An 81-year old man who had a
plasmacytoma
of the ileum presented with melena for which no bleeding site could be established before surgery. He developed
intussusception
and the tumor was located at laparotomy. Metastases to the skin and axillary lymph nodes were documented prior to his operation, to mesenteric lymph nodes at surgery and to intrathoracic lymph nodes and a substernal goiter at autopsy. The liver, spleen, bones and marrow were not involved by myeloma or amyloid. Immunoelectrophoresis demonstrated an Ig-A (lambda) monoclonal protein in the serum (with nondiagnostic serum zone electrophoresis patterns) and lambda light chains in the urine. Immunoflourescense of Ig-A (lambda) globulin was present in imprint preparations from the bowel tumor.

in two patients, one with metastatic melanoma and a second with a
plasmacytoma
, both of whom had previously nondiagnostic standard oral contrast medium examinations. The findings consist of a dilated loop of bowel (the intussuscipiens) with a thickened wall, within which is an eccentric soft tissue mass (the intussusceptum) and an adjacent crescentic low density mass on the mesenteric side, representing the invaginated mesentery.

We report a case of Epstein-Barr virus (EBV)-positive ileal extraosseous
plasmacytoma
containing plasmablastic lymphoma components with CD20-positive lymph node involvement. A 34-year-old healthy Japanese male developed
intussusception
due to an ileal
plasmacytoma
. The lesion was positive for EBV-encoded small nuclear RNA in in situ hybridization, with the surrounding lymph nodes showing the expression of CD20. Tumor cells in the ileal and lymph node lesions contained high-grade malignant features compatible with plasmablastic lymphoma. Because his abdominal lymph nodes recurred 6 months after resection, he received six cycles of R-CHOP chemotherapy (rituximab, cyclophosphamide, doxorubicin, vincristine, and prednisolone), and had a complete remission. Although his case was complicated by acute promyelocytic leukemia, he has so far survived, recurrence-free, for more than 7.5 years after chemotherapy for extraosseous
