An intussusception of the small intestine in association with a long tube usually occurs in the vicinity of the mercury-filled bag and can be visualized radiographically by instilling barium directly into the tube. On rare occasions, an intussusception develops in the proximal jejunum and is difficult to recognize. We report the fifth and sixth cases of a proximal jejunal intussusception with a long tube in situ and outline a clinical approach that facilitates a prompt, accurate diagnosis. A proximal jejunal intussusception should be suspected if copious bilious vomiting and abdominal pain occur following intubation of the small intestine with a long tube.

An improved technique for reduction of intussusception by rectal inflation is presented. This is done under fluoroscopy by means of a controllable pressure apparatus. Its pressure is controlled by adjusting the height of the mercury column in a U-tube manometer. The rectal inflation reduction technique has been widely practised for about 20 years in thousands of cases in China. An analysis of 2,496 cases in the years from 1978 to 1983 revealed a nonoperative reduction rate of 91% with two perforations but no deaths. It is good for early cases of intussusceptions. The apparatus is described. Its contraindications and criteria of irreducibility are discussed in the text.

Intussusception secondary to long intestinal tubes is a relatively uncommon but potentially fatal occurrence. From 1976 to 1979 we have studied five patients with this complication. The mercury-filled bag stimulates peristalsis, the tube is drawn forward, and there is resultant telescoping and "pleating" of the proximal small bowel. If these pleats become fixed by adhesions, they may act as a lead point for intussusception, even after tube withdrawal. Early diagnosis is essential for effective therapy.
