101 outpatients with irritable bowel syndrome were randomly allocated to two treatment groups. Both groups received the same medical treatment, but patients in one group also received dynamically oriented individual psychotherapy in ten hour-long sessions spread over 3 months. After 3 months there was a significantly greater improvement in somatic symptoms in the psychotherapy group. The difference became more pronounced a year later, with the patients given psychotherapy showing further improvement, and the patients who received medical treatment showing some deterioration. The combination of medical treatment with psychotherapy improves outcome, not only in the short term but also in the long run.

For more than thirty years the pathophysiology of the so-called "irritable bowel syndrome" has given rise to numerous theories. These can be summarized in a series of questions to which this review paper does not pretend to provide definite answers. Is there a basic myoelectric anomaly specific to the irritable bowel syndrome? Are there disorders of colon motility that characterize the symptoms experienced by the patient? Are specific tests available that would stimulate colonic contractions in a reproducible and comparative manner? What place should be given to disturbances in bowel sensitivity? Is the colon the only target-organ in functional intestinal disorders? Is the role of digestive hormones underrated or at least insufficiently known? Do patients with the syndrome have a special psychological profile? Has intolerance to various foodstuffs been excluded too hastily? The secrets of the irritable colon syndrome are far from being entirely unravelled. This heterogeneous group of symptoms seems to be due to many different causes associated to varying degrees in each patient.

Abdominal epilepsy is an uncommon cause for abdominal pain in children and adults. Although its abdominal symptoms may be similar to those of the irritable bowel syndrome, it may be distinguished from the latter condition by the presence of altered consciousness during some of the attacks, a tendency toward tiredness after an attack, and by an abnormal EEG. Abdominal epilepsy is usually treated with anticonvulsant medication. This unusual cause for abdominal pain should be considered in patients with the appropriate abdominal symptoms who do not respond to the usual symptomatic therapy.

It has been shown that genetic factors within the HLA region are involved in the etiology of several diseases. For some of these, the existence of another genetic factor has been suggested, although not proven. A possible way to give evidence for another locus (G) is to show that the disease and an unlinked HLA-marker locus (M) do not segregate independently. The usual lod-score method, which assumes monogenic inheritance, is inappropriate for this test. We propose a correction of this method for performing a linkage analysis between the G and M loci, taking into account the role of HLA. A very simple way of using the HLA information is by modifying, for each individual of a pedigree, the penetrance values at the G locus according to the number of HLA haplotypes shared with the index case. These penetrance values are inferred from the observed IBD (identity-by-descent) distribution of HLA haplotypes in a sample of affected sib-pairs. The advantage of using this empirical distribution is that it is not based on any assumptions concerning the mode of inheritance at the HLA-linked locus. This correction method was established using a two-locus model with restrictive assumptions. Its value is discussed for various sets of parameters in more general and realistic two-locus models using simulations.

It has been proposed that the presence of elevated serum titres against various Bacteroides species among patients with chronic inflammatory bowel disease (CIBD) may yield aetiological, pathogenic, or prognostic information. Using a crossed immunoelectrophoretic method, we investigated circulating antibodies against four Bacteroides species in 122 patients with CIBD (80 with ulcerative colitis (UC) and 42 with Crohn's disease (CD)) and in 32 patients with the irritable colon syndrome. In this cross-sectional study we found raised titre scores (greater than 0) among 26% of the patients with CD, among 46% of the patients with UC, and among 34% of the patients with the irritable colon syndrome. These differences are not significant. There were no correlations between the antibody titres and the duration of the disease, the clinical disease activity, or the site of the disease. Furthermore, the antibody titres yielded no prognostic information as to the necessity of surgery--that is, colectomy and/or small-bowel resection.

The significance of recent motor data on the distal colon in irritable bowel syndrome is obscured by methodological questions such as bowel preparation, variability and reproducibility of motor activity and segmental differences. Thirty subjects were studied after prior bowel cleansing with a water enema on two separate occasions 1 wk apart (10 with one bowel movement per day, 10 with two to three per week, and 10 with three to 12 per day). Two additional studies were performed in 10 of the subjects, one with and one without prior cleansing. The motility indices of the cleansed and unwashed colon were dissimilar. The motility index per 3 min varied markedly over 3 h, and the indices between the paired studies were rarely identical. Marked segmental differences were noted. The mean motility index per 3 min of the constipated and the diarrhea groups was significantly greater than that of normals only in the 15-cm segment. In view of these variables, caution should be exercised in the interpretation of basal motor activity of the distal colon.

To understand the disorders of bowel motility, it is important to know the range of bowel patterns in the general population. We have devised a brief self-administered questionnaire which, when used among a group of 789 students and hospital employees, disclosed that 94.2% had stool frequencies between three per day and three per week, and that 17.1% had bowel dysfunction. When compared with the remaining sample, the bowel dysfunction group was predominantly female. This group also reported more often that stress influenced their bowel function, and more often used laxatives and visited physicians for bowel complaints. Further medical evaluation to characterize this subgroup is needed. We believe that selection, for psychologic and physiologic study, of subjects with bowel dysfunction not seeking health care will provide a needed comparison group in our understanding of patients with irritable bowel syndrome.

Irritable colon syndrome of infancy is the most frequent cause of chronic diarrhea in children between eight months and three years of age who are still gaining weight at a normal rate. The diarrhea occurs in response to a wide variety of stimuli, and a strong family history of functional bowel disorders is common. The usual dietary regimens for diarrhea perpetuate the problem. A regular diet, without snacks, is recommended, with fluids restricted to unchilled beverages taken with meals.

Previous studies suggested that the ingestion of a mixture of amino acids inhibited the fat-stimulated increase in colonic motility. This study determine the effect of the ingestion of an amino acid mixture on the postprandial distal colonic spike potential (SP) response to a standard 1000-calorie meal in normal subjects and in patients with the irritable bowel syndrome. The distal colonic response was measured following a standard meal with and without the preadministration of a protein hydrolysate solution. After the meal, normal subjects had their maximum response within the first 30-min postprandial period (41.0 +/- 6.3 SP/30 min). Spike activity returned to fasting activity by 60 min. Most patients with irritable bowel syndrome did not have a significant early postprandial distal colonic spike response (P > 0.05), but all patients had a large late increase in spike activity occurring 60-90 min postprandially (50.0 +/- 5.0 SP/30 min) (P < 0.001). Ingestion of the protein hydrolysate solution prior to the meal resulted in suppression of the early colonic spike response in normal subjects (P < 0.01) and the late colonic spike response in the irritable bowel patients (P < 0.01). This study suggests that amino acids can modulate postprandial colonic motility and that dietary alteration may be beneficial in the irritable bowel syndrome.
