral pneumonia. Isolates included respiratory syncytial virus (55%), enteroviruses (15%), rhinoviruses (15%), adenoviruses (10%), parainfluenza virus (7.5%) and herpes simplex virus (5%). Most infants were previously healthy but had ill family members. Nine were born at less than 37 weeks of gestation. Symptoms and signs included tachypnea, decreased feeding, cough, cyanosis,
lethargy
, retractions, apnea, bradycardia, seizures and depressed consciousness. Seasonality and clinical features, but not radiographic patterns, suggested specific pathogens. Patients were moderately to severely ill. The median duration of hospitalization was 7 days; therapies administered included oxygen (90%), mechanical ventilation (45%), blood transfusions (25%) and supplemental oxygen after discharge (27%). The case fatality rate was 7.5%. Prematurity, ill appearance at presentation, lobar consolidation and adenovirus infection were risk factors for severe disease.

We report on 21 male patients who presented after 28 days of age with ornithine transcarbamylase (OTC) deficiency, which we define as late-onset OTC deficiency. These patients appeared normal at birth, but irritability, vomiting, and
lethargy
, which were often episodic, later developed. The age at presentation ranged from 2 months to 44 years. Biochemical testing revealed hyperammonemia, hyperglutaminemia, hypocitrullinemia, increased urinary orotate excretion, and decreased liver OTC activity measured in vitro, which ranged from 0% to 15% of normal. Male patients who were older at presentation had a somewhat different pattern of presenting symptoms and were more likely to die. These data illustrate the phenotypic variability of OTC deficiency. Unexplained episodes of repetitive or protracted vomiting in association with progressive alterations in behavior or neurologic findings should suggest the diagnosis of a urea cycle defect (or another symptomatic inborn error of metabolism), regardless of the age or medical history of the patient.

Two cases treating aneurysms of the distal PICA were reported, and 36 cases with 39 aneurysms in the literatures in Japan were reviewed concerning the distribution of aneurysms and their findings on CT. Case 1; a 68-year-old female suffered from sudden onset of severe headache and nausea. On admission, it was found she was
lethargic
. However, her consciousness deteriorated down to semicoma with tetraparetic condition soon after. CT revealed subarachnoid hemorrhage in the basal, quadrigeminal and supravermian cisterns and blood clots in the entire ventricle. Cerebral angiography demonstrated an aneurysm located at the distal segment of the left PICA. She was initially treated conservatively because of being in Hunt and Kosnik Grade 5, and then, 3 weeks after onset, suboccipital craniectomy was performed and the aneurysm was clipped successfully. Case 2; a 60-year-old, female, suddenly experienced severe suboccipitalgia and vomiting. CT revealed subarachnoid hemorrhage in the entire subarachnoid space and intraventricular hemorrhages in the 4th, 3rd and lateral ventricles. Subsequently cerebral angiography was performed and left VAG demonstrated an aneurysm at the left A2-A3 junction. She underwent bifrontal craniotomy and the aneurysm was clipped via the interhemispheric approach. Her postoperative course was uneventful. Postoperative left CAG showed successful clipping of the aneurysm. However, left VAG suggested an aneurysm-like shadow in the right PICA. Right BAG carried out one week later demonstrated an aneurysm at the distal segment of the right PICA. This study quantifies the relationship between subjective complaints of symptoms and certain work activities and exposures in a plant where herbicide is packed. Data relating to symptoms were collected from 27 subjects (20 men and 7 women) on a daily basis with the use of a questionnaire. In addition, data were collected relating to work activity and exposure to the stenching agent added to the herbicide, atmospheric levels of which were measured with personal monitoring, on a daily basis. Although associations were found between the occurrence of
lethargy
and certain factors of job history, there was no significant association with exposure to the stenching agent as measured by personal monitoring during the study.

The effects of feeding high levels of Mg were evaluated in a 130-d study with 24 steers. Six steers were allotted to each of four diets supplemented with MgO to attain .3, 1.4, 2.5 or 4.7% Mg, DM basis. Chromic oxide was used as digesta marker; fecal grab samples were collected once daily during nine consecutive 10-d periods. Steers fed 2.5 and 4.7% Mg refused some feed during the study, so their respective dietary Mg intakes were 2.4 and 3.7% (DM basis). Severe diarrhea and a
lethargic
appearance were observed in steers fed the two higher Mg levels. Fecal DM and apparent DM digestibility decreased linearly (P less than .01) with increased dietary Mg. Apparent Mg absorption (g/d) increased linearly (P les than .01) and apparent Ca and P absorption (g/d) decreased linearly (P less than .01) with addition of Mg to the diet. Increasing dietary Mg linearly elevated blood serum and erythrocyte Mg (P less than .01; P less than .05, respectively) and serum inorganic P (P less than .05) and linearly decreased serum Ca (P less than .01). Magnesium concentration in liver, kidney, skeletal muscle and rib-bone and P in skeletal muscle all increased linearly (P less than .05) with dietary Mg. Increasing dietary Mg in the steers caused a progressive degeneration of the stratified squamous epithelium of rumen papillae. A progressively more serious Mg toxicosis condition developed over time in steers fed diets containing 2.4 and 4.7% Mg.

Children with attention deficit disorder with hyperactivity (ADD+H; N = 48) were compared with those without hyperactivity (ADD-H; N = 42), as well as with learning disabled and control children, on an extensive battery of interviews, behavior ratings, tests, and direct observations. ADD+H children had more externalizing and internalizing symptoms by parent and teacher report, were more off task during vigilance testing, and had more substance abuse, ADD+H, and aggression among their relatives than did the other groups. ADD-H children were more day-dreamy and
lethargic
by teacher report, more impaired in perceptual-motor speed, and had more anxiety disorders among their relatives than did ADD+H children. Results indicate that these 2 types of ADD may be separate, distinct childhood disorders rather than subtypes of a common attention deficit.
