Surgical emergencies can be missed easily in children, who are not always able to volunteer relevant information. Awareness of the entities discussed in this review might help the EP uncover subtle clues to early diagnoses that might not be initially apparent. Ill-appearing children who have abdominal pain and vomiting should be considered to have ischemic or necrotic bowel until proven otherwise. Possible diagnoses include
volvulus
,
intussusception
, and necrotizing enterocolitis. Bilious vomiting, especially in a young infant, should be considered to be an indication of a high bowel obstruction such as midgut
volvulus
, which warrants immediate surgical consultation. Significant rectal bleeding with abdominal pain can result from
intussusception
,
volvulus
, or an inflamed Meckel's diverticulum. Rectal bleeding with unstable vital signs can result from an upper GI bleed (eg, peptic ulcer disease). Painless rectal bleeding can result from a Meckel's diverticulum, polyps, arteriovenous malformation, or a tumor. Examination of the genitalia is imperative, especially in boys, to exclude the possibility of an incarcerated hernia or testicular torsion.

in a Chinese 10-year-old boy affected by the blue rubber bleb nevus syndrome is presented and discussed. The syndrome is rare, sporadically found with possible dominant inheritance, and due to a gene mutation mapped on the short arm of chromosome 9. It presents with distinctive cutaneous and gastrointestinal malformations together with possible other organ involvement. Gastrointestinal malformations tend to bleed and lead to anaemia. Infrequent complications of the gastrointestinal malformations are

, intestinal infarction and

. The age of the patient and the jejunal

precipitated by a vascular malformation containing calcifications (which were also found in different gut segments) make this case remarkable.

Altered state of consciousness and lethargy could be early and cardinal complaints in diseases that actually originate within the abdomen. Thirteen children were seen at our departments in whom impairment of the individual's mental state preceded the appearance of common gastrointestinal symptoms. Whereas in infants with

Most cases of isolated nonduodenal bowel atresia are thought to be due to ischemic events from hypotension, vascular accident,
volvulus
,
intussusception
and cocaine. We present a case of isolated ileal atresia suggested using results of prenatal sonography which showed multiple markedly dilated fluid-filled intestinal loops with changeable shapes and positions secondary to active peristalsis. Postnatal oral contrast radiography demonstrated obstruction located at the ileal level. Exploratory laparotomy confirmed isolated ileal atresia. Primary end-to-end anastomosis was done. The infant endured the procedure well. Cases with multiple intestinal atresia always have fatal prognosis, even after surgery. Cases of isolated intestinal atresia usually have better prognosis and deserve more attention. Early diagnosis and early intervention are both important for good outcomes.

Two infants, aged 3 months and 4 months, with acquired ileal atresia in the distal ileum are described. Both of them presented with features of intestinal obstruction following a diarrhoeal episode. In both patients the time interval between the onset of symptoms and detection of atresia at surgery was very short, i.e. 2 weeks. In our opinion, strangulation with subsequent resorption of gangrenous gut could have been the underlying mechanism in both patients. However, a short interval between initiation of symptomatology and detection of atresia prompted us to postulate a thromboembolic phenomenon as the cause of these lesions. While in one patient

was noted on sonography, in the other there was no clear-cut aetiology present except for a high-lying caecum with a long mesentery that might have led to
volvulus
. We have reviewed the pertinent literature of this very rare sequela of gut inflammation.

Pneumatosis cystoides intestinalis is a rare condition that can be located in any part of the gastrointestinal tract. It is usually associated with a wide variety of gastrointestinal or pulmonary diseases. The primitive form is much less frequent and usually involves the left colon. The pathogenesis of pneumatosis cystoides intestinalis is still unclear. The mechanical theory, which is the most accepted explanation, postulates that gas is forced into the bowel wall by breaks in the mucosa; this is more likely to occur when the intraluminal pressure is higher, as happens in obstructive conditions, during endoscopies, or during infections from gas-forming bacteria. Pneumatosis cystoides is often asymptomatic, representing an occasional finding during investigations for other abdominal conditions. Complications occur in about 3% of cases and include obstruction,
intussusception
,
volvulus
, haemorrhage and intestinal perforation. When presenting acutely or in association with other abdominal conditions the differential diagnosis is rarely a problem. More important is to diagnose asymptomatic primitive submucosal pneumatosis of the colon, in order to avoid unnecessary intestinal resections. The simultaneous occurrence of

in the paediatric age group is rare. We report the case of a

of an ileoileal
intussusception
in an 8-year-old boy. This is the first time that computerised tomography (CT) images of a
volvulus
of an ileoileal
intussusception
have been published, and they clearly demonstrate both pathologies. This case highlights the use of CT in determining the nature of an abdominal mass and demonstrates how helpful it can be in diagnosing the cause of small bowel obstruction in children.
