The purpose of this pilot study was to describe the 3-year experience of ambulatory medical care provided by one family physician in an Army community hospital. Data collected at each outpatient visit included the patient's age, race, gender, problems actively addressed, and procedures. There were 7,895 outpatient visits, representing 3,665 patients and 2,292 families. Of the 13,158 problems recorded, the 10 most frequent were pregnancy care, general medical examination, alcohol and tobacco use, hypertension, depression/anxiety, contraception, otitis media, acute upper respiratory infection, vaginitis, and headache. Further studies should be done to validate this pilot study.

AFR 35-11, dated 10 April 1985, included standards for physical fitness performance tests with the option of a 1.5-mile run or a 3-mile walk. Since that time, ANG units have begun initial physical fitness testing of all personnel. This program brought with it new responsibilities for ANG medical units including the screening of individuals in whom health problems might indicate that they are at risk in taking the physical fitness test (PFT). The 111th TAC Clinic used a questionnaire, screened by physicians utilizing a predetermined grid of responses to designate individuals cleared for the run or walk test or as at risk. Of 823 individuals screened, 91 (11%) were designated at risk. These individuals are being further evaluated and 31 (29%) have been subsequently cleared to date. Physical testing of cleared individuals was accomplished, and both running and walking courses were carefully monitored by ambulance crews. Four casualties came to medical attention (blisters, severe fatigue, dizziness, and indigestion). The patient with severe fatigue was a patient on medical hold, S/P myocardial infarction, who had not been medically cleared to participate. The patient with light-headedness was found to have newly diagnosed hypertension. In view of the relatively small number of casualties incurred during this initial PFT, it is felt that the screening process employing a questionnaire evaluated by medical personnel is an appropriate method of minimizing risk.

This study further evaluated the adequacy of a brief Health Promotion Tracking Form (HPTF) for collecting data during the periodic physical examination required of all Navy personnel. Responses on the HPTF were compared to other recent Navy studies. Overall, HPTF indicators were very similar to results from other, more extensive studies. Tobacco use, back problem prevalence, body fat, hypertension prevalence, cholesterol levels, and elevated cholesterol rates closely resembled those from other recent studies. Seatbelt use, several dietary habits, alcohol consumption, exercise behavior, and Physical Readiness Test performance showed some differences in that the HPTF sample reported slightly more positive health status and practices in these areas than did comparison samples. Overall, the proposed system appears to be a workable option for providing accurate and timely Navy health promotion data on a continuing basis.

Cimino fistula is an anastomosis created for chronic hemodialysis, between the radial artery and cephalic vein near the wrist. Its complications include thrombosis (most frequent), infection, congestive heart failure, arterial insufficiency, venous hypertension, vascular access neuropathy, and aneurysms. Of these, venous hypertension appears to occur rarely. We report here a case where it resulted in pigmentation, swelling, and ulceration of the middle finger. It was surgically treated, without ligating the arteriovenous fistula, by selective ligation of tributaries from the affected fingers.

The association of noise-induced hearing loss from gunfire noise with the occurrence of elevated blood pressure was investigated with screening audiometric measurement results and a questionnaire in a randomly selected group of 699 army officers. After age correction, hearing was found to be deteriorated (i.e., more than 20 dB at all of the measured frequencies) in 352 (50.4%) subjects. Of the 699 officers, 535 (76.5%) reported normal and 164 (23.5%) reported elevated blood pressure. In 122 (17.5%), elevation was permanent and verified by a physician. The occurrence of elevated blood pressure showed a highly significant correlation with severity of hearing loss (chi-square = 25.4; p < 0.001), but not with exposure to gunfire noise. Self-reported hypertension seemed to be associated with severity of impulse noise-induced hearing loss, but not with the amount of gunfire noise exposure. Thus, the present study does not support the suggestion that there exists a simultaneous correlation between impulse noise exposure, noise-induced hearing loss, and self-reported elevated blood pressure.

The purpose of the present study was to delineate a health profile of professional Danish army personnel. Two-hundred twenty officers, noncommissioned officers, and gunners on active duty at Varde Barracks, housing the South Jutland Artillery Regiment and the Danish Army Artillery School, were asked about their physical and psychological health, interpersonal relations, and working conditions as well as their dietary, drinking, and smoking habits. Measurements were made of resting pulse rate, blood pressure, height, weight, waist and hip girth, and pulmonary function. The ratio of waist-to-hip girth and body mass index (BMI) were calculated. Psychological well-being was evaluated using the 12-item version of the General Health Questionnaire (GHQ). Psychosomatic symptoms were frequently reported, but very few of those surveyed appeared to have psychiatric disorders as measured by the GHQ. Also, somatic health problems were frequently reported, the most frequent being lower-back pain, mild chest pain, and sensory disorders. Differences in interpretation and reporting of "lasting health problems" may explain the relatively high score for this question. The interpersonal relations, both upward and downward in the hierarchy rank order, received high scores. Compared with the general population, alcohol consumption was very low, whereas smoking-in particular heavy smoking-was much more frequent among professional Danish army personnel. Lung function testing showed significantly poorer mean values of forced expiratory volume in 1st second of expiration and mean forced expiratory flow 25 to 75% of forced vital capacity among smokers compared with nonsmokers, although the mean values for the whole group of both smokers and nonsmokers were well above reference values for all lung function parameters. The frequency of moderately overweight individuals (25 < BMI < or = 30) was significantly higher among the male army personnel than in the general population, whereas this was not the case for obesity (BMI > 30). Abdominal obesity, regarded as an independent risk factor for the development of ischemic heart disease, stroke, diabetes, hypertension, and all-cause mortality, was present in 5%, and 3% belonged to the highest-risk group by having a low BMI as well as abdominal obesity.

Beginning in 1990, the Department of Psychiatry. Tripler Army Medical Center developed a formal treatment program for post-traumatic stress disorder (PTSD). Between 1990 and 1996, 632 patients, the vast majority of whom suffered from combat-related PTSD, were treated. Historically, many PTSD patients were treated with benzodiazepines, often in high dosages. The risks attendant to benzodiazepine management of PTSD, coupled with poor clinical outcome, prompted the staff to explore treatment alternatives. This paper describes the role of pharmacotherapy in the management of PTSD. The medications described in this paper have other primary uses in clinical practice (e.g., hypertension, insomnia, seizure control, depression, and anxiety). Medications were selected for use based on the putative modes of action and the degree of symptom relief. The therapeutic rationale was to decrease hyperarousal and sleep disturbance to permit the patients to engage in other psychotherapeutic efforts.

This case study describes myocardial ischemia and stunning after the topical application of phenylephrine-soaked pledgets (0.25%) in a 63-year-old female undergoing elective endoscopic sinus surgery. The patient had no previous history of cardiovascular disease or illicit drug use. Transient myocardial ischemia was associated with acute hypertension, chest pain, and S-T segment changes 4 minutes after pledget placement. Angiography revealed normal coronary blood flow and severe left ventricular systolic and diastolic dysfunction. Follow-up echocardiography demonstrated improved left ventricular function within 1 week and total resolution of dysfunction by 4 weeks after ischemia.

This study examines long-term health and physical readiness trends in the U.S. Navy. We mailed lifestyle questionnaires to all participants in baseline studies between 1983 and 1989 who were still on active duty in 1994. Commands provided body composition and physical readiness test scores for the participants. Two longitudinal cohorts were created: an 8-year sample (N = 640) with matched data from 1986, 1989, and 1994; and an 11-year sample (N = 1,576), with data from 1983 and 1994. Analyses of both cohorts revealed significant improvements in cardiovascular fitness, muscle strength, exercise, lean body mass, dietary habits, and sleep, as well as significant decreases in tobacco and alcohol use and job stress. However, hypertension rates, percentage of body fat, and body mass index increased over time. Women's scores were significantly better than men's on a number of factors. Overall, these findings suggest that the Navy's health promotion efforts have had a significant positive effect on the fitness and health behaviors of career Navy men and women.

Dietitians are multifunctional and play an important role in humanitarian missions as educators, planners, and consultants. Three dietitians deployed to Thailand in support of the 16th Annual Joint and Combined Exercise, Cobra Gold 1997. The goal of the Medical Civic Assistance Program (MEDCAP) was to promote long-term public health improvements in rural Thai villages. The dietitians counseled 140 patients and taught an additional 5,300 individuals during nutrition classes. The primary nutrition-related clinical diagnoses included malnutrition, anemia, diabetes, hypertension, goiter, and poor appetite. The dietitian who deployed as the medical planner and MEDCAP executive officer facilitated coordination and planning for all phases of the MEDCAP operation. The teams were made up of U.S. and Thai military forces and Thai civilian medical personnel. The mission requirements were established with the Royal Thai Supreme Command, Thai governors, Ministry of Public Health officers, military and medical officers, and veterinarians of the three provinces.
