Two cases of
lipomatosis
of the ileocaecal valve, causing
intestinal obstruction
are reported. Surgery was performed on an emergency basis. A careful assessment of clinical and radiological findings is important, in order to distinguish such as a disease from caecal carcinoma. In fact this is a benign disease, for which conservative surgery is indicated.

A case of
lipomatosis
of the ileocecal valve causing recurrent
intestinal obstruction
is described. Barium enema with air contrast provides an important diagnostic tool. If confirmed intraoperatively and by histopathologic examination on frozen section, limited resection of the ileum and cecum is advocated rather than blind right hemicolectomy.

Two cases of segmental, small-intestinal
lipomatosis
are described. In both, the patients had crampy abdominal pain secondary to partial
intestinal obstruction
. A review of the literature uncovered reports of 16 similar cases, in some of which virtually the entire small intestine was involved. Diverticulosis and intussusception are associated lesions. Diagnosis may be made by an abnormal roentgenographic pattern. Provided the bowel is not so extensively involved that complete resection is impractical, surgical removal of the affected segment results in cure.

Pelvic lipomatosis is a recently reported, rare entity of uncertain clinical significance. Although it is usually an incidental finding from a gastrointestinal or genitourinary evaluation, various symptoms, usually urologic, have been ascribed to its presence. The authors recently encountered three patients with this condition, however, all of whom had complaints that caused them to seek general surgical consultation. the first patient presented with abdominal pain associated with a pelvic mass; the second developed lower extremity thrombophlebitis; and the third had
bowel obstruction
from colonic cancer obscured by the pelvic
lipomatosis
. As it becomes increasingly apparent that this process may be associated with conditions of importance to the general surgeon, an awareness of its presentation, resultant complications, and the possibility of associated disease processes is required. Thorough, systemic evluation of all these patients is necessary, and exploratory laparotomy may be indicated to establish complete diagnosis.

The cases of two female patients with
lipomatosis
of the ileocecal valve inducing episodes of
intestinal obstruction
are presented. A barium enema with air contrast was performed in patient 1; patient 2 was operated on to treat ileus. Resection of the fatty tissue from the ileocecal valve was performed at operation, after histologic diagnosis on frozen section. These cases suggest that limited resection removing fatty tissue is effective and preferable to more radical resection.

We report a patient who presented with
intestinal obstruction
; his small intestine showed intense segmentary
lipomatosis
associated with unusually situated multiple intussusceptions. Preoperatively, we diagnosed intussusceptions in the ileocecal region by ultrasonography and computed tomographic scan. During surgery, the ileum was dilated and contained numerous movable polypoid masses. One reducible intussusception was encountered in the ileocecal region. In addition, an ileoileal intussusception that could not be reduced was resected with an end-to-end anastomosis. At histological examination, more than 150 submucosal lipomas were found.

A rare case of a segmental small intestinal (jejunal)
lipomatosis
is described. A 33-year-old male was admitted with a clinical diagnosis of an acute
intestinal obstruction
. A plain erect abdominal x-ray showed multiple air-fluid levels. On an exploratory laparotomy, a jejunojejunal intussusception was found secondary to a segmental submucosal
lipomatosis
. This was treated by a segmental resection and anastomosis, which resulted in a complete cure. Here we present this case with a review of the relevant literature.

Intestinal
lipomatosis
is a rare entity and few cases have been reported in the literature. The condition is usually asymptomatic. Symptomatic cases usually present as obstruction or, less frequently, as bleeding. Intestinal barium studies, ultrasonography and computed tomography are useful diagnostic techniques. We present the case of a 47-year-old man with no relevant medical history who presented with
intestinal obstruction
of several months' duration. Complementary investigations yielded a diagnosis of
intestinal obstruction
due to ileocecal invagination secondary to endoluminal tumors of the ileum. Surgery and pathological analysis revealed the latter to be intestinal
lipomatosis
. This rare clinical entity has been associated with diverticulosis and intestinal volvulus.

A patient presented with progressive abdominal distention, discomfort, nausea, vomiting, and constipation. The patient also had congenital macrodactyly of the third, fourth, and fifth digits of the left hand. He [corrected] was diagnosed with macrodactylia fibrolipomatosis with multiple small bowel lipomas and complete small
bowel obstruction
. Macrodactylia fibrolipomatosis is a rare condition characterized by benign fibrofatty infiltration involving the soft tissues of the distal arm or leg with associated dactylomegaly of the associated digits. This patient had a rare presentation that included extensive small bowel
lipomatosis
.

Intestinal
lipomatosis
is a rare disease with an incidence at autopsy ranging from 0.04% to 4.5%. Because the lipomas are diffusely distributed in the intestine, most patients are symptom-free, and invasive intervention is not advised by most doctors. Here, we describe a case with intussusception due to small-bowel
lipomatosis
. Partial small bowel resection and anastomosis were performed because the intestinal wall was on the verge of perforation. This case indicates that regular follow-up is necessary and endoscopic treatment should be considered to avoid surgical procedures if the lipoma is large enough to cause
intestinal obstruction
.
