Twenty patients with keratoconjunctivitis sicca used three different viscoelastic tear formulations and a polyvinyl alcohol artificial tear for two weeks each. Each formulation was used once every two waking hours in a controlled double-masked study. Eighteen patients reported marked improvement over the course of the study, in terms of the severity of itching, burning, and foreign body sensation. Corneal staining and mucous strand formation were also reduced in patients with these manifestations. No formulation was preferred by a majority of patients or proved superior in treating signs of keratoconjunctivitis sicca. However, patients with low Schirmer test scores uniformly preferred a solution containing chondroitin sulfate, while patients with moderate Schirmer test scores tended to prefer a solution of polyvinyl alcohol or hyaluronic acid.

Primary sclerosing cholangitis and primary biliary cirrhosis are chronic cholestatic syndromes that may be difficult to differentiate clinically. Destructive cholangitis occurs in both diseases and leads to similar clinical and biochemical abnormalities. Therefore, we compared the clinical, biochemical, immunologic, radiologic, and hepatic histologic features of these syndromes in two large groups of patients prospectively selected by predefined criteria. Primary biliary cirrhosis (n = 258) occurred predominantly in middle-aged women who were usually symptomatic with fatigue and pruritus, commonly had keratoconjunctivitis sicca, and often were hyperpigmented. Tests for antimitochondrial antibodies were always positive, usually in very high titer. Although the extrahepatic bile ducts were normal radiographically, smooth tapering and narrowing of the intrahepatic bile ducts was occasionally noted. Hepatic histology was diagnostic when a florid duct lesion was present. In contrast, primary sclerosing cholangitis (n = 60) occurred primarily in young men who were usually symptomatic with fatigue and pruritus and frequently had chronic ulcerative colitis. Tests for antimitochondrial antibodies were nearly always negative and cholangiography demonstrated abnormalities of the extrahepatic and intrahepatic bile ducts in all cases. Although hepatic histology was often compatible with the diagnosis, it was usually not diagnostic, and considerable overlap existed with the abnormalities seen in primary biliary cirrhosis. Likewise, biochemical tests of copper metabolism were similar in both syndromes. These results call attention to the differences and similarities in the clinicopathologic features of these two cholestatic syndromes and provide a basis for a rational diagnostic strategy.

This trial was a randomized, double-masked, crossover study during which patients with keratoconjunctivitis sicca underwent 6 weeks of treatment with either cyclosporine 1% ophthalmic ointment or placebo followed by 6 weeks of the alternative treatment. Washout periods using only unpreserved artificial tears preceded both treatment cycles. Twenty-five patients completed the first treatment period, but only eight met entry criteria for period II. Cyclosporine ointment was associated with initial mild to moderate redness, itching, and burning that returned to baseline levels within 1-2 weeks. Rose Bengal results and results of four subjective (patient diary) efficacy parameters favored cyclosporine: foreign body sensation, overall symptoms, hours of symptom control per day, and overall effectiveness. No systemic adverse events or laboratory abnormalities occurred. We conclude that (a) the crossover design is inappropriate for studying this disease; (b) mild to moderate itching, redness, and burning occur initially with cyclosporine administration, although tolerance quickly develops; (c) cyclosporine appears to benefit the ocular surface in keratoconjunctivitis sicca; and (d) further trials in this syndrome are warranted.

The medical records of 141 patients with dermatochalasis seen during the 32-month period of January 1989 through August 1991 were reviewed. Patients were classified on the basis of symptoms and mode of treatment and were examined for effectiveness of blepharoplasty in ameliorating these symptoms. Seventy-three patients (51.8%) had symptoms similar to those found in keratoconjunctivitis sicca, including mattering, burning, itching, redness, epiphora, foreign-body sensation, and photophobia. Of these 73 patients, 38 (52.1%) underwent upper eyelid blepharoplasty. Subjective improvement in symptoms was achieved in 33 of these patients (86.8%) postoperatively. Upper eyelid blepharoplasty may represent an effective component in the treatment of patients with dermatochalasis and dry-eye symptoms.

Multiple cutaneous xanthomas, associated with fasting hyperlipidaemia, are described in a 9-month-old domestic long-haired cat. A severely pruritic, papular, and crusting dermatitis affecting the head and neck, initially diagnosed as lesions of the eosinophilic granuloma complex, progressively developed on the head and pinnae. Pruritus was controlled with administration of prednisolone and chlorambucil. Repeat histological examination confirmed the diagnosis of cutaneous xanthoma and concurrent mild demodicosis. Marked fasting hypercholesterolaemia, hypertriglyceridaemia and transient hyperglycaemia were subsequently confirmed. Treatment for hyperlipidaemia and xanthomas with a low-fat diet (Hill's Feline r/d) and the previously unreported treatment for feline demodicosis of daily oral milbemycin were commenced. Multiple pink, alopecic plaques and papules gradually regressed, however pruritus recurred if immunosuppressive treatment was reduced, and well-demarcated areas of alopecia developed on the head, limbs and trunk, despite negative skin scrapings for demodex mites. Fungal culture of hair samples yielded Microsporum canis. All cutaneous lesions resolved with the addition of griseofulvin to the treatment regimen. Concurrent corneal ulceration and keratoconjunctivitis sicca ultimately resolved with treatment, including topical cyclosporin. Diabetes mellitus developed 6 months after resolution of skin lesions. No cutaneous or ocular abnormalities were present 6 months later with continued low-fat diet and insulin administration, although transient recurrence of papules and pruritus occurred after inadvertent access to a fatty meal. An underlying primary hyperlipidaemia was suspected, causing pruritic xanthomas. This may represent the first report of concurrent cutaneous xanthomas, demodicosis and dermatophytosis in a cat.

Tears-which contain fibronectin, growth facors, and vitamins that support the migration, proliferation, and differentiation of the conjunctival and corneal epithelium-nourish, protect, and refresh the eye. An inadequate tear film produces a variety of symptoms (ocular irritation, itching, burning, eye strain or fatigue, photophobia, a foreign-body sensation, a sharp stabbing pain, blurred vision) that wax and wane in intensity and are characteristic of disorders such as dry eye (keratoconjunctivitis sicca) and neurotrophic keratitis. In patients for whom commercially manufactured eye drops fail to provide relief from the symptoms of those conditions, autologous serum eye drops (plasma eye drops, serum eye drops) have proven very effective. In this report, we examine the present case studies that reveal the efects of autologous serum eye drops on the signs and symptoms of both dry eye and neurotrophic keratitis. A procedure for preparing autologous eye drops is included.
