We investigated the clinical, immunologic, and imaging features of parotid enlargement in 10 children seropositive for human immunodeficiency virus (HIV) who were infected prenatally. In seven patients, sonography revealed many small hypoechoic areas suggestive of lymphoid infiltration. In three older children, large anechoic areas suggesting lymphoepithelial cysts were present. The lesions probably represent generalized lymphoid infiltration akin to pulmonary lymphoid hyperplasia, although only two of our patients had radiologic evidence of pulmonary lymphoid hyperplasia. Four of five of the children with parotid enlargement who had immunologic studies were phenotypically HLA-DR5, an immunophenotype associated with improved immune response, occurring in less than one in four in the normal population. This suggests that painless parotid enlargement may be a good prognostic sign in HIV-seropositive children. Parotid enlargement was found in 10 HIV-positive children from 6 months to 11 years old. All except one were infected prenatally.

In Europe and in the United States, bilateral parotid gland swelling has been observed as a sign of human immunodeficiency virus (HIV) infection in children, but it has not been associated with HIV infection in adults. We observed a chronic parotid gland swelling in nine HIV-seropositive patients during a nine-month period in Kinshasa, Zaire. Parotid gland enlargement was bilateral in seven patients (78%), slightly painful in seven patients (78%), and painless in two patients (22%). No evidence of inflammation was observed around Stensen's duct. One of the two patients in whom a parotid gland biopsy was performed had a malignant lymphoma of the large-cell, histiocytic type. In the other patient, the parotid gland showed normal morphology with minor inflammation. Among 284 adults and 40 children with symptomatic HIV infection, chronic parotid gland enlargement was observed in none of the patients. However, two (0.7%) of the adults presented with an acute pyogenic parotitis. Further studies are needed to determine whether parotid gland enlargement is associated with HIV infection.

Parotid enlargement is common in patients infected with human immunodeficiency virus type 1. We present a case of cytomegalovirus infection of the parotid gland in a patient with the acquired immunodeficiency syndrome. This case is notable because fine-needle aspiration provided the diagnosis. We therefore present a quick and inexpensive method of detecting a potentially fatal infection that has previously been unrecognized by the medical community.

Parotid gland enlargement is common in patients infected with the human immunodeficiency virus. Although parotitis is the usual histopathological feature in such cases, patients with acquired immunodeficiency syndrome are at high risk of developing both lymphoma and Kaposi's sarcoma of the parotid gland. Human immunodeficiency virus, however, is not detected within the parotid parenchyma even in the presence of Kaposi's sarcoma. The pathway of the virus' entry into the saliva remains unknown.
