Objectives Platynosomum species are cat-specific parasitic trematodes that parasitize the biliary ducts and gall bladder. Due to the common connection to the major duodenal papilla of the pancreas and common bile ducts in addition to the periductal proximity of the pancreas, it is possible that platynosomosis could cause pancreatitis. The objective of this study was to determine whether platynosomosis, a commonly diagnosed
parasitic disease
in cats on St Kitts, has any association with pancreatic disease. Methods To investigate this possibility, the pancreas of free-roaming cats with naturally acquired platynosomosis were evaluated via ultrasound, serum concentrations of feline pancreatic lipase (fPL), cobalamin, folate and feline trypsin-like immunoreactivity (fTLI) and histopathology. Twenty free-roaming, young adult, feral cats, positive for feline
immunodeficiency
virus, and diagnosed with Platynosomum species infection via fecal analysis were recruited. The liver, biliary system and pancreas were evaluated via ultrasonography during a short duration anesthesia. Serum concentrations of fPL, fTLI, folate and cobalamin were measured. Sections of the right limb, left limb and body of the pancreas were evaluated histopathologically using hematoxylin and eosin (H&E) stain. Results None of the cats had sufficient criteria to fulfill the ultrasonographic diagnosis of pancreatitis. One cat had an elevated fPL concentration in the range consistent with pancreatitis. Four cats had cobalamin deficiencies and 11 had abnormal folate concentration. The fTLI concentration was equivocal for the diagnosis of exocrine pancreatic insufficiency in one cat. With a single exception, histopathology changes, when present (n = 12), were mild, non-specific and predominantly characterized by lymphocytic infiltrates and fibrosis. The exception was a cat that presented a chronic interstitial and eosinophilic pancreatitis of slightly increased severity, likely the result of platynosomosis. Conclusions and relevance The results of this study suggest that platynosomosis rarely induces pancreatic damage in cats. With only one exception, chronic pancreatitis diagnosed in cats with fluke-induced cholangitis and cholangiohepatitis was subtle and interpreted as an incidental background lesion unrelated to platynosomosis.

Therefore, we determined the prevalence and correlates of Chagas disease in regional and international migrant populations at the Mexico/Guatemala border. Data were collected as part of a larger study of human

virus (HIV) and migration. Participants were a sample of recent regional and international migrants who used an illicit substance or had recent problem drinking.
Trypanosoma cruzi
infection was classified as testing positive on two different enzyme-linked immunosorbent assays (ELISAs). Interviewer-administered surveys captured sociodemographics, migration history, Chagas disease knowledge, and access to care. We enrolled 389 recent migrants, and the prevalence of Chagas disease was 3.1%. Only 19% of the participants reported having ever heard of the disease and less than 1% had been previously tested.

Monitoring response to treatment is a key element in the management of infectious diseases, yet controversies still persist on reliable biomarkers for noninvasive response evaluation. Considering the limitations of invasiveness of most diagnostic procedures and the issue of expression heterogeneity of pathology, molecular imaging is better able to assay in vivo biologic processes noninvasively and quantitatively. However, there are currently no clinical criteria or recommended imaging modalities to objectively evaluate the effectiveness of antimicrobial treatment. Therapeutic effectiveness is currently gauged by the patient's subjective clinical response. Blood transfusion is a common procedure in the hospital setting, and the safety of the blood supply has been vastly improved over the past few decades largely due to improvements in screening for viral transmissible diseases, especially human
immunodeficiency
virus (HIV) and viral hepatitis. However, more recent efforts to improve blood safety have focused on non-transmissible disease risks such as transfusion-related acute lung injury (TRALI), non-viral transmissible diseases such as bacterial contamination of blood products (especially platelet components which are stored at room temperature) and Chagas disease (a
parasitic disease
caused by Trypanosoma cruzi), and prion transmissible agents (e.g., variant Creutzfeldt-Jakob disease, also known as the agent of mad cow disease) as well as more recently-recognized transmissible viral disease risks such as West Nile virus. Appropriate blood utilization has also come under more intense scrutiny in recent times due to healthcare costs and the recognition that many blood transfusions are given under circumstances in which the benefit to the patients is unclear and may be potentially harmful due to the above risks as well as the emerging concept that blood transfusions may cause long-term damage to the immune system resulting in worse patient morbidity and mortality outcomes. Toward that end, accreditation agencies such as the Joint Commission and the American Association of Blood Banks (AABB) are advocating for healthcare organizations to implement appropriate patient blood management strategies. This review will examine these issues along with newer blood safety technological innovations and further highlight contributing studies from our institutions.
