There has been tremendous interest in 5-HT3 receptor antagonists since their discovery and the subsequent identification of 5-HT3 receptors in the CNS. Based on the results of early behavioural tests with these compounds, there has been substantial interest in their potential use for the treatment of various CNS disorders. In this review, Andrew Greenshaw attempts to clarify the status of the therapeutic potential of these drugs, discussing inconsistencies in preclinical findings and identifying areas in need of clarification through future research. 5-HT3 receptor antagonists are claimed to be potentially useful in the treatment of nausea, inflammatory pain (migraine and irritable bowel syndrome), anxiety, depression, schizophrenia, dementia and drug abuse!

Neuropsychiatric diseases viewed as multifaceted expression of a dysfunctional brain in which atypical responses are evoked by various sensory inputs. Disease entities have traditionally been classified according to the predominant manifestation ( ) without regard to the overlapping features of many of the diseases (+/-). Thus, mild to moderate pain, mood, cognitive, and neurosomatic symptoms are frequently present in chronic fatigue syndrome (CFS) patients. Fibromyalgia syndrome (FMS) is listed as an example of a predominantly chronic pain syndrome. Affect (mood) disorders include depression (Depress.), anxiety, panic reactions, blunted affect, mania, etc. Schizophrenia (Schizo.) is listed as an example of a major cognitive psychosis. Autism as well as various forms of dementia would be included in this category. Irritable bowel syndrome (IBS) is an example of a neurosomatic disease.

A 76-year old white married female had rapid onset dementia with myoclonus and was admitted to an HMO hospital where she was initially diagnosed as Alzheimer's disease. The physician- husband suggested that the condition might be due to the Pepto-Bismol which she had taken chronically to control her irritable bowel syndrome. An elevated serum bismuth level of 242 &mgr;g/L (normal is 5 &mgr;g/L) established bismuth toxicity as the cause of the dementia. With treatment the patient returned to a normal mental state. The possibility of bismuth encephalopathy needs to be considered in the differential diagnosis of possible Alzheimer's dementia.

This study investigated whether an association exists between irritable bowel syndrome (IBS) and the risk of Parkinson's disease. This is a retrospective cohort study using the dataset of the Taiwan National Health Insurance Program from 2000 to 2010. We identified 23,875 patients (aged 20 years or older) with newly diagnosed IBS as the IBS group and 95,500 subjects without IBS as the non-IBS group for comparison. The main outcome was incident Parkinson's disease compared between both groups by the end of 2010. We measured the hazard ratio (HR) to evaluate the association between IBS and Parkinson's disease. The overall incidence of Parkinson's disease in the IBS group was 1.76-fold higher than that in the non-IBS group (16.4 vs. 9.33 per 10,000 person-years). The multivariable Cox proportional hazards regression analysis revealed that the adjusted HR of Parkinson's disease associated with IBS was 1.48 (95% CI 1.27, 1.72), compared with the non-IBS group. Age, women, hypertension, dementia, cerebrovascular disease and depression were also significantly associated with Parkinson's disease. Patients with irritable bowel syndrome are at an increased risk of developing Parkinson's disease. Further studies are required to explore the pathophysiological connection between these disorders.

The irritable bowel syndrome (IBS) is a common chronic functional gastrointestinal disorder world wide that lasts for decades. The human gut harbors a diverse population of microbial organisms which is symbiotic and important for well being. However, studies on conventional, germ-free, and obese animals have shown that alteration in normal commensal gut microbiota and an increase in pathogenic microbiota-termed "dysbiosis", impact gut function, homeostasis, and health. Diarrhea, constipation, visceral hypersensitivity, and abdominal pain arise in IBS from the gut-induced dysfunctional metabolic, immune, and neuro-immune communication. Dysbiosis in IBS is associated with gut inflammation. Gut-related inflammation is pivotal in promoting endotoxemia, systemic inflammation, and neuroinflammation. A significant proportion of IBS patients chronically consume alcohol, non-steroidal anti-inflammatories, and fatty diet; they may also suffer from co-morbid respiratory, neuromuscular, psychological, sleep, and neurological disorders. The above pathophysiological substrate is underpinned by dysbiosis, and dysfunctional bidirectional "Gut-Brain Axis" pathways. Pathogenic gut microbiota-related systemic inflammation (due to increased lipopolysaccharide and pro-inflammatory cytokines, and barrier dysfunction), may trigger neuroinflammation enhancing dysfunctional brain regions including hippocampus and cerebellum. These as well as dysfunctional vago-vagal gut-brain axis may promote cognitive impairment. Indeed, inflammation is characteristic of a broad spectrum of neurodegenerative diseases that manifest demntia. It is argued that an awareness of pathophysiological impact of IBS and implementation of appropriate therapeutic measures may prevent cognitive impairment and minimize vulnerability to dementia.

BACKGROUND: There is recent evidence linking rosacea to systemic disease. OBJECTIVE: We sought to identify correlations in Google searches (Google LLC, Mountain View, California) for rosacea and comorbid conditions to assess whether the public is seeking information regarding these trends. METHODS: Google search data from January 1, 2004, to February 28, 2018, for rosacea and search terms representing common comorbid conditions were investigated. This analysis included searches occurring in the United States (US), Canada, the United Kingdom (UK), and Australia. Search volume index (SVI) data were plotted over time and Pearson correlation coefficients were calculated from search data to assess for correlations between search terms. RESULTS: The level of interest in rosacea was highest in the spring and lowest in the winter in the US, Canada, and UK. Seasonal search trends in Australia were the inverse of those in northern hemisphere nations. Significant correlations were found between depression and rosacea SVI in the US (R=0.481; p<0.001), dementia and rosacea SVI in the UK (R=0.774; p=0.011), and hypothyroidism and rosacea SVI (R=0.752; p<0.001) in the UK. Additionally, search trends for irritable bowel syndrome (R=0.399; p<0.001) and ulcerative colitis (R=0.514; p=0.032) correlated significantly with rosacea in Canada and the UK, respectively. In Australia, search trends for osteoporosis significantly correlated with rosacea (R=0.394; p<0.001). CONCLUSIONS: Our findings indicate growing interest among the general public regarding rosacea and comorbid diseases, which behooves clinicians to adopt a more comprehensive approach in managing rosacea patients.
