Although the figure of unreported cases of neglected children is much higher than that of physical ill-treatment of children, neglect has rarely received attention, in most cases only when traces of physical ill-treatment were proven at the same time. Symptomatic are in the first place intense emaciation as well as dry puckered, scaly and extremely dirty skin, lack of subcutaneous fatty tissue and of Bichat's fat pad, matted hair, aged face, sunken eyes, and
eczema
from urine with ulcerations in the buttock and thigh regions. In addition one finds signs of localized
hypothermia
or rickets. My own observations cover 54 cases in which neglect and malnutrition have caused the death of the children involved. All the children had been living in extremely bad social conditions. The backgrounds of the children's mothers were also socially poor. Most of the mothers were too young to fulfill their duties. The fathers, where known, were alcoholics, unwilling to work and seldom cared for their family. They too were often too young to cope with their role as father. In this day and age, which has provided us with a much better understanding of the importance of the early development of the child for its later social attitude, mothers with all their problems and difficulties should not be left on their own. Motherlike behaviour is not necessarily programmed with the birth of a child. Mother duties must be learned as early as possible. This process should begin during pregnancy at the latest. A few proposals will be made.

Early treatment reportedly prevents neurological disturbance in patients with Menkes disease. To promote early diagnosis, a questionnaire concerning symptoms during the neonatal period and at the age of 1 month was sent to the mothers of 14 patients with Menkes disease. The height, body weight and head circumference were almost normal in most of the Menkes patients. At the age of one month, all patients had abnormal hair. The mothers noticed it in 8 cases, and in the remaining 6 cases, the abnormalities were confirmed by the photos at the age of one month. At this age, the mothers noticed weak activity in 5, jaundice in 5, dyspnea in 4 cases, feeding disturbance in 3,
hypothermia
in 2 and
eczema
in 2. These results suggest that recognizing hair abnormalities at a one-month check-up may provide a clue to the early diagnosis of Menkes disease.

Common variable immunodeficiency is a heterogeneous group of disorders with arthritis and/or arthralgia as its most commonly associated autoimmune manifestation. We report 2 cases of common variable immunodeficiency in siblings who also had other unusual signs and symptoms. A 10-year-old boy suffered from bradycardia,
hypothermia
, hypoglycemia, and chronic
eczema
. His 13-year-old sister suffered from Kikuchi lymphadenitis, hypoperfusion and atrophy of the left cerebral hemisphere, and hemiparesis. They both showed classical laboratory findings of common variable immunodeficiency and the boy's associated symptoms responded to intravenous immunoglobulin therapy. The findings from these cases suggest that a defect in the neuro-endocrine-immune axis may be one of the genetic bases of common variable immunodeficiency.
