Only a few agents with antiviral activity are available for regular clinical use. Amantadine hydrochloride is effective in the prophylaxis of influenza A2. Idoxuridine and adenine arabinoside have found application as topical agents in the treatment of herpes simplex keratitis. Adenine arabinoside is also under investigation in the treatment of disseminated infections due to herpes zoster and herpes simplex. Ribavirin, an agent with a wide spectrum of activity in vitro, has not fulfilled expectations in clinical trials. Because of the near eradication of smallpox, methisazone has become less important as a prophylactic agent in smallpox.

The 50% infectious dose of a preparation of herpes simplex virus was measured in eyes of rabbits by a multiple corneal inoculation method. One hour after inoculation of virus, one eye was treated with drops of human leukocyte interferon, and the other was treated with saline or with a different dose of interferon. Results from groups of three to four rabbits were combined for analysis. Treatment reduced the 50% infectious dose of virus in proportion to the concentration of interferon applied (within the range of 6.5 X 10(4)-1.3 X 10(6) units/ml) and not according to the total number of units instilled. Different treatment schedules were tried. Two applications of interferon each day were as effective as eight applications at intervals of 15 min or 1 hr. One application produced near-maximal antiviral effects for 18-24 hr. Thus, in human herpetic keratitis, a single daily application of the most concentrated available preparation of human interferon might be the most efficient schedule of treatment.

While vidarabine (Ara A) is in several ways superior to idoxuridine (IDU) it still has the disadvantages of rapid inactivation and poor solubility. The monophosphate ester or vidarabine, Ara AMP, however, is metabolized very slowly in humans and is extremely soluble with good tissue penetrance. The present study indicates that Ara AMP drops are equipotent or superior to vidarabine ointment in treatment of type 1 and type 2 herpes simplex keratitis. While iritis was significantly less in all treated groups, a suspicion of chemical side effect was raised but could not be proven in subsequent toxicity tests.

Two cases of herpes zoster ophthalmicus with dendritic keratitis are reported. Virological studies confirmed the double infection with herpes simplex type 1 virus in the corneal lesions and herpes zoster virus in the cutaneous lesions. We suggest the use of the immunoperoxidase test to identify the viral agent mainly because of its rapid and specific results. We are against the use of local steroids in dendritic keratitis unless the etiological agent is proved to be herpes zoster virus and not herpes simplex virus.

From 1969-1974 1000 unselected enucleated globes have been examined histopathologically. 277 derive from the University Eye Hospital in Hamburg, 723 from various Eye Hospitals in northern and southern Germany. They originate from 589 men and 408 women, three times the sex was unknown. 86 globes had to be removed from children less than 15 years old. 6 groups of etiologies have been distinguished: trauma (308), histologically confirmed neoplastic disease (281), ocular manifestations of systemic diseases (diabetes mellitus, occlusions of central retinal vessels presumably following generalized vascular disease etc.: 128), "operative ocular disease" (164), primary inflammatory disease (71), miscellaneous (malformations, high myopia, pseudo-glioma and pseudo-melanoma: 48). The etiology "operative ocular disease" consists of 67 primary glaucomas (57 adults, 10 buphthalmus), 41 idiopathic cataracts (7 of these congenital) and 3 primary corneal dystrophies, as well as 53 cases of primary retinal detachment. Among the 281 neoplastic diseases, there are 238 primary intraocular malignant melanomas of the uvea, 18 retinoblastomas, 4 primary reticulumcellsarcomas of the retina, 2 choroidal nevi, 10 intraocular metastases and 9 orbital tumors. 16 enucleations among the 1000 enucleations have been performed for pseudo-gliomas (5 x Coats disease, 5 x persistent primary hyperplastic vitreous, 2 x retrolental fibroplasia, others 4 x). The manifestations of systemic disease are consisting of 68 central retinal vein-occlusions, 30 complications of diabetes mellitus and 10 central retinal artery occlusions as well as 20 other generalized diseases. A primary inflammatory disease led to enucleation 50 times due to an intraocular process, 5 times due to scleritis and 18 times as a consequence of keratitis (including 13 times herpes simplex). As the final clinical cause for enucleation the following categories have been elaborated: secondary glaucomas (416), clinical diagnosis of "tumor" (275), atrophy and phthisis bulbi (118), inflammation (112), acute trauma to 4 weeks after the accident (72), others (7). In conclusion the central role of rubeosis iridis leading to secondary angle closure glaucoma is emphasized. This process presents a challenge to ophthalmologic research. Finally the significance of early surgery for primary angle closure glaucomas and for complete restoration of the anterior chamber after trauma and any intraocular procedure is stressed.

Treatment of established experimental keratitis caused by herpes simplex virus with 9-beta-d-arabinofuranosyladenine 5'-monophosphate (Ara-AMP) or 9-beta-d-arabinofuranosylhypoxanthine 5'-monophosphate (Ara-HxMP) showed that the Ara-AMP, in a concentration of 2 or 20%, had a significant effect on the keratitis but that 0.4% Ara-HxMP showed only minimal activity. Ara-AMP was also effective in the treatment of idoxuridine-resistant keratitis. No local toxicity with a high concentration (20%) of Ara-AMP was seen, but the duration of therapy was brief.

The various surgical managements of herpes simplex keratitis are listed. Our management is "atraumatic" keratoplasty, if vision is reduced, if the disease is protracted, or if perforation occurs. Our preoperative care is aimed at the production of a quiet eye with no active virus infection, by the use of antiviral drops, steroids, antibiotics, and bandage contact lenses. Removal of all corneal disease is desirable, but not mandatory. Perforations are managed in the same manner, but the host window is started at the perforation. Postoperative management is characterized by the use of therapeutic modalities necessary to prevent or treat complications, including steroids and frequent observation to check for complications of treatment. Our understanding of herpes simplex virus infections as they relate to surgical management is presented. The results of treatment warrant continuation of our efforts--but lead to the conclusion that it would be most helpful if latent virus infection could be eradicated.

A pathomorphological study of antiviral chemotherapeutic activity of benaphthon was carried out in tests involving intranasal inoculation of mice with influenza A2 (Hong Kong) 68 viruses (infection doses of 1 DL75 and 10 DL75) and of rabbits with herpetic keratitis, provoked by the herpes simplex virus (strain Ela-5699). A single prophylactic introduction of bonaphthon with subsequent 4-day treatment of experimental influenzal pneumonia of mice was found to produce a significant diminution in the frequency of the pneumonia development in experimental animals, a reduction in the number of severe and lethal forms of the malady and, by preventing the development of morphologically grave forms, to improve the prognosis. The use of bonaphthon in experimental herpetic keratitis of rabbits, irrespective of the mode of its administration (locally or by mouth) cuts down the period of the keratitis cure almost in half, contributes to a speedier epithelization of the ulcerated surface and to the disappearance of inflammatory manifestations in the very substance of the cornea.

12 african green monkeys were inoculated in both eyes with herpes simplex virus typ 1 and 16 rhesus monkeys with vaccinia virus. The right eyes were treated with human leukocyte interferon (HIF) while the left eyes served as controls and showed the typical keratitis, 7 out of 8 herpes eyes and all 9 vaccinia eyes which were treated prophylactically or simultaneiously with HIF showed no signs of disease. When HIF was given later in the course of the infection the keratitis was either little influenced or not modified at all. The reasons are discussed.

Morphologic changes in rabbit cornea accompanying herpes simplex keratitis especially under the treatment of ethyldeoxyuridine (EDU) are summarized in regard to clinical aspects, light microscopy and electron microscopy. Untreated eyes show virus-dependent characteristic changes of the cell structure: peripheral migration of the cell chromatin, swelling of the nucleus, and disappearance of the nucleolus are persistent. In the nucleus as well as in the cytoplasm mature and immature virus particles are visible which demonstrate the normal virus-replication course. In the EDU treated cornea these particular changes are observed only in the primary stage. After prolonged treatment in the nucleus of the infected cells there are only immature virus particles with optically empty center. This can be evaluated as a sign of inhibition of the normal replication. No virus formation was detected in the cytoplasm. After 7 day treatment of EDU, the corneal epithelium is almost of normal structural appearance. Accordingly, the present results on the rabbit seem to correlate well with the reported therapeutic antiherpetic studies in the human cornea.
