Oculomotor nerve palsy
is very rare as the only manifestation of internal carotid artery (ICA) occlusion, and the angiographic findings have not been described in detail. We present a case of ipsilateral ICA occlusion manifesting as only sudden onset of oculomotor nerve palsy. Cerebral angiography showed occlusion of the ICA from the cervical portion to the ophthalmic portion. Distal blood flow was compensated through an anastomosis from other arteries.
Ischemia
of the oculomotor nerve was thought to be caused by loss of supply from branches arising from the occluded cavernous portion of the ICA.

Diplopia is frequently encountered in neurological practice and may occur as part of the clinical spectrum of 3rd cranial nerve palsy. Correct localization of the underlying problem is the first step in making an accurate diagnosis. Pathologies affecting the supranuclear structures, cranial nerves and nuclei, extraocular muscles and the neuromuscular junction may cause or simulate
3rd nerve palsy
. We report a case of a patient with longstanding quiescent SLE who presented with sudden onset of diplopia and discuss possible aetiologies including
ischemia
, demyelination, neuromuscular conditions and medication-induced.

Oculomotor nerve palsy
can be due to varied causes that include diabetic neuropathy, myasthenia gravis, brainstem infarction, demyelinating conditions, and cerebral aneurysms. Among the aneurysmal causes of oculomotor nerve palsy, aneurysm of the posterior communicating artery has been observed to be the most common. Pupillary dysfunction is considered to be an important feature of aneurysmal oculomotor nerve paresis. A case of a 7-year-old boy with partial oculomotor nerve palsy with pupillary sparing is being reported here, the etiology of which is tortuous and ectatic distal internal carotid artery. This is a rare cause of oculomotor nerve paresis and to the best of our knowledge has not yet been reported in children.
Ischemia
rather than compression seems to be the most plausible cause in this case.
