Of 300 patients who underwent subtotal thyroidectomy, 204 had Graves' disease, 52 solitary toxic goiter and 39 multinodular toxic goiter. Cardiotoxicity was present in 40 cases. Immediate post-operative complications could be avoided by the use of propranolol, neuroleptanalgesia and, in a few very severe cases, hypothermia. Long term results were satisfactory, even after surgery for diffuse hyperthyroidism. Relapses occurred in only 2%, and permanent hypothyroidsm developed within 3 years in only 18% of patients with Graves' disease, this being due to the preservation of 6 grams of thyroid tissue systematically measured by comparative double-weighing. The authors insist on the value of this technique, guess evaluation being unreliable. Cardiotoxicity did not increase post-operative complications, nor the risk of permanent hypothyroidism. The good results obtained should encourage wider surgical indications for the treatment of Graves' disease, solitary toxic goiter or multinodular toxic goiter.

Thyroid dermopathy is not a frequent feature of hyperthyroid Graves' disease, being present in less than 5% of the patients. Graves' disease has been shown to exist in euthyroid or hypothyroid forms in untreated patients. Here, we describe a case of hypothyroid Graves' disease with elephantiasis nostras verrucosa (ENV), which is an extreme form of thyroid dermopathy (TD). A 58-year-old female patient was admitted to the emergency department with somnolence, hypothermia, and bradycardia. Her mental status gradually worsened, resulting in a deep coma. She was intubated and followed in the intensive care unit, as she needed mechanical ventilatory assistance due to respiratory failure. She also had bilateral non-pitting edema, a cobblestone-like appearance, and hyperkeratotic greenish-brown-colored lesions in the pretibial and dorsal regions of the feet that were compatible with ENV. Hypothyroid Graves' disease is a very rare condition among autoimmune thyroid disorders, and ENV is an extremely rare form of TD. Here, we present a patient with hypothyroid Graves' disease and ENV.

A 56-year-old woman with Graves' disease presented with the complaints of diarrhea and palpitations. Physical examination and laboratory data revealed hypothermia and signs of mild hyperthyroidism, heart failure, hepatic dysfunction with jaundice, hypoglycemia, and lactic acidosis. The patient was diagnosed as having developed the complication of thyroid storm in the absence of marked elevation of the thyroid hormone levels, because of the potential hepatic and cardiac dysfunctions caused by heavy alcohol drinking. A year later, after successful treatment, the patient remains well without any clinical evidence of heart failure or hepatic dysfunction. Thyroid storm associated with lactic acidosis and hypothermia is a serious condition and has rarely been reported. Prompt treatment is essential even if the serum thyroid hormone levels are not markedly elevated. We present a report about this patient, as her life could eventually be saved.
