We describe the surgical management of 2 patients with staghorn stones in kidneys previously operated upon and an ileal

. Bivalve nephrolithotomy was accomplished in situ using local
hypothermia
and anatrophic nephrotomy. This approach seemed to be the most suitable in these cases because other procedures could have been difficult and risky for the pelvioileo-anastomosis.

Injuries missed at initial operation have the potential to cause the most disastrous complications in trauma patients. Over the past 5 years, 12 patients have required re-operation for 14 injuries missed at initial laparotomy and/or thoracotomy. Six missed injuries were vascular, two each in the thorax, pelvis, and retroperitoneum. The other eight were visceral: three small bowel (one patient), two pancreatic, and one each of the heart,
ureter
, and diaphragm. Five patients (42%) died, three with missed vascular and two with missed visceral injuries. Three died due to complications directly related to their missed injuries, while the unrecognized injury did not play a significant role in the other two. Indications for re-operation in patients with vascular injuries were hypotension in two patients, persistent output from drains in three, and refractory acidosis in one. Re-exploration in visceral injuries was for clinical sepsis in three patients, DIC in one, cardiac tamponade in one, and persistent chest tube drainage in one. Eleven of the 12 patients presented to the E.D. in shock. All patients had multiple injuries with a mean of 3.25 organ systems injured. Hypotension, coagulopathy, and/or
hypothermia
(T less than 92 degrees) were felt to have contributed to missing the injury in five of the patients with vascular, and three of the patients with visceral injuries. Renal preservation and cure of bilateral occurrence of renal tumors or stones as well as occurrence in solitary kidney put all urologists on the thorns of dilemma. Application of extracorporeal surgery resulting in autotransplantation of the kidney for such complicated cases seems useful. Case 1 was a bilateral renal Wilms' tumors in a 17-month-old boy, case 2 and 3 were undiagnosed tumors in renal pelvis and
ureter
. Extracorporeal surgery enables an accurate partial nephrectomy and yet helps to avoid not only the dissemination of tumor cells, but also undesired nephrectomy in case of benign origin. On the contrary, bilateral nephrectomy followed by dialysis or homo-transplantation will result in an immunosuppressive state and the radicality may only be temporary. Besides, the 5-year survival of dialysis therapy is in the range of 60-65%. Thus the indication of autotransplantation was signified in the tumor situation of case 1. For the 2nd and 3rd cases, this method seems to be the best in satisfying the dilemma in discussion. Likewise, the complicated renal calculi could be completely removed without further deterioration of renal function if they were extracorporeally treated and autotransplanted. We proved that 83% of the difficult stones can be completely removed under
hypothermia
in situ. For application of autotransplantation to the calculous disease, we feel observation of contraindication is essential for better results at this stage.

Hamsters have never before been used for experimental kidney transplantation. An original anesthetic and microsurgical protocol is presented for renal transplantation in hamsters based on a series of 152 orthotopic renal transplantations. This operation is performed with the animal under general anesthesia of acepromazine and sodium thiopental. A left nephrectomy is followed by left renal orthotopic transplantation. The transplantation is performed in the following order: anastomosis of the renal artery and aorta, anastomosis of the renal vein to the inferior vena cava, and implantation of the
ureter
in the bladder. The survival rate after contralateral nephrectomy was 71 % in 45 female hamsters. Male hamsters should not be used because of a high risk of severe periureteral stenosis caused by fibrosis secondary to necessary dissection of the large seminal vesicles. The prevention of
hypothermia
in hamsters is mandatory.
