Legionella pneumonia
is an increasingly frequently reported complication in immunocompromised patients, particularly patients with
hairy cell leukemia
(
HCL
) in active phase. The most important predisposing factor seems to be the quantitative and qualitative defect of the monocytic-macrophagic system characteristic of
HCL
. We report a case of severe
Legionella pneumophila infection
with multisystem involvement in a patient with
HCL
in stable partial remission obtained after therapy with interferon. In our patient recovery of a normal monocyte count did not protect against a legionella infection, indicating that this pathogen should always be sought in
HCL
patients even those in clinical and hematologic remission. Early diagnosis and appropriate treatment may reduce the mortality of this serious complication.

A case of community-acquired pneumonia caused by
Legionella
dumoffii in a patient with
hairy cell leukemia
is described. Diagnosis was confirmed by isolation by culture of sputum and broncho-alveolar lavage specimens, positive direct fluorescent antibody stains, and antibody seroconversion from 1:16 (acute) to 1:4096 (six months). The blue white autofluorescence of the L. dumoffii colonies when viewed under ultraviolet light was particularly useful in preliminary identification. The patient recovered from his pneumonia after administration of erythromycin and rifampin.
Legionella

Hairy cell leukemia

A 63-year-old man was admitted to the respiratory intensive care unit for pneumonia. Fifteen years earlier,
hairy cell leukemia
had been diagnosed and the patient underwent splenectomy. A clinical suspicion of legionellosis, later confirmed by both serology and isolation of the microorganism, prompted initiation of high dose erythromycin intravenously. The patient steadily deteriorated and passed away eight days later. This is the first reported case of
hairy cell leukemia
in which the diagnosis of
Legionella
longbeachae sero-group 1 infection was based on both serology and isolation.

Both sporadic cases and outbreaks of
legionnaire's disease
have been reported. To date, no outbreaks have occurred but several case reports have been published in Switzerland. The newly organized surveillance system of notifiable diseases, introduced in 1987, makes it possible for the first time to analyze reported sporadic cases more precisely. In 1988, the laboratories reported a total of 32 cases with cultural or serologic proof of legionellosis. In 75% of cases patients were aged over 40 years, 78% occurred among males. The majority of them were known to be smokers. In 9 cases an underlying predisposing condition was known:
hairy cell leukemia
(3 cases), immune hemolytic anemia (1), type 2 diabetes (2), chronic lung disease (1), heart failure (1). The case fatality was 9%. A possible source of exposure, such as air-conditioned rooms or evaporative condensers, was reported in 4 cases.

. The patient presented with community-acquired pneumonia resistant to common antibiotics. Serology provided the diagnosis and the patient improved with adequate treatment. Early diagnosis and appropriate treatment may reduce the mortality of this serious complication.
