Susan Pearson BSc, a freelance journalist and communications consultant specialising in medicine and the environment (see also HEJ - April 2013), reports on discussions, at a recent educational seminar, on a pilot project undertaken by the Environmental Microbiology Unit at the Brighton and Sussex University Hospitals (BSUH) NHS Trust, which compared the effectiveness and accuracy of conventional 'culture' testing for Legionella in water systems, with a new, 'less labour-intensive', DNA-based testing system that can produce results 'in a matter of hours'.

The Legionella Approved Code of Practice and guidance (ACoP) has recently undergone a review by the Health and Safety Executive. Steve Mount BSc (hons) CBiol MIBiol MWMSoc, an independent specialist consultant in water hygiene and safety, reports on the first conference (held in Birmingham) to discuss these changes, entitled 'Legionella management: The revised ACoP (L8); what impact will the changes have on you?'. The event was hosted by Immerse Training, and delegates' views on various issues, including the non-definitive terminology, and their general concerns regarding Legionella management and competence, were recorded, along with the impact that the proposed changes will have on people who manage water systems.

Since its refurbishment in 2008/2009, the 'old' Saint Mary's Hospital has predominantly been used from 9.00 am to 5.00 pm weekdays (its Sexual Assault Referral Centre, or SARC, facilities are sometimes open evenings and weekends), presenting a significant water stagnation risk. During the trial of the Kemper KHS water flow management system, none of the 125 water samples analysed for Legionella and coliforms were positive, while just 11 of the 116 tested for Pseudomonoas spp exhibited the bacteria, only two exceeding Department of Health-set limits.

Although temperature control has been the UK's longest-serving means of controlling the growth and proliferation of Legionella in hot and cold water systems, there are other factors, including major rises in energy costs, that warrant the use of biocides--including in the healthcare sector. In 2000, the HSE's new 'L8' guidelines took this into account, giving equal weight to both temperature reg~mes and biocides, such as chlorine dioxide, as control methods. A 'high-level overview' of some of the emerging rapid microbiology technologies designed to help healthcare engineering and infection control teams working in hospitals and other healthcare facilities more rapidly identify potentially hazardous levels of waterborne microorganisms in their water systems, enabling them to take prompt remedial action, and a look at the some of the 'pros and cons' of such testing techniques, was given by Nalco technical director, Howard Barnes, the vice-chair of the Legionella Control Association (LCA), at a recent LCA open day. Speaking at a Legionella Control Association Open Day on 9 October last year in Tamworth, Mike Quest, an LCA director and Committee Member who is an independent water hygiene and safety consultant and an NHS Authorising Engineer, presented his standpoint on effective risk assessment and monitoring of complex hot and cold water systems. He also focused on some of the challenges for engineering and estates teams in maintaining water temperatures within 'safe limits' in modern buildings, with reference to the complications he had seen in a hospital project he has recently been working on.

The estates team at St George's Hospital in south London say the deployment of a web-based software system which verifies that 'responsible' staff across the site have regularly flushed low-use water outlets to maintain flow, and thus prevent potentially dangerous waterborne bacteria building up in pipework, has 'very substantially' increased compliance with approved flushing practice--as set out in the HSE's L8 Approved Code of Practice on controlling Legionella bacteria in water systems, and the new HTM 04-01, Safe Water in Healthcare Premises. As HEJ editor, Jonathan Baillie, reports, the hospital's use of Digital Missives' L8guard software system has also saved considerable staff time, eliminated the need to manually input data from thousands of paper flushing return forms, and enabled the Estates team to easily identify departments not undertaking regular flushing.

'Efficient productivity through innovation' was the theme of the first ever Hospital Innovations conference and exhibition at Olympia, London in late April. At the two-day event--supported by organisations including IHEEM, the Legionella Control Association, the Water Management Society, the BRE, and a sizeable number of English NHS Trusts--the Day Two keynote address by Lord Carter very much reflected this theme. Following his address at Healthcare Estates 2015, the Labour Peer focused further in London on his team's recent review of the 'productivity and efficiency' of English NHS Trusts, and explained how the initiative would progress in coming months. One of his key conclusions was that while the NHS consistently rates as one of the world's most efficient public health systems, innovation and good practice are rarely sufficiently shared or widely replicated service-wide, resulting in a considerable 'gap' between the best and worst-performing Trusts. Managing outbreaks of Legionella and Pseudomonas aeruginosa in hospital washrooms remains a serious issue, with individual infection prevention and control (IPC) teams tasked with deciding which strategies and products are most suitable for their hospital. While determining the most effective infection control can be challenging even for experienced IPC personnel, commercial tap manufacturers face the equally demanding task of designing sufficiently versatile and effective products to meet the requirements of a wide range of healthcare facilities.
