Although hypothermic cardioplegic arrest is a basic method of myocardial protection in cardiac surgery, the beta-adrenergic receptor (BAR) system has been little investigated in the heart subjected to hypothermic
ischemia
. Additionally, although the hypothermic arrest is often induced in hearts with preischemic desensitization of the BAR system by preceding congestive heart failure, the functional state of the BAR system after
ischemia
has not been studied in these hearts. We investigated alterations in the BAR system after hypothermic
ischemia
in normal rat hearts and in those with preischemic desensitization of the BAR system produced with isoproterenol (
ISP
: 400 micrograms/kg/hr for 24 hr). Both normal and BAR-desensitized hearts were isolated and subjected either to 40 min of hypothermic (10 degrees C) global
ischemia
followed by 40 min of reperfusion or subjected to time-matched aerobic perfusion with modified Krebs-Henseleit solution. At the end of perfusion (1) BAR binding properties with [3H]CGP-12177 and adenylate cyclase activity were measured in crude membrane fraction and (2) the inotropic response to
ISP
(delta LV + dP/dtmax) was evaluated in an isovolumetric contracting heart preparation. Following reperfusion, normal hearts without desensitized BAR showed a higher Bmax value than those of nonischemic time-matched hearts (41.8 +/- 3.1 vs 35.4 +/- 2.4 fmole/mg protein, P less than 0.05), whereas the Kd value was in a similar range in the two groups.(ABSTRACT TRUNCATED AT 250 WORDS)

Impedance spectroscopy has been proposed as a method of monitoring mucosal injury due to hypoperfusion and
ischemia
in the critically ill. The present study evaluates spectral differences in elective cardiovascular surgery patients compared with the impedance spectra of the gastric mucosa in healthy adults. An impedance spectroscopy probe and nasogastric tube (
ISP
/NGT) was placed into the stomach of 77 heart surgery and 21 healthy volunteers. The recorded impedance spectra were classified into 3 groups: group 1 for healthy volunteers; group 2 for patients with neither
ischemia
nor complications; and group 3 for patients with
ischemia
and complications during the first 3 days in the recovery unit. There were statistical differences in resistance (p<0.001) and reactance (p<0.001) among the 3 groups. The results show that impedance spectra are distinct and significantly different in patients with probable ischemic mucosal injury compared to healthy subjects, and therefore this technology may be a useful prognostic and diagnostic monitoring tool.

Impedance spectroscopy has been proposed as a method of monitoring mucosal injury due to hypoperfusion and

in the critically ill. The present paper presents an algorithm developed to calculate the characteristic electrical values that best describe human gastric impedance measurements and simplify the information obtained with this method. An impedance spectroscopy probe and nasogastric tube (
ISP
/NGT) was placed into the stomach of healthy volunteers, cardiovascular surgery and critically ill patients, and a database with 16199 spectra was obtained. The gastric spectrum forms two semi circles in the complex domain, divided into low frequency (F < 10 kHz) and high frequency (F > 10 kHz). A fitting algorithm was developed based on the Cole model, and central characteristic parameters were calculated. The parameters were validated using the normalized mean squared error and 0.66% of the spectra were discarded. From the experimental data obtained in humans, the greatest changes observed as the gastric mucosa becomes ischemic occur at low frequencies, which are specific and sensitive to tissue damage, and vary with the degree of hypoperfusion.

Endothelial nitric oxide synthase-derived NO and its derivative, peroxynitrite (ONOO(-)), suppresses oxygen consumption by nitration of mitochondrial proteins after reperfusion. However, very few nitrated proteins are identified to date. In this paper,
ischemia
/reperfusion (I/R) injury was induced in mouse heart by ligation and release of the left anterior descending coronary artery. Western blotting showed that tyrosine nitration was higher in I/R hearts. Nitrated proteins were identified by capillary-liquid chromatography-nanospray tandem mass spectrometry. A total of 23 proteins were identified as being nitrated after I/R and 10 of them were from mitochondria. The nitrated mitochondrial proteins included 4 subunits from the oxidative phosphorylation system (the 24 and the 30 kDa subunits of complex I, the Rieske
ISP
of complex III, and the alpha subunit of ATP synthase), five enzymes in the matrix, and voltage-dependent anion channel. In purified complex I treated with ONOO(-), 3-NT was identified locating at the residue of Y247 of the 30 kDa subunit and the residues of Y47, Y53 of the 49 kDa subunit. In conclusion, I/R induced protein nitration and mitochondrial proteins were the major targets. Selective nitration of proteins from the oxidative phosphorylation system at the beginning of reperfusion may contribute to the suppression of oxygen consumption.
