The treatment of massive upper gastrointestinal hemorrhage by gastric hypothermia was studied clinically in 23 patients: five with peptic ulcer, six with multiple gastric erosions, nine with portal hypertension and varices, and three with coagulation defects. Hemorrhage was controlled in 13 of the patients. The high mortality (14 out of 23 patients) was attributed to the severity of the bleeding and to the underlying disease, particularly in patients with liver failure. This form of treatment is a useful method of treating selected patients with upper gastrointestinal hemorrhage.

We report here two rare surgical cases of tetralogy of Fallot with portal venous atresia. Both patients, previously diagnosed with tetralogy of Fallot, developed portal hypertension and showed hematemesis and/or melena. One patient needed endoscopic sclerotherapy. Intracardiac repair was performed under cardiopulmonary bypass (CPB) with moderate hypothermia. After surgery, one of the patients developed cardiogenic shock due to cardiac tamponade with accumulated chyle in the pericardial sac in the ninth postoperative day, and showed deteriorated liver function, suggesting that liver might have been vulnerable because of the absence of portal blood.

Patients undergoing orthotopic liver transplantation (OLT) are susceptible to massive blood loss and require transfusion. Possible reasons for increased transfusion demands include platelet abnormalities, thrombocytopenia secondary to hypersplenism, clotting factor deficiencies, fibrinolysis, increased surgical blood loss associated with portal hypertension and previous surgical procedures, and hypothermia. The purpose of this study was to review trends in blood product usage during our first 6 years of experience performing OLT.
