Two cases of follicular conjunctivitis due to Chlamydia trachomatis followed by punctate epithelial
keratitis
are described. Both cases were initially treated with either oral tetracycline or doxycycline with resolution of the follicles. These two patients subsequently had recurrent, bilateral grayish lesions at various levels in the corneal epithelium that stained in a punctate fashion with fluorescein. There was anterior stromal edema associated with some of these lesions in one case. The lesions were confined mostly to the central cornea. These recurrent lesions were unassociated with a conjunctival reaction, were unresponsive to oral tetracycline, but were exquisitely responsive to low doses of topical steroids. Chlamydial conjunctivitis and the associated
keratitis
typically shows no response or actual exacerbation of symptoms with topical steroids, and the
keratitis
shows a predilection for the upper half of the cornea. These patients demonstrate that chlamydial keratoconjunctivitis might result in a clinical appearance consistent with
Thygeson's superficial punctate keratitis

Since the antivectorial control in 1948, the parasitological and clinical aspects of the onchocerciasis focus at Kinsuka/Kinshasa have not been studied further until now. The population examined was selected for its daily contact with the vector. It constituted 143 adults, including fishermen, stonebreakers and women cultivators, and 26 children aged from 10 to 16 years. People were subjected to an ophthalmological examination as well as to an investigation for Onchocerca volvulus microfilariae. Adults were examined for the presence of nodules as well as for dermal and lymphatic manifestations of onchocerciasis. The microfilaria carrier rate (MfCR) was 59.2%. The mean microfilaria density (MfD) was 9.7 mf mg-1. Only 20% of the adults presented with small onchocercal nodules. Cutaneous lesions were mild. Lymphatic complications such as adenolymphocele, hydrocele and elephantiasis of the lower limbs were observed in 18% of the adults. No one was blind from onchocerciasis, although nine patients of 60 years or over had lost the use of one eye.
Punctate keratitis
occurred commonly in young people. Ocular lesions, except for punctate
keratitis
, developed progressively after 55 years and they became worse with the degree of infection, age and length of stay. Hyperendemic and insufferable in 1940, onchocerciasis has become, in 1985, hypoendemic and no longer a public health problem. One could deduce that a situation similar to that in Kinsuka may occur within 30 years in other foci which are today hyperendemic and where prophylaxis has been started.

Little is known of the epidemiology and clinical picture of ocular onchocerciasis in South America. A survey of onchocercal eye disease was performed in the hyperendemic area of a rain forest focus of onchocerciasis in Esmeraldas Province in Ecuador. A total of 785 skin snip positive individuals from black and Chachi Amerindian communities were examined. The blindness rate attributable to onchocerciasis was 0.4%, and 8.2% were visually impaired. Onchocercal ocular lesions were seen in a high proportion of the study group: 33.6% had punctate
keratitis
, microfilariae in the anterior chamber and cornea were seen in 28.9% and 33.5% respectively, iridocyclitis was seen in 1.5%, optic atrophy in 5.1%, and chorioretinopathy in 28.0%. Sclerosing keratitis was not seen. The prevalence of all ocular lesions increased with age.
Punctate keratitis
was strongly associated with microfilarial counts in the cornea and chorioretinopathy was correlated with infection intensities in the cornea and anterior chamber. Chachi Amerindians had higher anterior chamber microfilarial counts and a greater prevalence of punctate
keratitis
than blacks though blacks had a greater prevalence of iridocyclitis and optic nerve disease. The pattern of ocular disease resembled rain forest onchocerciasis in west Africa with few severe ocular lesions in the anterior segment and all blinding lesions attributable to posterior segment disease.
