Palpable purpura in a follicular localization developed in association with acute
epididymitis
in a white man who was seropositive for the human
immunodeficiency
virus (HIV). Biopsy specimens revealed a leukocytoclastic vasculitis with follicular accentuation. With antibiotic therapy the vasculitis resolved, but it recurred during repeated episodes of the
epididymitis
. Follicular accentuation of skin disease is often seen in HIV-seropositive patients. Leukocytoclastic vasculitis with a follicular localization may be the presenting skin manifestation of HIV infection.

Between May 1987 and November 1988 we performed the human
immunodeficiency
virus (HIV) antibody serological test on 586 patients of the Urology and Nephrology Services, and it was positive in 14 cases. Of these, 6 came on account of urological pathology: bilateral cryptorchidism, giant condylomata acuminata, acute pyelonephritis and three acute orchi-
epididymitis
. All the patients were intravenous drug addicts. Although it is a case of common urological pathology and not secondary to the acquired immunodeficiency syndrome, it takes on a different significance as regards the risk population in which it occurs.

Sexually transmitted diseases (STD) constitute a frequent presenting complaint. The epidemiology of human
immunodeficiency
virus (HIV) infection is identical to that of STD and must therefore be systematically investigated in the presence of any STD. Chlamydia trachomatis (CT) is involved in the majority of cases of urethritis and
epididymitis
in young subjects and is present in the urethra of 10% of subjects with a genital ulcer. Genital ulcers are due to either Treponema Pallidum, Haemophilus ducreyi, or Herpes simplex virus: there is little clinicobacteriological correlation and it is therefore essential to perform laboratory examinations in order to establish the diagnosis. The prevalence of venereal vegetations due to HPV viruses has increased markedly over recent years and require effective treatment and surveillance because of the risk of carcinoma induced by viral oncogenesis. Other viral diseases such as hepatitis B are also classified as STD. The main diagnostic techniques used at the present time for each STD are reviewed and the consensually accepted therapeutic protocols are also proposed.

The trial entered 49 patients with
epididymitis
and 18 age-matched control subjects without this disease. The patients were divided into 3 groups: 21 patients of group 1 were treated surgically (epididymotomy with drainage of the scrotal cavity), 15 patients of group 2 were inoperable, 13 patients of group 3 suffered from exacerbation of chronic
epididymitis
. Before treatment the patients were examined clinically, bacteriologically and immunologically. Measured were: main populations and subpopulations of lymphocytes using indirect immunofluorescence and monoclonal antibodies; concentration of serum immunoglobulins using radial immunodiffusion; overall content of circulating immune complexes precipitating by polyetilene glycol-6000; phagocytosing neutrophil function by absorption of latex particles. The immunogram of
epididymitis
patients was indicative of the inflammation stage and activity which is important for choice of treatment. Immunogram of group 1 patients was characteristic of the peak of acute inflammation, of group 2 was typical for attenuation of acute inflammation. Immunogram of group 3 patients corresponded to exacerbation of chronic inflammation and secondary
immunodeficiency
.

Sexually transmitted diseases (STDs) affect the physiology of male/female reproduction. Chronic bacterial infection of semen is uncommon, but may be a cause of male infertility. Antibacterial treatment results in improvement in sperm quality, once the infection is eradicated. Little is known about how infection with Mycoplasma hominis affects semen quality, but treatment with antibiotics improves motility and decreases the percentage of coiled tails. Chlamydia trachomatis is not frequently isolated from the urethral cultures of normal men, but is a major cause of nongonococcal urethritis and
epididymitis
. Chlamydia is an important cause of epididymal and oviductal obstruction. Trichomonas vaginalis most frequently colonizes the vagina and cervix of women and the anterior urethra of the male sexual partners. The highest prevalence is in sexually active men and women and Trichomoniasis may well be the most common STD. Syphilis may be an important cofactor in facilitating transmission of the human
immunodeficiency
virus (HIV). A history of syphilis or a positive serologic test for syphilis is associated with HIV seropositivity in men. In South Africa, the seropositivity in pregnant black women ranges from 11-20%. Ga-Rankuwa Hospital is the referral center for 40 peripheral hospitals and over 4 million people. Since the inception of the Andrology Laboratory in June 1985, more than 5300 semen analyses have been performed on 2000 patients.

Genital Chlamydia trachomatis infection is the leading cause of bacterial sexually transmitted disease in industrialised countries, particularly among young people. The consequences of chlamydial infection may involve urethritis, cervicitis, pelvic inflammatory disease, ectopic pregnancy, tubal factor infertility,
epididymitis
and prostatitis. In addition, chlamydial infection increases the risk of acquisition of human
immunodeficiency
virus and has been associated with cervical cancer. Although screening programmes exist in a number of countries, the continuously increasing prevalence of chlamydial infections demonstrates the necessity for health authorities to establish effective screening policies, and the importance of defining a comprehensive European screening policy is emerging.

Symptoms of urethritis in men typically include urethral discharge, penile itching or tingling, and dysuria. A diagnosis can be made if at least one of the following is present: discharge, a positive result on a leukocyte esterase test in first-void urine, or at least 10 white blood cells per high-power field in urine sediment. The primary pathogens associated with urethritis are Chlamydia trachomatis and Neisseria gonorrhoeae. Racial disparities in the prevalence of sexually transmitted infections persist in the United States, with rates of gonorrhea 40 times higher in black adolescent males than in white adolescent males. Recent studies have focused on identifying causes of nongonococcal urethritis and developing testing for atypical organisms, such as Mycoplasma genitalium and Ureaplasma species. Less common pathogens identified in patients with urethritis include Trichomonas species, adenovirus, and herpes simplex virus. History and examination findings can help distinguish urethritis from other urogenital syndromes, such as
epididymitis
, orchitis, and prostatitis. The goals of treatment include alleviating symptoms; preventing complications in the patient and his sexual partners; reducing the transmission of coinfections (particularly human
immunodeficiency
virus); identifying and treating the patient's contacts; and encouraging behavioral changes that will reduce the risk of recurrence. The combination of azithromycin or doxycycline plus ceftriaxone or cefixime is considered first-line empiric therapy in patients with urethritis. Expedited partner treatment, which involves giving patients prescriptions for partners who have not been examined by the physician, is advocated by the Centers for Disease Control and Prevention and has been approved in many states. There is an association between urethritis and an increased human
immunodeficiency
virus concentration in semen.

Gels have been considered as a popular mode of delivering medicament for the treatment of sexually transmitted diseases (STDs) (e.g. human
immunodeficiency
virus, bacterial vaginosis,
epididymitis
, human papillomavirus infection and condylomata acuminata etc.). The present study discusses the development of novel olive oil based emulsion hydrogels (EHs) using sorbitan monopalmitate as the structuring agent. The developed EHs may be tried as drug delivery vehicle for the treatment of STDs. The formation of EHs was confirmed by fluorescence and confocal microscopy. FTIR studies suggested intermolecular hydrogen bonding amongst the components of the EHs. X-ray diffraction study suggested the amorphous nature of the EHs. The developed EHs have shown non-Newtonian flow behavior. The EHs were found to be biocompatible. The formulations were able to effectively deliver two model antimicrobial drugs (e.g. ciprofloxacin and metronidazole), commonly used in the treatment of the STDs.
