Diarrhea is common in the acute care setting, particularly among critically ill patients. Factors that cause diarrhea are usually multifactorial; some of the most common include medications, hyperosmolar or rapidly delivered tube feedings, atrophy of intestinal epithelium or ischemic bowel, short bowel syndrome,
pseudomembranous colitis
, infection (Salmonella and Shigella species), opportunistic infections in patients with acquired immunodeficiency syndrome and severe
hypoproteinemia
. This article reviews different types and mechanisms of diarrhea commonly encountered in acute care. It includes current concepts of managing diarrhea, such as calculation of stool osmotic gap, identification of medications that cause diarrhea, modification of enteral therapy, and the use of antisecretory agents. Nursing responsibilities and contributions in the collaborative assessment and clinical management of diarrhea are also explored.

OBJECTIVE: To alert about the
pseudomembranous colitis
in children, a consequence of the use of antibiotics. METHODS: This report is the result of the clinical follow-up of a patient with chronic diarrhea after the use of several antibiotics. The bibliography was obtained through Medline system. RESULTS: Case report of a girl two years and seven months old, previously healthy, with a clinical picture of chronic diarrhea with dysenteric characteristics after the use of antibiotics, associated with important weight lost and
hypoproteinemia
. The diagnosis of pseudomembranous was established clinically and was confirmed by colonoscopy and the presence of Clostridium difficile toxin A in the stools. CONCLUSIONS: It is necessary to consider the C. difficile infection in any child with diarrhea associated to the use of antibiotics. The antibiotics in pediatric patients should always be prescribed with caution and precise indications.

Protein-losing enteropathy (PLE) is characterized by loss of serum proteins into the gastrointestinal tract. It may lead to
hypoproteinemia
and clinically present as protein deficiency edema, ascites, pleural or pericardial effusion and/or malnutrition. In most cases the site of protein loss is the small intestine. Here we present an unusual case of severe PLE in a 55-year old female with a one-year history of recurrent diarrhea, crampy abdominal pain, and peripheral edema. Endoscopy and MRI showed a diffuse inflammatory thickening of the sigmoid colon and the rectum. Surgical resection of the involved colon was performed and the symptoms were significantly resolved. The final histologic evaluation confirmed a diagnosis of a
pseudomembranous colitis
with cap polyposis-like features. Such a cause of PLE has never been described before.
