Imperforate anus is one of the most common causes of
intestinal obstruction
in the newborn and the exact estimation of the level of the blind rectal pouch is of primary importance. We present 11 cases of imperforate anus and demonstrate the accurate location of the rectal pouch by transperineal injection of contrast media. This method shows an advantage in comparison to the classical plain roentgenogram in the inverted position of the baby. No unnecessary colostomies were performed since intraduction of the transperineal injection. The method is simple and there were no complications due to injection 10-15 cc of 20%

. Further studies are not in progress to demonstrate the presence or absence of an associated internal fistula in all cases.

The differential diagnosis of paralytic ileus vs mechanical
intestinal obstruction

is a useful method because a correct decision can be made for or against surgery in doubtful cases.
Gastrografin
is a valuable diagnostic aid in cases with pseudo-obstruction since recurrent obstruction often seems to be a pseudo-obstruction. We doubt if a recurrent obstruction should be treated by any method of intestinopexy.

During the 14 yr from 1965 through 1978, 49 infants presented shortly after birth with
intestinal obstruction
due to impacted meconium. Three of these patients did not have fibrocystic disease. Eight patients were cured by a
Gastrografin
enema. There were 18 patients who had complications that included associated atresia, volvulus, and/or peritonitis. Various operations were done including resection with either primary anastomosis or enterostomy or varieties of the foregoing. Twenty-three babies had the simple uncomplicated form of meconium ileus. Eleven of these underwent resection and six patients died. Twelve patients were treated by laparotomy, ileotomy through a purse-string suture and prolonged irrigations using acetylcysteine. Of this group only one succumbed. This latter course of management is recommended for patients with simple uncomplicated meconium ileus as it involves no resection, no enterostomy, nor any primary anastomosis.

(subileus) occurred in the infant as a result of a neonatal small left colon syndrome. Diagnosis was established by radiological examination. Contrast enema with
Gastrografin
resulted in a permanent curative effect. Follow-up studies showed normalisation of the colonic lumen demonstrating the functional character of the disturbance. The etiology of the syndrome has not yet been clarified. It appears to be of differential diagnostic importance to make a clear distinction between this syndrome and Hirschsprung's disease (congenital megacolon) in newborn, because this involves a different therapy.

Seventeen cases of neonatal
intestinal obstruction
due to inspissated milk curds were seen at this hospital in the 9 years 1964-1972. The obstruction started on the 2nd to 10th day of life. Half the babies passed blood from the rectum and in most the x-rays were diagnostic. All survived, 14 after operation and 3 after medical treatment with a
Gastrografin
enema. Incomplete absorption of solids, particularly the fat from cows' milk feeds, is suggested as the cause.

examination proved reliable in 28 (68 per cent). The reliability of the examination was defined as the relative number of cases in which a correct decision could be made for or against surgical operation. It is concluded that

is a valuable diagnostic aid in controversial cases of ileus. A "negative" result of the examination, however, would never be allowed to damp the surgeon's awareness of possible serious intra-abdominal conditions, which the contrast examination has failed to disclose and which require immediate surgical intervention.

In order to evaluate the Vest-Margulis test in the differential diagnosis between mechanical and paralytic ileus a retrospective study was undertaken on 51 such tests performed before and after operation. Transit of gastrografin (
Sodium amidotrizoate
) through the caecum 4 hours after ingestion was controlled through roentgenograms. The diagnostic value of a positive test in pure paralytic ileus can be confirmed. Indeed 23 positive cases proved to have paralytic rather than mechanical ileus. On the contrary a negative test has no diagnostic value. Twenty-eight patients in which the contrast medium had not reached the caecum within 4 hours had either form of
intestinal obstruction
. The attention is drawn on the interesting therapeutic action of gastrografin in patients with a paralytic ileus and the simplicity of the investigation.

Two newborns with
intestinal obstruction
of the terminal ileum without mucoviscidosis were cured by
Gastrografin
enema. A further case of a premature infant showed, at autopsy, a large intestine containing normal meconium, but an ileum which was plugged with tenacious meconium containing PAS-positive and alcian blue-negative mucus. Our 3 cases were similar to cases of "meconium disease" reported by Rickham and of "meconium plug in the small intestine" reported by Emery. The primary cause of this disease lies in the large amount of PAS-positive mucus secreted from goblet cells of the ileal mucosa and the hypertrophy of lymph nodules in the ileal submucosa.

or other water-soluble contrast materials are far outweighed by their disadvantages, which include the hazards of hypertonic dehydration and the danger of missing the diagnosis of Hirschsprung's disease. Five patients are presented, all of whom had the diagnosis of Hirschsprung's disease missed in the neonatal period with one use of
Gastrografin
enemas. All five were subsequently admitted to the Surgical Neonatal Intensive Care Unit, critically ill with enterocolitis of Hirschsprung's disease.
