A 56-year-old male with a history of orthotopic cardiac transplantation secondary to cardiac sarcoidosis presented with recurrent episodes of syncope preceded by dizziness. While on telemetry, he had transient episodes of high-grade atrioventricular (AV) block that reproduced his prodrome. After excluding allograft rejection,
ischemia
, recurrent cardiac sarcoidosis, and vagally mediated block as a cause of high-grade AV block, adenosine testing was done which reproduced the spontaneous high-grade AV block. We concluded that hypersensitivity to endogenously released adenosine was the likely mechanism of AV block in our patient. This is the first reported case of adenosine testing in a cardiac transplant patient for diagnosing episodic high-degree AV block of unclear etiology.

For many years tourniquets were perceived as dangerous due to the belief that they led to loss of limb because of
ischemia
. Their use in civilian and military environments was discouraged. Emergency medical responders were not taught about tourniquets and commercial tourniquets were not available. However, research by the United States military during the wars in Iraq and Afghanistan has demonstrated that tourniquets are safe life-saving devices. As a consequence, they have been widely deployed in combat situations and there are now calls for the use of tourniquets in the civilian prehospital setting. This article presents a report of the successful application of a tourniquet by the LIFE STAR crew to control bleeding that local emergency medical services (EMS) personnel could not control with direct pressure. Tourniquets should be readily available in public places and carried by all EMS.

Despite their high embolic potential, however, widespread systemic and peripheral embolization, particularly to the lower extremities, is still uncommon. We describe a case of a 60-year-old female with a history of metastatic carcinoid disease with liver involvement who developed coronary vasospasm and mesenteric
ischemia
. The carcinoid syndrome is known for its cardiac involvement most well characterized by fibrous tissue deposits on the endocardium.(1,2) Case reports of coronary artery vasospasm have been previously described and hypothesized to be mediated by vasoactive amines and polypeptides synthesized by the tumor.(3-9) Intestinal ischemia is another reported complication of the carcinoid syndrome and is hypothesized to have a similar mechanism to that of the coronary vasospasm.(10-17) We have reviewed the literature and describe a case of coronary vasospasm and mesenteric
