The clinical picture of a 12 year-old boy with Reiter's syndrome, manifested by conjunctivitis, keratitis, polyarthritis and urethritis, is presented and compared with the other 26 boys and girls with this syndrome that have been reported elsewhere. Light microscopy and inmunofluorescence studies of synovial membrane showed nonspecific changes. However, with the electron-microscope two types of cellular inclusions were seen in both, synovial cells and macrophages. Such changes were similar to those described in adults with Reiter's syndrome.

Data are reviewed that illustrate the efficacy of the immunofluorescence (IF) technique as a method of rapid viral diagnosis. Its constant application in the "Stefan S. Nicolau" Institute of Virology has allowed the detection of viral and/or inframicrobial antigens in cells from patients with eye diseases (keratoconjunctivitis, keratitis, uveitis), stomatitis, skin diseases, benign and malignant gynecopathies, male urethritis, respiratory diseases, facial paralysis, etc. The IF technique has an incontestable advantage as regards the detection of the simultaneous presence of several infectious agents in the same patient. The prerequisites and prospects of a wider application of IF methods for rapid viral diagnosis are briefly discussed.

Twenty-five consecutive cases of adult inclusion conjunctivitis were studied. Diagnosis was based on the finding of typical Halberstaedter-Prowazek inclusion bodies on conjunctival scrapings. Corneal involvement was common (20/25 patients); most frequent was superficial epithelial keratitis (15/25). Other changes seen included subepithelial nummular keratitis (6/25), marginal keratitis (2/25), (4/25). There seemed to be a predilection for involvement of the upper half of the cornea. Conjunctival scarring occurred in one patient only. Associated genitourinary symptoms were spontaneously reported in three patients only; however, on careful questioning, an additional 12 patients were found to have a history of urethritis or cervicitis. A comparison with viral follicular conjunctivitis is made. The location and pattern of keratopathy, associated genitourinary complaints, mucopurulent nature of discharge, and lack of response to standard topical therapy would seem to suggest chlamydial cause. Conjunctival scrapings are very helpful in differential diagnosis between viral and chlamydial conjunctivitis.

We studied characteristics of ocular inflammation in Reiter's disease after Salmonella enteritis in eight patients. After an acute onset with diarrhea, fever in six patients, and headache in three patients, all patients developed arthritis; six patients had myalgia; six patients, urethritis; and one patient, carditis. Sacroiliitis was found in four patients. All patients had HLA-B28 antigen. Conjunctivitis occurred in seven patients. It was mostly mild with no chemosis, follicles, or keratitis, and resolved in ten days. In one case palpebral edema, chemosis in the conjunctiva, and purulent exudate were seen. One patient had transient episodes of keratitis and corneal erosion for two months and episodes of conjunctivitis for 11 months. Three patients developed unilateral acute anterior uveitis with aqueous flare, cells, fine keratic precipitates, and fibrinous exudation from three to four years after the onset of the illness. One of these patients had vitritis and macular and papillary edema.
