Obstructive pathologies of the pulmonary tract may cause various levels of hypoxia. To compensate for the hypoxia, pulmonary arterial pressure and pulmonary arterial flow may increase. We investigated 35 patients with seasonal allergic rhinitis (AR) whether hypoxia caused by AR with a high level of obstruction in the airways may lead to an increased pulmonary arterial pressure. An echocardiographical evaluation was made following the determination of the symptomatic and non-symptomatic symptom scores. We found a positive correlation between the symptom scores both in the symptomatic and non-symptomatic periods, nasal obstruction scores and the mean pulmonary arterial pressures during these periods. Further studies with more cases are needed in order to determine the cardiac effects of hypoxia in AR, mainly pulmonary arterial

Data on the prevalence of
Hypertension
(
HTN
) among elderly Asians is limited. We investigated the prevalence of elevated blood pressure (EBP) and its risk factors in a multiracial Southeast Asian elderly population who participated in the National Kidney Foundation Singapore nationwide screening programme. Among 19,848 subjects > or =65 years (approximately 6% of the total Singapore population), the mean age was 70.6 +/- 5.26 yrs. After excluding 36.6% with known
HTN
, analysis revealed that 5,889 (46.8%) of the remaining population had previously undetected EBP > 140/90. Increasing age, male gender, BMI > or =23 kg/m2 and pre-existing diabetes were significantly associated with previously undetected EBP on multivariate analysis. 6% of cases with undetected EBP had proteinuria suggestive of longstanding EBP and renal damage. We conclude that there is a high prevalence of undetected EBP in elderly Asians, suggesting the need for increased efforts in screening in the elderly population.

Cardiovascular disease has become the first cause of death in Indonesia. The highest morbidity is found in the aged, and among cardiovascular disorders or diseases, the prevalence of
hypertension
is the highest. Many studies of the relationship between nutritional factors and
hypertension
have been conducted, especially with reference to the metabolic syndrome, but studies to understand determinants of blood pressure in Indonesia are lacking. There is an urgent need to gather information about various blood pressure risk factors in Indonesian elderly, which will allow policy makers to provide appropriate intervention programs. The primary purpose of this study was to investigate various determinants of blood pressure in Indonesian elderly using multistage random sampling. Data were collected through interview using structured questionnaires, anthropometric measurements, biochemical blood analysis, and blood pressure measurements. Daily nutrient intake was analyzed using the World Food 2 Dietary Assessment Program. General Linear Model and Multiple linear regression analysis were performed to determine determinants of systolic and diastolic blood pressure. Monounsaturated fatty acid, saturated fatty acid, and sodium intake, plasma total cholesterol level, the ratio of total cholesterol to HDL-cholesterol and a sport Index were determinants of blood pressure in the normal weight elderly individuals, while potassium intake, calcium intake and BMI were determination of blood pressure in the overweight elderly individuals.

This study was designed to gauge the attitude and practice towards monitoring of blood pressure among dentists in Fiji and to assess whether they were willing to participate in a national health campaign to screen blood pressure of dental patient without remuneration. Sixty-two questionnaires were mailed out to all General Dental Practitioners in the Republic of the Fiji Islands. Responses were obtained from 49 (79 %) dentists following a second reminder letter. The majority of the practitioners (85 %) had been taught to measure blood pressure as undergraduates and approximately two-third of them stated that blood pressure screening in dental office was very important. Only about one-third of the respondents reported measuring blood pressure routinely, however this figure rose dramatically to 92 % when assessing patients with known
high blood pressure
. Approximately two-thirds of the practitioners were willing to participate in a national health campaign to screen blood pressure of dental patients without remuneration. Most of the dental practitioners in Fiji acknowledged that blood pressure monitoring in dental office was very important. They also stated that additional training in blood pressure monitoring was of necessary. Therefore, it is essential for stakeholders such as the Fiji Dental Association and the Ministry of Health to assist in providing guidelines and training in blood pressure monitoring for dental professionals in Fiji as well as to assist in conducting national health campaigns to screen for blood pressure.

The widespread use of Kura/Noni came to the attention of researchers from both the University of the South Pacific and the Fiji School of Medicine. Amongst other associated research undertakings to better understand the use and potential benefits of this herbal medicine, a nationwide qualitative research was undertaken from September 2002-February 2004. Interviews, participant observation and participatory activities involved over 400 respondents. These research activities showed Kura is thought to be beneficial for a large number of ailments that ranged from skin conditions to
high blood pressure
. In total Kura has been reported to be useful for 66 medical conditions and 1 spiritual application. Based on the findings of the Research Team, it is concluded that there is sufficient grounds to proceed further with the next two phases of this research project: biochemical analysis to identify the active ingredients in the different parts of the plant, and later clinical trials to determine opportunities for developing pharmaceutical drugs.

To determine the influence of personal medical history on mortality from cancer and other chronic diseases, participants in the JACC study were questionned and followed up. Consistent across the sexes, risk of deaths of all causes was increased with
hypertension
, diabetes mellitus, apoplexy, liver diseases, surgical operations, and blood transfusions. All cancers were similarly related to a history of diabetes and liver diseases, surgery and transfusions. In addition, risk of liver cancer was elevated with diabetes, liver disease, cholecystectomy, renal disease, surgical operations and blood transfusions. Apoplexy was related to a past history of blood transfusion and diabetes, the latter also predisposing to ischemic heart disease. Links with infectious disease were also elucidated. Clearly, a past medical history can exert a strong influence on chronic disease development.

To determine the influence of family medical history on mortality from cancer and other chronic diseases, participants in the JACC study were questioned about tuberculosis, apoplexy,
hypertension
, heart disease, diabetes mellitus and malignant diseases and followed up. Consistent across the sexes, a family history of tuberculosis was apparently linked to reduced risk of lung cancer, and also lowered ischemic heart disease. No consistent findings were obtained for
hypertension
, diabetes mellitus or cancer.

A number of characteristics, including body mass index (BMI), blood pressure and childhood location and educational attainment were compared for their effects on mortality in the apan Collaborative Cohort Study (JACC Study). The lowest body mass index was associated with a higher risk of death overall in both sexes, but the highest body mass index was also associated with a rise in risk in females. A low BMI was also linked with certain cancers, for example in the lung and oesophagus. A higher body mass index was associated with a higher risk of death due to ischemic heart disease.
High blood pressure
was also adversely linked to mortality, especially from ischemic heart and cerebrovascular diseases. The childhood environment also appeared to play a role, living in a city being associated with higher risk of mortality overall and from cancer. A high educational level was found to decrease the risk of total death and of cancer, particularly the lung and liver.

In addition to the burden of undernutrition, many recent studies in Vietnam demonstrate that overweight/obesity and a number of nutritionally- related chronic diseases (NRCD) (
hypertension
, diabetes, cardio-vascular diseases) are on the rise at an alarming rate. This could be the result of dietary and life style changes. The double burden of malnutrition (DBM) - a typical phenomenon during nutrition transition in developing countries, has been clearly reported in our country. Controlling nutritional deficiencies and newly emerging nutrition problems to reduce the double burden of malnutrition in Vietnam requires comprehensive and appropriate measures as set out in the National Nutrition Strategy 2001 - 2010, as well as a greater research effort on transitional nutrition.

The metabolic syndrome is increasingly prevalent in worldwide. The quality and quantity of dietary lipids could be important modulators associated with the cardiovascular morbidity and mortality. At present, functional lipids such as conjugated linoleic acid (CLA) and phospholipids have attracted considerable attention because of their beneficial biological effects in attenuating metabolic syndrome. Supplementation of CLA reduces abdominal white adipose tissues, serum triacylglycerol (TAG) level, and liver TAG level in obese Otsuka Long-Evans Tokushima Fatty OLETF rats. These effects were attributed to enhanced fatty acid beta-oxidation and suppressed fatty acid synthesis in the liver. In addition, CLA enhanced energy expenditure in these rats. Anti-hypertensive properties of CLA have also been demonstrated. In obese/diabetic OLETF and Zucker rats, feeding of CLA prevented the development of obesity-induced
hypertension
. This was associated with an altered production of physiologically active adipocytokines, such as adiponectin, leptin and angiotensinogen. In addition, CLA could alleviate the development of insulin resistance and fatty liver. Dietary phospholipids have physiological functions that are different to dietary TAG. We recently reported that phosphatidylcholine (PC) alleviated orotic acid-induced fatty-liver through the suppression of hepatic lipogenesis in rats, and omega 3-PC from salmon roe prevented the development of obesity-related diseases through the suppression of lipogenic gene expressions and the enhancement of lypolytic gene expressions in the liver of obese rats. However, reports which studying the nutritional functions of minor phospholipids, such as phosphatidylinositol (PI), are scarce. Our study indicated that dietary PI lowered lipids in the plasma and liver by suppressing hepatic TAG synthesis.
