Peutz Jeghers (PJ) polyps are rare hamartomatous tumors of the gastrointestinal tract frequently associated with skin and mucosal pigmentation. Despite their benign nature there is a certain increased risk of progression to malignancy in some cases, justifying a sustained follow-up of the patients. We present 3 cases of Peutz Jeghers syndrome (PJS) diagnosed in our hospital on gastrointestinal specimens obtained by endoscopy and opened surgery. We analyzed different degrees of dysplastic changes, epithelial
intussusception
, association with other types of polypoid lesions and other various aspects possibly related with disease progression. Clinico-pathological correlations were made. Two of these cases were related (mother and daughter); both of them were operated in another hospital for small bowel tumors with a subsequent diagnosis of
adenocarcinoma
. The daughter (28 years old) was referred to our hospital for endoscopic follow-up; a small polyp of the transverse large bowel was excised by colonoscopy with a histopathologic diagnosis of PJ polyp; a careful histopathologic reevaluation of both specimens of enterectomy (slides and paraffin blocks) revealed an overdiagnosis of cancer due to the epithelial cystic dilatation and pseudoinvasion in both patients. The other case showed diagnostic changes of PJS and also various aspects of adenomatous polyps some of them with mild and moderate dysplastic changes. When a PJ polyp is diagnosed, the possibility of pseudoinvasion should be kept in mind, in order to avoid overdiagnosis of malignancy; also, due to the fact that the malignant transformation of a PJ polyp is still on debate (hamartoma-dysplasia-carcinoma sequence versus malignant transformation of an adenomatous aria of a hamartoma versus coincidental association of a digestive cancer due to genetic aberrations of PJS), all the other associated microscopic aspects of the lesion should be carefully analyzed.

Small intestine metastasis from primary lung cancer is uncommon and jejunojejunal
intussusception
secondary to metastatic lung cancer is extremely rare. We report a case presenting with a one-week history of abdominal pain associated with poor appetite, vomiting and absent defaecation. Physical examination revealed abdominal distention with decreased bowel sounds. Chest roentgenogram showed a mass lesion in the right upper lung zone. Biopsy of the lung mass lesion confirmed adenocarcinoma of the lung. Computed tomography (CT) of the abdomen demonstrated a "target mass" lesion in the right lower abdomen, representing a small intestinal
intussusception
. Emergency segmental resection of the affected small intestine with jejunojejunal anastomosis was performed. Histological examination of the specimen revealed metastatic
adenocarcinoma
of lung origin. The patient had an uneventful postoperative course and was discharged home two weeks after surgery.

An 86-year-old man was admitted complaining of abdominal pain. A tender hard mass was palpated in the right upper abdomen. On computed tomography,
intussusception
was visualized with a mass about 4 cm in size. Gastrographin enema examination showed typical so-called crab finger's sign findings. The mass was suspected to be cancer of the transverse colon which had led to the
intussusception
. Operation was performed. Histopathologically, the tumor was type 1 and poorly differentiated
adenocarcinoma
.
Intussusception
in adults is rarely caused by transverse colon cancer, with only 18 cases reported in Japan. We discuss our case in light of our review of the literature.

Intestinal metastasis from lung primary is very uncommon and seen at the terminal stage of the disease. Clinically, patients present as perforation or bleeding and rarely as
intussusception
. We report the case of a 78-year-old man who came with sudden onset of abdominal complaints of four to five days duration. A computerized tomography (CT)--scan abdomen showed mural thickening of short loop of jejunum with ileoileal
intussusception
. Resection-anastomosis revealed two separate nodules in the small intestine. The patient, a diagnosed case of primary carcinoma of lung seven months ago, had been treated with one cycle of chemotherapy. Histopathology of the small intestinal nodules showed features of
adenocarcinoma
consistent with the known primary lung cancer. We present this case to arouse a clinical suspicion of intestinal metastasis in known cases of primary lung cancer presenting with the sudden onset of abdominal complaints. Early diagnosis and management improves the survival of these patients.

Other than in childhood,
intussusception
is unusual and nearly always caused by a structural and well demonstrable lesion. In contrast with the colon tract, the incidence of primary malignancies in the small bowel is very low. We report the case of a 51-year-old man presenting with jejunal
intussusception
due to a primary
adenocarcinoma
. To our knowledge, only a few similar cases have been reported in the literature to date. The patient was referred to our division for bowel obstruction. A CT scan showed a jejunal
intussusception
and surgical exploration was hence considered. At laparotomy, jejunal
intussusception
located just after the ligament of Treitz due to a polypoid lesion was confirmed and resection of the first jejunal loop was carried out. Histological examination of the specimen resulted in a diagnosis of a primary
adenocarcinoma
of the small bowel. In adult intestinal
intussusception
, resection without reduction is considered the optimal management if an underlying primary malignancy cannot be excluded.

A 31 year-old Nigerian man with jejuno-jejunal
intussusception
with the lead point being an
adenocarcinoma
complicated by small intestinal volvulus is presented. The subtle clinical features of an underlying small bowel malignancy were masked by the overwhelming clinical and radiological features of
intussusception
. rare case is reported to remind clinicians to have an increased index of suspicion of malignancy in patients who present with the usual features of chronic anemia, weight loss and loss of appetite with an intra-abdominal mass. The presentation of acute intestinal obstruction, with mesenteric vein thrombosis probably due to
intussusception
or volvulus should not however lower the suspicion. Histological evaluation of surgical biopsies is of immense importance.

Intussusception
is a rare cause of intestinal obstruction in adult patients, but is common in children. In fact, it accounts for an estimated 1% of all cases of bowel obstruction in adults, although adult
intussusception
of the large intestine is rare. Sigmoidorectal intussusception, however, is a rare variety with few cases reported in the literature. A mucinous
adenocarcinoma
, a subtype of
adenocarcinoma
, is characterized by extracellular mucin production and accounts for between 5% and 15% of the neoplasms of the colon and rectum. Despite the general consensus supporting surgical resections for adult intussuceptions, controversy remains over whether intussuceptions should be reduced before resection. Most cases of colon
intussusception
should not be reduced before resection because they most likely represent a primary
adenocarcinoma
. However, prior reduction followed by a resection can be considered for the sigmoidorectal
intussusception
to avoid inadvertent low rectal cancer sugery. We experienced one case of sigmoidorectal
intussusception
caused by a mucinous
adenocarcinoma
of the sigmoid colon in a 79-year-old woman. Abdominal computed tomography demonstrated a sigmoidorectal
intussusception
. After the end-to-end anastomosis-dilator-assisted reduction, the patient underwent a laparoscopic oncological anterior resection under the impression that a sigmoidorectal
intussusception
existed. We report a successful laparoscopic anterior resection in a patient with an
intussusception
caused by a sigmoid malignant tumor.

Intussusception
in adults is a rare clinical entity. The individual surgeon will not often encounter this condition. A lead-point for adult
intussusception
exists in 90% of cases and is frequently malignant. We present four cases and discuss treatment and diagnostic modalities. A comprehensive review of the literature is given. All cases were treated by resection according to oncologic principles without previous reduction. Histological examination showed non-Hodgkin lymphoma, lipoma, clear cell sarcoma and
adenocarcinoma
as the lead-points.

The liver is the most common and critical site for the development of colon cancer metastases. Tumor angiogenesis in liver metastasis from colon carcinoma is a controversial subject. Liver microenvironment, immunophenotypical and morphological particularities of hepatic vessels are only few aspects, which establish difficulties in quantification of tumor vascularisation from liver metastasis. The aim of this work is to study the distribution of CD105 positive vessels and the proliferation rate of endothelial cells from liver metastasis of colon carcinoma based on double immunostaining CD105/Ki67. In liver metastasis from well-differentiated
adenocarcinoma
we found a high number of CD105+/Ki67- vessels. On the other hand, in liver metastasis from poorly differentiated
adenocarcinoma
we noticed rare CD105+/Ki67+ vessels. It is hypothesized that neoangiogenesis of liver metastasis is performed through intussusceptive mechanism rather than sprouting and could be supported by the presence of kissing phenomenon, CD105 positive transcapillary pillars and the absence of endothelial cells proliferation in this vessels. We conclude that in liver metastasis principal mechanism of neovascularisation formation is based on
intussusception
.

Intussusception
of the bowel is rather rare in adults and in about 80-90% of the cases is secondary to an underlying intraluminal pathology that serves as a lead point. In cases of colonic
intussusception
malignancy occurs in 63-66% of patients and it is usually
adenocarcinoma
and rarely lymphoma. The presenting symptoms are non specific and are in most cases of long duration, consistent with a chronic
intussusception
causing partial intestinal obstruction. We present a rare case of primary colorectal lymphoma in a 29 years old female that was complicated by acute ileocecal
intussusception
, treated with en bloc resection without reduction.
