Human
immunodeficiency
virus (HIV)/acquired immunodeficiency syndrome (AIDS)-prevention efforts supported by the federal government include programs offered through community-based organizations (CBOs) and state and local health departments (1). To assess the extent of community awareness and use of these HIV/AIDS-prevention services among Hispanics and non-Hispanic blacks in three cities in Connecticut, the Connecticut State Department of Health Services (CSDHS) included questions on HIV/AIDS-prevention programs in its population-based
chronic disease
and health risk survey. This report summarizes survey results regarding awareness and use of these community-based programs during 1991.

States and cities have effectively used community-level intervention projects to reduce cigarette smoking and other risk behaviors associated with
chronic disease
(1-3); similar strategies have been introduced to prevent human
immunodeficiency
virus (HIV) infection in men who have sex with men and who identify themselves as homosexual or bisexual (4,5). Many of these projects used community volunteers to deliver interventions (2,4,5). An important concern for community-level intervention projects that rely on volunteers is the need to address the recruitment and retention of these volunteers. CDC AIDS Community Demonstration Projects have developed and maintained volunteer networks among hard-to-reach populations for HIV prevention. This report summarizes methods used by demonstration projects in five cities to develop and maintain volunteer networks during 1991-1992.

The blastformation tests using concanavalin A and pokeweed mitogen were carried out on peripheral blood lymphocytes obtained from cats infected with feline
immunodeficiency
virus (FIV). The infected cats included those showing multiple
chronic disease
, those with a single mild sign, and one with no clinical sign. The infected cats showed significantly lower mitogenic responses of peripheral lymphocytes to both mitogens and lower lymphocyte counts as compared to uninfected healthy cats. These immunologic alterations in the infected cats may be closely related to the development of
immunodeficiency
-like disorders associated with FIV.

Of 467 cat serums tested for antibody to feline
immunodeficiency
virus (FIV) 120 (26%) were positive. The average age of positive cats was 7.5 years (range 1 to 16 years), and 67% were male. Of 110 serums collected in 1980, 27 (24.5%) were positive. A wide variety of clinical signs including oral cavity disease, anorexia, weight loss, lethargy, depression, fever, respiratory and urinary tract disease, conjunctivitis, abscesses, anaemia and lymphadenopathy were observed in the cats with serum antibody. There was often a history of
chronic disease
or recurrence of particular or various clinical signs in these cats. FIV was isolated from 4 of 8 FIV antibody positive cats by cocultivation of patient lymphocytes with donor lymphocytes in the presence of interleukin 2.

Recently, a newly discovered lentivirus--Feline
Immunodeficiency
Virus (FIV)--was identified as the cause of an AIDS-like syndrome in the cat. For the detection of FIV antigen in cell culture, an antigen capture ELISA was developed. In this assay FIV specific IgG was used as a capture antibody and the same preparation coupled to biotine as a conjugate. With this test viral antigen could be demonstrated in FIV infected cell cultures within four days after infection and the assay proved to be at least as sensitive as a reverse transcriptase test in demonstrating virus infected cultures. A modification of this assay in which a titrated amount of FIV antigen was incubated with dilutions of serum samples of naturally or experimentally infected cats, allowed the detection of FIV specific antibodies. In a serological survey, conducted among cats with different clinical histories in The Netherlands and The Federal Republic of Germany, the antibody inhibition ELISA was compared with a commercially (IDEXX) available indirect ELISA system. It was shown that both assays identified nine seropositive cats among 200 animals with a history of undefined
chronic disease
, whereas no positive reactions were obtained with 65 serum samples from apparently healthy animals. Both assays also gave positive results with serum samples from experimentally infected cats, within five weeks after infection. Of an additional panel of eight serum samples which had been found positive in the IDEXX indirect ELISA system, only five were found positive in the antibody inhibition ELISA and western blotting assay.(ABSTRACT TRUNCATED AT 250 WORDS)

Recent psychoneuroimmunologic findings have suggested that it may be useful to evaluate the influence of behavioral factors on immune functioning and disease progression among human
immunodeficiency
virus-Type 1 (HIV-1) infected individuals. Behavioral interventions with immunomodulatory capabilities may help restore competence and thereby arrest HIV-1 disease promotion at the earliest stages of the infectious continuum. Evidence describing benefits of behavioral interventions such as aerobic exercise training on both psychological and immunological functioning among high-risk HIV-1 seronegative and very early stage seropositive gay men is presented. The HIV-1 infection is cast as a
chronic disease
for which early immunomodulatory behavioral interventions may have important physical and psychological impact.

The prevalence in the Netherlands and the Free Republic of Germany (FRG) of the newly discovered retrovirus of cats, which causes an
immunodeficiency syndrome
in this species and is termed feline T-lymphotropic lentivirus (FTLV), was estimated by conducting a serological survey among cats with different histories of disease. In an enzyme-linked immunosorbent assay (Petcheck, Agritech Trademark, Portland, USA) 265 samples of cats with
chronic disease
symptoms, and 78 samples of clinically healthy cats in the Netherlands, and 138 samples of cats with
chronic disease
symptoms in the FRG, were tested for the presence of FTLV-specific antibodies. All these samples had been taken from cats which were negative for FeLV antigen in the immunofluorescence test. In the groups of chronically ill animals 18 (7%) and 6 (4.5%) seropositive animals were found respectively, whereas in the group of clinically healthy cats no seropositive animals could be demonstrated.

This review of the epidemiology of acquired immunodeficiency syndrome (AIDS) focuses on the AIDS virus, immunology, clinical manifestations, surveillance, epidemiologic studies, and prevention. Although no treatment has been able to reverse the
immunodeficiency
characteristic of AIDS, epidemiologic studies have identified the routes of transmission of this virus and thus suggest strategies for prevention. The number of reported cases of AIDS in the US is increasing rapidly. The 1st 1000 cases were reported over 17 months, the next 2000 were accumulated over 12 months, and an additional 3000 were recorded in only 11 months. 5 groups of patients account for 93% of all AIDS cases: homosexual and bisexual men (73%), intravenous drug abusers (17%), hemophiliacs (1%), sexual contacts of these groups (1%), and transfusion recipients (1%). Opportunistic infections and cancers are not equally distributed among the different risk groups, leading to speculation that a cofactor such as cytomegalovirus infection or exposure to nitrate inhalants may predispose individuals with AIDS to develop Kaposi's sarcoma, for example. Epidemiology has contributed significantly to the understanding and control of AIDS. In addition, AIDS research has contributed to the field of epidemiology, pointing to the need for combined efforts of acute and
chronic disease
epidemiologists as well as coordination with other specialties. Studies of AIDS have also pointed to the need for further epidemiologic research on the health problems of homosexual men, intravenous drug abusere, hemophiliacs, and transfusion recipients.

The sensitivity of mice to mouse hepatitis virus 3 (MHV3) varies according to strain, age, and immune status of the animals. In semisusceptible strains, mice surviving the acute phase of infection develop a
chronic disease
characterized by the occurrence of paralysis, virus persistence, and
immunodeficiency
. Persistent MHV3 infections established in vitro in YAC and RDM -4 mouse lymphoid cell lines were characterized by virus production, presence of cytoplasmic viral antigens, and cell lysis. The occurrence of cell "crisis" in YAC cells was manifested by a sharp increase in cell lysis and in the number of fluorescent cells and, concomitantly, by a marked decrease in virus titers. A relationship was observed among the percentage of fluorescent cells, cell lysis, and virus yield and was modulated by renewal of culture media, change in temperature, or inhibition of cellular RNA synthesis. Cell cloning and antibody treatment experiments indicated that viral transmission was performed by viral infection of newly permissive cells produced by the division of uninfected cells in the culture and not by transmission of viral information by infected dividing cells. The biological and biochemical properties of MHV3 variants derived from persistently infected YAC lymphoid cells were characterized. Thermosensitivity and thermolability of cloned viruses originating from persistently infected YAC cells, as well as parent virus suspensions, were studied. A similar heterogeneity was observed when YAC-derived cloned substrains (YAC-MHV3) were compared with parent-derived cloned viruses, indicating that no selection of temperature-sensitive mutants was induced in persistently infected YAC cells. However, the capacity of MHV3 to induce a lethal acute disease when injected into susceptible mice was lost very rapidly. The absence of pathogenicity was related to the induction of a subclinical infection which elicited defense mechanisms. These data suggest, therefore, that MHV3 replication in lymphoid cell lines leads to induction or selection of variants which maintain pathogenicity in vitro but display reduced pathogenic effects in vivo.

The causes of fever in a child can vary from minor brief illnesses to life-threatening infectious, malignant, or autoimmune diseases. The physician often has to evaluate children with fevers of as yet undiagnosed cause lasting fewer than 2 weeks, in whom it is important to determine whether localizing findings are present. Fever without localizing signs and fevers complicating
chronic disease
and resulting from specific localized infection are considered in the sections concerning infectious causes,
immunodeficiency
diseases, and rheumatic diseases. The diagnostic and therapeutic approaches to the child with both prolonged fever and fever of unknown origin are then discussed, with emphasis on rheumatic diseases.
