Twenty adult Indian patients suffering from the spastic form of
irritable colon
, i.e. abdominal pain and constipation, were given trimebutine (Mebutin), 2-phenyl, 2-dimethylamino-n-butyl 3, 4, 5-trimethoxybenzoate. Patients were given treatment with 200 mg trimebutine three times daily, or placebo for 4 weeks, and then crossed over. In addition, stool transit times were assessed by the single stool transit time (SST) method of Cummings. Results showed a statistical improvement in abdominal pain and constipation with both trimebutine and placebo after 4 weeks, but only with trimebutine after 8 weeks. Single stool transit time was significantly reduced after trimebutine.

To determine whether circulating immune complexes are present in the sera of patients with inflammatory bowel disease (IBD), a 125I-Clq binding assay was performed. Of the 55 IBD serum samples tested, the 24 ulcerative colitis samples demonstrated significant binding (33.1 +/- 8.3%, p = 0.02), whereas the 31 Crohn's samples bound essentially normal amounts (29.2 +/- 7.4%). A positive control group consisting of 27 patients with rheumatoid arthritis was also studied. Sera from 4 patients wiht IBD and colonic cancer when tested, bound 40.2 +/- 8.0% of the available 125I-Clq, while 10 patients with previous colectomies and ileostomies gave results similar to those of 15 healthy controls and 11 patients with
irritable colon

Three hundred and thirty-three consecutive patients with
irritable bowel syndrome

) were interviewed. By the age of 15 years, 31% of the patients had either lost a parent through death, divorce or separation; 19% had an alcoholic parent, and 61% reported unsatisfactory relationships with, or between their parents. The findings support the important influence of childhood deprivation in the aetiology of
IBS

The dietary fibre intake of 25 patients with the
irritable bowel syndrome
was assessed by dietary recall over one week for the period before onset of symptoms, at diagnosis and after six months treatment with bran and a fibre-rich diet, and compared with controls matched for age and sex. Mean intake of patients fell from 20.2 g per day before onset of symptoms to 17.8 g at diagnosis (n.s.), when it was significantly less than the controls' intake of 22.6 g (P less than 0.01). The 20 patients who improved with treatment increased their intake from 18.9 g to 24.8 g (P less than 0.02); the five patients who did not improve did not increase their intake significantly (13.5 g to 15.6 g).

It has been well known in Japan that fasting therapy has an excellent effect on various kinds of psychosomatic diseases. From 1967 to 1977 the therapy was carried out on about 382 cases of psychosomatic diseases in our clinic with an efficacy rate of 87%. The following diseases were considered as suitable indications for the therapy;

, behavioral disturbance of eating, functional disorders of the G-I tract, neurocirculatory asthenia, labile hypertension, variable psychosomatic symptoms of puberty, conversion hysteria and many kinds of neurosis with somatic complaints. The mechanism of effectiveness of the therapy is discussed from the psychological and biological aspects. It is concluded that the regulating mechanisms of the autonomic nervous system and endocrine system may change the whole functioning of the body including the brain, resulting in a spontaneous deconditioning of maladaptive bodily and mental behavior and an improved homeostatic adjustment to life.

This study tested the hypothesis that, patients with
irritable bowel syndrome
(
IBS
), there is a primary hyperalgesia of the colon. Previous work, which examined these patients and normals, has not included subjects who provide a control for relevant psychological characteristics. We compared ratings of pain, following varying degrees of distension of the sigmoid colon, in normals, patients with
IBS
, and patients who were psychologically disturbed but without bowel symptoms. Psychological characteristics were assessed by a psychiatric interview and psychometric inventories; response to distension was tested by placing a tube in the rectosigmoid colon and successively inflating a nd deflating a balloon at its tip at 10 cm3 increments up to 50 cm3. Ratings of pain were recorded at each volume. The results indicated that the two patient groups were psychologically similar and both were more disturbed than normals. A linear relation was found between reports of pain and volume of distension in all three groups. There were no significant differences between the proportions of subjects experiencing pain in each group or the average of the ratings. There were no significant associations between the pain ratings and measures of anxiety, depression, neuroticism, and extraversion. The data do not support the hypothesis that colonic hyperalgesia is an important contributory factor in the etiology of the
irritable bowel syndrome

A comparative study was conducted on the psychological features of chronic pancreatitis, peptic ulcer, ulcerative colitis and the
irritable colon
syndrome by means of psychological tests and interviews. The patients with
irritable colon
syndrome were found to be the most neurotic of all, whereas those with the definite type of chronic pancreatitis and ulcerative colitis presented prominent alexithymic characteristics. About one half of the patients having peptic ulcer showed alexithymic features. Despite their neurotic tendencies shown on the psychological tests, the patients with the suspicious type of chronic pancreatitis were socially better adjusted than those with
irritable colon
syndrome. It was also noted that the concept of alexithymia needs to be elucidated by including such factors as over-adaptation to the environment, certain distorted life habits and lack of bodily sensations.

The Tohoku University method of fasting therapy was performed on 380 patients. The clinical results revealed an efficacy rate of 87%. With regard to psychosomatic diseases,
irritable colon
syndrome, neurocirculatory asthenia, mild diabetes mellitus, obesity and borderline hypertension were good indications for this therapy. In order to clarify the therapeutic mechanism, several clinical examinations were administered before, during and after therapy. EEG data was analysed according to the power spectral method. The peak frequency decreased as fasting progressed, while it increased as re-fed continued. Percent energy of alpha waves after fasting therapy was significantly higher than that of the pre-fasting stage. The dexamethasone suppression rate of urine 17-OHCS after fasting therapy was significantly lower than that of the pre-fasting stage. It seems that ketone nutrition may work as a strong stressor in the brain cell, temporarily placing all biological mechanisms in a stress state and then activating the natural healing power inherent to the human body, thereby bringing about homeostasis.

Significant changes on a standard barium follow-through examination in celiac disease have been determined by comparison with functional changes (
irritable bowel syndrome
), malabsorption without a villous lesion (chronic pancreatitis), and a villous abnormality without malabsorption (dermatitis herpetiformis). Patients with iron deficiency anemia formed the control group. Slight jejunal dilatation (26-30 mm) was found in 15% of the celiacs and 17% of the irritable bowel patients. Dilatation in excess of 30 mm and/or effacement of jejunal fold pattern occurred only with an abnormal jejunal biopsy, in 54% of the celiacs and 33% of the dermatitis herpetiformis patients. Patients with malabsorption by itself and 46% of the celiacs could not be distinguished from those with
irritable bowel syndrome
. The concept of a malabsorption pattern is considered invalid, and the diagnosis of celiac disease can be reliably established only by peroral jejunal biopsy.
