Intra-intestinal pressure was recorded with open-tip catheters in experimental low small-
bowel obstruction
in the cat. At zero time the pressure was 2-4 mmHg. After 72 hours sustained intraluminal pressure ranged mainly between 5 and 10 mmHg. Long periods of inactivity alternated with short bouts of spontaneous bowel activity. During activity the pressure rose to about 20 mmHg. In motility responses elicited by
cholecystokinin
, momentary pressure waves amounting to 40-50 mmHg were recorded. The result of simple mechanical obstruction is predominantly a bowel distension, whereas the magnitude of sustained intraluminal pressure is rather modest. Possible effects of distension on intestinal microcirculation are discussed.

We investigated myoelectric activity in an 8-month-old male who presented with a perinatal
bowel obstruction
, duodenal band, congenital short small intestine, and persistent feeding intolerance. Serosal electrodes were surgically implanted on stomach, duodenum, and jejunum during Nissen fundoplication and ileostomy. A 5-cm ileal specimen was taken for in vitro studies. Spontaneous migrating myoelectric complexes (MMC) were present in stomach and small intestine. Bethanechol increased electrical response activity (ERA) in stomach and duodenum. Morphine induced intense ERA and distinct phase III activity. Pentagastrin infusion did not disrupt MMC cycling. Feeding disrupted MMC complex cycling 30-40 min after the meal. Metoclopramide before feeding delayed disruption of the MMC cycling after the feeding. Intermittent gastric arrhythmias were present after the fifth postoperative day. In vitro muscle strips showed spontaneous contractions and electrical control activity (ECA). Bethanechol, McNeil A-343, motilin, and
cholecystokinin
induced contractions, but pentagastrin had no effect. We conclude that in spite of a major clinical motility dysfunction, several of our findings were normal. The abnormalities include short MMC period, absence of disruption of MMC by pentagastrin, and gastric arrhythmias.

The results of our preliminary experience with the gastric balloon program for weight loss in morbidly obese patients are reported. In a pilot project, we measured gastric-acid secretion, gastrin and
cholecystokinin

) levels in ten patients before and during balloon therapy in a study of the impact of the balloon on gastric physiology. Gastric-acid secretion tended to decrease following balloon treatment, while gastrin and
CCK
levels were unchanged suggesting that weight loss is achieved by mechanisms, which are not mediated by gastrin or
CCK
. The balloon program was then expanded to a group of 29 patients who met the criteria. They were followed for a period of 4 months. Average weight loss for the group was 31 +/- 4 pounds for a monthly average of 8 pounds. The main complications were gastric ulcers in four patients and a small-
bowel obstruction
in one patient. Satisfactory weight loss was achieved in 80 per cent of patients, but this benefit must be balanced against a relatively high incidence (17%) of side effects, some of which were quite serious. Therefore, the gastric balloon program should still be considered experimental.
