A dog that had a chronic history of keratoconjunctivitis sicca and was being treated topically with antibiotics and corticosteroids was examined for evaluation of a melting corneal ulcer. Cytologic examination and cultures revealed the pathogen to be Pseudallescheria boydii, a saprophytic fungus. The eye was surgically removed and evaluated histologically. Fungal keratitis is rarely reported in dogs, but needs to be considered in refractory infections or in dogs treated with antibiotics or corticosteroids.

Sterile corneal ulceration is a serious complication in patients with keratoconjunctivitis sicca. The records of 134 patients, 19 males and 115 females, who presented with dry eyes in the Ophthalmologic Clinic were reviewed. Over a period of 6 years, 33 eyes of 23 (17%) patients developed a sterile corneal ulcer. The etiologies of dry eyes in these patients were: Primary Sjogren's syndrome: 10 cases, rheumatoid arthritis: 5 cases, ocular pemphigoid 6 cases, atopy: 1 case, local irradiation: 1 case. Patient's age and sex were not significantly associated with ulcer development (p greater than 0.05). The presence of a major underlying disease was the major contributing factor. Appropriate local treatment and patient compliance were also contributing factors. Blepharitis was found in 90% of patients.

The authors found the tear lactoferrin content in normal Chinese adults to be 1.46 +/- 0.32 mg/ml (mean +/- 1 SD), and the mean counting rate of its unsaturated binding power of iron 63,660 +/- 17,010/min/ml (mean +/- 1 SD), with very significant positive correlation between the two, irrespective of sex or eye distinction. Tear lactoferrin began to decrease after 40 years of age, and remarkably so after 70. The authors deemed the measurement of tear lactoferrin a reliable and sensitive marker for the diagnosis of keratoconjunctivitis sicca. In patients with bacterial corneal ulcer and acute catarrhal conjunctivitis, the lactoferrin content in tears was normal, in chronic catarrhal conjunctivitis, the unsaturated binding power of iron was decreased.

Tears were collected from patients with familial amyloidotic polyneuropathy (FAP), orbital tumors, sarcoidosis, trigeminal and facial nerve palsy and corneal ulcer. These tears were analyzed by crossed immunoelectrophoresis. Two cases of FAP with deficiency of lacrimation and keratoconjunctivitis sicca showed normal tear protein patterns. In these cases, the deficiency of lacrimation may have been due chiefly to disturbance of the peripheral parasympathetic nerve innervating the lacrimal gland. Five other patients with FAP showed from slight to severe decrease in the tear-specific proteins. One of two cases with lacrimal gland tumor showed a decrease in the tear-specific proteins before removal of the tumor. After the surgery two patients showed significant decrease or disappearance of the tear-specific proteins. Three cases with other orbital tumors revealed no alterations in production of tear proteins. The case of sarcoidosis with keratoconjunctivitis sicca showed a decrease in the tear-specific proteins. Lacrimal gland involvement in sarcoidosis was confirmed by the tear protein analysis. The patient with facial and trigeminal nerve palsy showed a decrease in the tear-specific proteins. This was thought to be due to atrophy of the main lacrimal gland caused by functional disturbance of the lacrimal nerve. The analysis of tear proteins is useful in the diagnosis of lacrimal gland tumor disturbing the gland function and also of other diseases involving the lacrimal gland.

A review of 100 patients who underwent penetrating keratoplasty revealed 47 who required therapeutic soft contact lenses in the early postoperative period. Twelve corneal ulcer-infiltrates, 11 of which were culture positive, occurred during soft contact lens wear (23% incidence of this complication in contact lens fitted eyes). The most common offending organism was coagulase-negative staphylococcus. The only statistically significant risk factor for infection if a lens was used was the presence of a persistent epithelial defect (p = 0.03). Factors which could not be statistically correlated with corneal ulcer-infiltrate included keratoconjunctivitis sicca, the type of contact lens, the method of donor cornea preservation, lens hygiene, antibiotic and steroid usage, the presence of blepharitis, preoperative bacterial keratitis, and the history of a previously failed penetrating keratoplasty.

The association of visual disturbances and rheumatic disease has been known for centuries. This review provides a synopsis of the ocular conditions that are associated with inflammatory rheumatic disease. The major ophthalmic manifestations of the rheumatic diseases include keratoconjunctivitis sicca, ulcerative keratitis, scleritis, uveitis, retinal vascular disease, and neuro-ophthalmic lesions. Each of these ocular conditions is most characteristically associated with a few, but not all, of the rheumatic disorders. Scleritis, for example, is most often seen with rheumatoid arthritis or with vasculitis. Acute anterior uveitis is most often seen with the seronegative spondylarthropathies. Retinal vascular and neuro-ophthalmic lesions are seen with disorders having either a vaso-occlusive component, such as systemic lupus erythematosus, or with one of the vasculitides. Important considerations for a successful collaboration between ophthalmologists and physicians/rheumatologists are discussed.

A 19-year-old Shetland pony presented with unilateral ocular discomfort and abnormal ocular appearance. Keratoconjunctivitis sicca, ulcerative keratitis and brown discoloration of the corneal stroma were identified on ophthalmic examination. The etiology of keratoconjunctivitis sicca was not determined in this case. For practical and financial reasons, the owners requested enucleation of the affected eye. Histopathologic examination revealed extensive loss of corneal epithelium overlying a zone of hypereosinophilic, degenerate, and necrotic corneal stroma. This well-circumscribed region of corneal stromal sequestration was surrounded by stromal vascularization, and an intense inflammatory, predominantly polymorphonuclear, cellular infiltrate. The clinical and histopathologic features of this case were considered remarkably similar to those observed in feline corneal stromal sequestration.

In the case of external ocular diseases such as conjunctivitis, keratoconjunctivitis sicca (KCS) and superficial corneal ulcers, topical administration of eyedrops containing an antibacterial agent is often prescribed. Numerous daily instillations of eyedrops over several days are required for successful treatment, often leading to bad compliance. In addition, the reflex lachrymation following instillation promotes rapid elimination of the drug from the corneal surface. To overcome the disadvantage of repeated instillations, a soluble bioadhesive ophthalmic drug insert (BODI) to be placed in the lower cul de sac of the eye was developed. The clinical efficacy, after deposition of one insert and a classical eyedrop treatment (Tiacil), Virbac Laboratories), was investigated in dogs presenting conjunctivitis, superficial corneal ulcer or keratoconjunctivitis sicca (KCS). Similar total clinical recovery results were obtained after 3 and 7 days for both treatments. BODI can therefore advantageously be prescribed for the treatment of external ophthalmic diseases, by reducing the treatment to a single application and therefore improving compliance compared to classical eyedrop treatment.

Systemic scleroderma is a progressive multi-system connective tissue disease. Ocular involvement includes keratoconjunctivitis sicca, progressive shallowing of conjunctival fornices, peripheral ulcerative keratitis and eyelid tightness. No association has been reported between scleroderma and pellucid marginal degeneration, which is a rare bilateral corneal ectasia. Pellucid marginal degeneration is characterised by non-inflammatory and progressive peripheral corneal thinning inferiorly, often with high against-the-rule astigmatism. We report a case of a 55-year-old woman with systemic scleroderma who presented with rapidly progressing against-the-rule astigmatism. The differential diagnosis of peripheral corneal thinning and the challenge of the surgical management of pellucid marginal degeneration are briefly discussed.

Five cases of ocular rosacea (one male and four females) are reported in this paper. Two of the patients were with keratoconjunctivitis sicca, one with conjunctivitis chronica and blepharitis, one with conjunctivitis chronica and meibomitis, and one with reccurent corneal erosions with meibomitis and chordeloum. In four patients ocular symptoms preceded the occurence of skin lesions. The treatment with oral tetracyclines significantly improved the state of ocular rosacea in four patients, while in one case the changes of the anterior eye segment progressed in more severe state of ulcerative keratitis. It is considered that in almost 20% of the patients with rosacea ocular lesions may precede the skin changes, representing a diagnostic problem. Thus, in those cases multidisciplinary approach is suggested. Such approach is particularly important because of the decrease of morbidity and the prevention of the onset of the eye complications such as drastic worsening of visual acuity, i.e., the blindness.
