Report is made of two patients with Epilepsia Partialis Continua (E.P.C.) from brain organic damage (since
carcinoma
metastasis and localized
ischemia
). Clinical EEG, neuroradiological and anatomo-pathological and therapeutical problems are dealth with the light of a review on previous papers. The EEG by itself is assumed as a not sufficient neurophysiological mean. Long time poligraphic enregistrations during awakeness and sleep havae, on the contrary, produced interesting data. The continuous and localized more or less rhythmic myoclonus, which are the distinctive feature in the E.P.C., were in both the patients confined to the first two fingers of their hands; the more they decreased the deeper was sleep (phase II and III-IV) and almost disappeared in the REM phase. Thus poligraphic enregistrations for E.P.C. patients are maintained as very significant.

This time trend of hemodynamics and mitochondrial functions were studied to determine whether the ligation of the hepatic artery would result in an antitumor effect on 3'-methyl-4-dimethylaminoazobenzene-induced hepatic
carcinoma
in rats. The studies revealed that the hepatic tumors were nourished predominantly by the artery and less by the portal vein; the size of the vascular beds in the hepatic tumors decreased as compared with those in the non-tumor area; and as the tumors grew larger, the artery became less predominant and the size of vascular beds decreased further. The mitochondria in the tumor were characterized by impaired growth, impaired oxidative phosphorylation, and by the low activity and nucleotide specificity of membrane bound ATPase. Hepatic dearterialization enhanced
ischemia
in the tumors and was accompanied by intensified impairment of the aerobic energy production, resulting in necrosis of the tumor. The effects of the dearterialization tended to decrease after the 5th day following the operation. In view of the gross findings upon relaparotomy and the recovery of hemodynamics and mitochondrial functions, this tendency appeared to be chiefly attributed to the increasing collateral circulation.

This report details the case histories of five patients with this entity who have been admitted to the surgical service of Presbyterian Hospital since 1967. The colitis described is clearly related to the obstructive lesion and does not represent chronic inflammatory colitis in association with a

. The patients may initially have symptoms secondary to the obstructing lesion or fulminant peritoneal signs secondary to the colitis. The roentgenographic appearance is typical and closely resembles that seen in colonic

Several large series have supported the success of retroperitoneal lymphadenectomy as a primary therapeutic modality in the management of non-seminomatous germ cell testicular neoplasms. In 1963 Tavel and associates reported on the inadequacy of this procedure in the complete extirpation of all the retroperitoneal lymph nodes. Our investigation was undertaken to test their conclusions, the sole difference in technique being that the infrarenal lumbar vessels were ligated and divided during the lymph node dissection. Results in 12 cadavers revealed that all retroperitoneal nodal tissue could be excised with the modified technique. These findings were further verified by the absence of any residual retroperitoneal lymph nodes in 3 patients who died of metastatic embryonal cell

. The extirpative efficacy of this procedure having been established, any major deleterious effects of lumbar ligation, primarily spinal cord

, must be considered. Based on our experience and that of others it may be concluded that this complication represents a minimal risk that should not discourage use of this effective therapeutic procedure.
