Three cases of congenital euryblepharon are presented. In these cases, horizontal width of the eyelid margin is abnormally increased in comparison to the globe and orbit. The 3 cases were all different. One case had involvement of all 4 lids. Another had unilateral involvement of upper and lower eyelids. A third case had bilateral lower lid involvement associated with other anomalies. The additional anomalies which have not been previously reported include a double row of meibomian gland orifices and an extreme lateral displacement of the lower canaliculus and punctum. All cases had varying degrees of vertical shortage of skin in the involved lids leading to
lagophthalmos
with a mild to moderate exposure
keratitis
. A review of previously reported cases of euryblepharon is made. The types of surgical procedures used to correct the deformities are discussed.

Different manifestations on some ocular and adnexal structures may accompany ichthyosis. Some ophthalmological manifestations in the clinical course of ichthyosis may endanger the eye and even lead to the sight loss. The paper reports a case of a patient with congenital ichthyosiform erythrodermia and ophthalmological manifestations on the lids and cornea. In addition to ichthyosis of the lid skin, squamous blepharitis and cicatricial ectropium of the lower lids were also present, while signs of exposure

on the right eye and corneal leucostaphyloma on the left eye were also noted. Presence of
lagophthalmos
and permanent exposure of both eyeballs led to deterioration of the condition which may result in the sight loss on the right eye, as well. Therefore, contracture release of both lower lids was performed followed by full-thickness skin graft. Surgical procedure and the postoperative course were uneventful. The operation successfully corrected position of the lower lids and enabled formation and reestablishment of the precorneal tears film which led to improvement of the condition of the cornea. The effect has been maintained for six postoperative years now.

Forty-eight leprosy patients in The Netherlands were re-examined 10 years after initial examination. Forty-six of these patients had received a course of multidrug therapy (MDT), according to the World Health Organization recommendation, at the time of their initial examination. Two patients had burned-out disease and had been merely under observation. Out of 40 patients, who initially did not show eye complications due to leprosy, 37 patients were essentially the same 10 years later. The eyes had changed in 3 multibacillary patients: 1 patient had developed a late type 1 reaction with facial nerve involvement and

; 2 patients had undergone intra-ocular surgery for cataract and acute glaucoma, respectively. Out of 8 patients with pre-existing eye involvement, 1 patient recovered and the lesions in 2 patients remained unaltered. One patient showed progression of pre-existing exposure

. Four patients had undergone cataract extractions; all four patients were lepromatous, with a long history of disease and signs of iris involvement at the first examination. The main progressive lesions were cataracts in lepromatous patients.

Facial paralysis can result in serious keratopathy because of corneal exposure and inadequate lacrimation. Thirty-seven patients underwent thirty-eight gold weight upper lid implants to rehabilitate paralysis of the eyelid from various causes. When indicated, implantation was combined with lower lid ectropion repair, medial canthoplasty or brow lift. Because of encouraging results in patients with longstanding facial paralysis, "early" implantation (within 1 month of paralysis) was offered to patients with severe

in whom (1) a severe neural injury was documented at the time of transtemporal surgery or (2) delayed, incomplete return of function was expected. Gold weight implantation resulted in excellent eyelid closure, protection, and cosmesis. There were no infections or extrusions.

often achieved. A mild worsening of one patient's pre-existing astigmatism developed, which resolved after reimplantation with a lighter weight. The implant is easily removed from those patients who, having undergone early implantation, eventually recover adequate function. Gold weight loading has become our procedure of choice for eyelid rehabilitation.

punctata superficialis. The results showed effective treatment which occupied 95.7%. In the discussion, the authors point out that Na-HA has the same stagnatisity and elasticity as biotears because of its adhesion to the corneal epithelium. Moreover, it has the water-retaining microsponge action similar to that of mucopolysaccharide. Hence, Na-HA has protective effects on corneal epithelium. Finally, the authors conclude that Na-HA eyedrop is so far the most ideal substitute for tears and superior to other artificial tears.

Patients with chronic progressive external ophthalmoplegia (CPEO) are often disabled by ptosis; however, conventional ptosis surgery may induce

. Ten patients with CPEO underwent ptosis correction via bilateral frontalis suspensions, using monofilament synthetic material. Three of these patients were also treated with lower eyelid horizontal tightening. The frontalis sling was adjusted to provide a firm linkage between the eyebrow and eyelid, but was loose enough to allow eyelid closure when the frontalis muscle is relaxed. All patients experienced lessening of ptosis and relief from visual obstruction. One patient required reoperation of one eyelid for undercorrection. No

We report on a male patient with exophthalmos of unclear etiology, basilar impression, syringohydromyelia and type II Arnold-Chiari malformation. Two diseases involving the orbital region were to be considered in differential diagnostic terms: endocrine orbitopathy and osseous orbit dysplasia. was striking. Tetraspasticity with pareses, bulbar symptoms, proximally pronounced muscular atrophy as well as a left hemihypesthesia was shown neurologically. Although the orbit CT was normal, sonography revealed thickened ocular muscles. There was euthyroidism in diffuse goiter with negative thyroid autoantibody findings. Because of lack of definitive detection of muscular swelling, tumor, or vascular anomaly in the various images, orbital dysplasia in the context of a malformation syndrome affecting several organs is suggested as the cause of the exophthalmos.

A clinical examination of 199 leprosy patients in Kenya showed that 62.5% had abnormal ocular findings, 51.2% of which were leprosy related. The potentially blinding lesions were uveitis,
keratitis
and
lagophthalmos
and these were found in 22.1% of the patients. 6 Patients (3%) had a visual acuity of less than 3/60 in the better eye and were classified blind. In all, there were 31 blind eyes of which only 12 had leprosy related causes. 10 of these (83%) belonged to lepromatous patients and 2 (17%) to tuberculoid patients. All the 10 eyes of the lepromatous patients were blind due to uveitis. It was shown that lepromatous leprosy patients are at a greater risk of leprosy related blindness especially from uveitis. The socio-psychological and economic implications of leprosy are discussed. Practical recommendations for the primary eye care of the leprosy patient are suggested.
