Legionella
often causes systemic manifestations. The clinical spectrum now includes cardiac legionellosis. The first case of
myocarditis
was reported by Gross in 1981. To date few additional cases have been described. Myocardial involvement might be more frequent than supposed in legionnaires' disease.

Myocarditis
, pericarditis and endocarditis are rare manifestations of
Legionella pneumophila infection
. We describe a case of
myocarditis
complicated with a potentially fatal arrhythmia: "torsades de pointes".

To our knowledge,
Legionella
pericarditis has been reported only four times in the literature and only once without pneumonia. We report the second case of nonpneumonic
Legionella
pericarditis in a man who eventually required pericardiectomy for constriction. Since
Legionella
endocarditis,
myocarditis
, and pericarditis have all been documented by culture or direct fluorescent antibody staining, it is important to be aware of the possibility of cardiac legionellosis.

Legionnaires' disease
, although primarily seen as a pulmonary affection, may also manifest systemic involvement. Evidence is given to demonstrate bacteremia and focal
myocarditis
in a patient with cirrhosis and pneumonia.

A case of
Legionnaires' disease
is described in which the characteristic features of multilobar pneumonia, rhabdomyolysis, renal failure, hepatic and CNS involvement are accompanied by the previously undescribed complication of
myocarditis
. Clinical and laboratory findings of myocardial involvement included overt heart failure, a new gallop, an abnormal ECG, elevated myocardial specific enzymes and an abnormal thallium scan. All of these abnormalities resolved completely after recovery.

Cardiac involvement during
Legionnaires' disease
has been rarely described; few cases of
myocarditis
, pericarditis and endocarditis were reported. We describe a case of
myocarditis
associated to pneumonia, with high antibody title suggesting a disease due to
Legionella
pneumophila. The patient had severe myocardial damage, with overt heart failure and important ECG and Doppler-echocardiographic abnormalities, without associated multiorgan involvement.

Legionella infection can manifest itself in many clinical forms, most commonly as pneumonia, but rarely in the form of myocardial involvement.
Legionella
with myocardial involvement independent of pneumonia is almost never seen in the adult population and therefore is cited only a handful of times in the medical literature. When reported,
Legionella
carditis itself typically occurs as an isolated pericarditis with effusion. Cases of isolated
Legionella
with myocardial involvement, but without associated pneumonia, have been reported among children. To our knowledge, there are no reported cases of
Legionella
myocarditis
and pericarditis presenting concurrently with or without pneumonia, in either an adult or a pediatric population. Herein, we report a rare manifestation of
Legionella
pneumophila-induced perimyocarditis (strongly suspected, if not incontrovertibly proved) in an adult, in the absence of pneumonia.

We herein describe the successful treatment of a patient with possible
Legionella
pneumophila serogroup 6 infection complicated by pneumonia and
myocarditis
. A 32-year-old man presented with a five-day history of cough, dyspnea and chest pain. Chest radiography revealed patchy opacities in both lungs suggestive of bilateral pneumonia, and a urinary antigen test for
Legionella
pneumophila was positive. After admission, the patient developed congestive heart failure due to pathologically confirmed
myocarditis
. He was successfully treated with minocycline, macrolide, steroids and noninvasive positive-pressure ventilation (NPPV). He eventually recovered with a normalized cardiac function. L. pneumophila serogroup 6 was isolated from the bathwater in the patient's home.

Myocarditis
is defined as inflammation of the myocardium. The clinical manifestations of
myocarditis
vary from flu-like symptoms to fatal fulminant forms. We report the case of a 39-year-old woman with a diagnosis of cardiogenic shock caused by fulminant
myocarditis
. Extracorporeal membrane oxygenation was used as a bridge to recovery. Etiological study revealed
Legionella
pneumophila serogroup 1 infection. Recovery of biventricular function was seen after treatment with azithromycin.

Legionnaires' disease
is the designation for pneumonia caused by the
Legionella
species. Among the rare extrapulmonary manifestations, cardiac involvement is most prevalent, in the forms of
myocarditis
, pericarditis, postcardiotomy syndrome, and prosthetic valve endocarditis. Mechanical circulatory support has proved to be a safe and effective bridge to myocardial recovery in patients with acute fulminant
myocarditis
; however, to our knowledge, this support has not been used in infectious
myocarditis
specifically related to Legionellosis. We describe a case of
Legionella
myocarditis
associated with acute left ventricular dysfunction and repolarization abnormalities in a 48-year-old man. The patient fully recovered after left ventricular unloading with use of a TandemHeart percutaneous ventricular assist device. In addition, we review the English-language medical literature on
Legionella
myocarditis
and focus on cardiac outcomes.
