During a one-year survey 283 corneal ulcers from 274 patients were seen at St John's Eye Unit of Baragwanath Hospital. Central bacterial ulcers constituted the largest problem, and the commonest isolate in this group was Streptococcus pneumoniae. Mycotic and dendritic keratitis were relatively uncommon, while marginal catarrhal ulceration secondary to chronic staphylococcal lid disease was frequently seen. The microbiology of the various ulcers is described, and the placing of organisms into classes is stressed in determining significance of isolates. Many of the patients were male Africans who were either manual labourers or unemployed. Half the patients had used topical antibiotics before presentation.

A 41-year-old white man presented with bilateral white infiltration of the cornea from limbus to limbus. Extensive examination revealed no manifestations of disorders of lipid metabolism and the patient gave no history of previous ocular disease. VDRL and other serological tests were negative. The corneal button removed by penetrating keratoplasty from the right eye was studied by light microscopy, histochemistry and electron microscopy. The light microscopic appearance was consistent with lipoidal degeneration of the cornea associated with stromal vascularization and chronic keratitis, more likely a secondary lipoidal degeneration. Cholesterol clefts were seen in all levels of the stroma focally surrounded by a foreign body giant cell reaction. Lipid vacuoles were observed in extracellular and intracellular locations within histiocytes and fibroblasts in vascularized and inflammed areas. Lipid stains disclosed cholesterol crystals, neutral fats, and phospholipids.

The clinical, histopathological, ultrastructural and microanalytic features of a case of iatrogenic ocular argyrosis are reported. The case, in a 63-year-old woman with long-standing herpetic keratitis, was unusual in that it occurred after apparently only one treatment with eyedrops consisting of a 1% solution of "mild silver protein" (Argyrol). Another unusual feature was that the silver deposited in the cornea was mainly conjugated to sulfur; selenium, which commonly demonstrates a high affinity for silver in tissues, was detectable in only trace amounts.

Mycotic keratoconjuntivitis caused by Acremonium kiliense in a German Shepherd bitch was diagnosed with the aid of laboratory tests. The dog presented with photophobia, tearing, corneal edema and reduction of the visual capacity. A thick white layer partially covered the right eye. The left eye showed irritation and small brown stains which were diagnosed as pigmentary keratitis. The initial treatment consisted of 2% yellow mercury oxide. Natamycin was used as final treatment. Seven days later, the natural brightness of the eye as well as the visual capacity were restored.

We determined the anti-inflammatory prophylactic effect of topical cyclosporine in a rabbit model of herpetic stromal keratitis. In a first study, we established that presensitization of rabbits prior to corneal intrastromal injection of herpes simplex virus produced a severe stromal keratitis beginning on day 7 after the intrastromal challenge. Twenty-four presensitized rabbits were thereafter divided into three groups for topical treatment five times a day: (1) 1% cyclosporine in a vehicle solution, started on day 4 after intrastromal challenge, plus 1% trifluridine started on day 7 after challenge, (2) the vehicle solution for cyclosporine, started on day 4, plus 1% trifluridine started on day 7, and (3) the vehicle solution for cyclosporine alone, started on day 4. The severity of stromal disease was significantly decreased in eyes treated with combined cyclosporine-trifluridine, whereas trifluridine alone had no effect on the stromal disease. Epithelial disease did not seem to be worse in eyes treated with combined cyclosporine-trifluridine; the incidence and duration of positive cultures, however, were increased in this group.

Clinical trials of the antiviral action of interferon have shown an effect on the replication of several viruses including varicella zoster, herpes simplex, cytomegalovirus and hepatitis B. These studies indicate that administration early in the course of infection, or in some clinical circumstances, prophylactic administration, is likely to result in viral inhibition. The studies of interferon efficacy in topical application, as in prevention of recurrent herpes simplex keratitis, have shown limited efficacy except with very high doses. These studies are being pursued with more concentrated preparations of interferon. The evaluation of interferon in human malignancy is just beginning, but some encouraging results have been obtained in open trials of the drug in patients with non-Hodgkin's lymphoma, melanoma, osteogenic sarcoma, and other diseases. With newer methods for the production of interferon, it may be possible to evaluate its antiviral and anti-tumor effects in carefully controlled studies with larger numbers of subjects.

Inflammation often is accompanied by neovascularization. This is especially evident in the normally avascular cornea, where an angiogenic response occurs with keratitis or during graft rejection. In the current experiments, supernatants from cultured lymph node cell populations were incorporated in Elvax 40 slow-release polymers and implanted in the corneal stroma. Separation of whole lymph node cells into highly T-enriched and macrophage populations permitted investigation of soluble cell products responsible for angiogenesis. It was found that sepharose-linked concanavalin A was an adequate stimulus for the generation of angiogenic activity, as assessed in the rabbit corneal micropocket assay. Cytoadherence, nylon wool column cell separation, and mitomycin C treatment of individual populations were used to demonstrate that stimulated T cells are a source of angiogenic material. The time-course and magnitude of the angiogenic response were equivalent in normal and x-irradiated leukopenic animals. These observations demonstrate that a potent angiogenic lymphokine secreted by stimulated T cells is active in the corneal micropocket assay system.

2-Fluoro-5-iodo-ara-C (FIAC), a new and potent drug, was tested for antiviral activity against several strains of herpes simplex virus (HSV), types 1 and 2. Effective dose-50% (ED-50) determinations for FIAC ranged from 0.023 to 0.51 muM for HSV-2. FIAC-treated cells did not exhibit any toxicity until the drug concentration was increased 2000-fold above the ED-50 level. Ocular herpetic keratitis in New Zealand white rabbits was treated with 1.0%, 0.1%, 0.01% FIAC beginning 3 days after inoculation wit HSV-1 (McKrae strain). Topical chemotherapy was administered five times per day for 7 consecutive days. After 4 days of treatment, corneal epithelial involvement, conjunctivitis, iritis, and clouding were not detectable in eyes receiving 1.0% FIAC. Toxic reactions were not observed in rabbit eyes treated with FIAC drug. HSV was not prevented from spreading into the central nervous system when topical FIAC therapy was initiated on day 3 after inoculation.

The clinical and aetiological findings are presented on 343 patients with clinical adenoviral conjunctivitis treated between July 1979 and July 1980 at 3 eye clinics in Sapporo. The age of the patients ranged from 12 days to 79 years, and the monthly incidence of the disease from 9 to 83, with clustering in the summer season. The aetiological diagnosis was established in 196 (57%) of 343 patients: adenovirus 3 in 24 cases; adenovirus 4 in 33 cases, including one case serologically diagnosed; adenovirus 8 in 124 cases; and adenovirus 19 in 15 cases. The different serotypes caused different clinical pictures. Adenovirus 19 conjunctivitis was more severe, with keratitis and preauricular lymphadenopathy more frequent than that of adenovirus 3 conjunctivitis. Adenovirus 4 conjunctivitis was generally similar to adenovirus 3 conjunctivitis. The clinical difference between the 2 groups adenoviruses 3 and 4, and adenoviruses 8 and 9, was statistically significant. Although in Japan adenovirus 8 was the most prevalent, adenoviruses 4 and 19 should be considered as causative agents of adenoviral conjunctivitis.
