Overdose of pentazocine (Talwin), an agonist/antagonist opioid analgesic, is relatively uncommon. Fifty-seven cases occurring over ten years are reported. Twenty-three patients (40%) had ingested only pentazocine and did not have the classic opioid toxidrome of CNS and respiratory depression with miosis. Most patients were awake, and no patient had a respiratory rate below 12/minute. Other findings included: grand mal seizures, hypertension, hypotonia, dysphoria, hallucinations, delusions, and agitation. Eleven of 23 patients received IV naloxone (0.4-2.4 mg), but only two showed improvement. Thirty-four patients (60%) had coingested pentazocine with one to five additional substances. Patients who had ingested pentazocine with alcohol, a sedative/hypnotic drug, or an antihistamine, showed increased toxicity, including apnea, deep coma, and recurrent seizures. One patient developed opioid pulmonary edema. One patient died. Three of five patients with coma and inadequate respirations responded to IV naloxone in doses of 0.4 to 1.2 mg.

We present the case of a 27-year-old woman with a history of drug abuse. Following her routine intravenous (IV) injection of solubilized pentazocine and tripelennamine tablets ("Ts and Blues"), the patient developed severe hypertension, a finding not characteristic of either drug alone or of the combination. The manufacturers of Talwin (pentazocine) recently have added naloxone to the tablets to discourage IV abuse of this oral preparation. Our patient unknowingly had injected the new pentazocine formulation, and she subsequently developed narcotic withdrawal symptoms. Her hypertension was treated, and she was discharged from the emergency department. We report the case as an "unusual reaction" that may develop in frequent abusers of pentazocine and its combinations.

Three patients with a 2-3 year history of parenteral abuse of pentazocine hydrochloride (Talwin) developed membranoproliferative glomerulonephritis type I. Clinical presentation included nephrotic syndrome, microscopic hematuria, and hypertension. Tissue studies disclosed 1+ to 4+ mesangial and subendothelial deposits of immunoglobulins (predominantly IgM) and complement components C1Q, C3 and C4 with focal reduplications of glomerular basement membranes. Glomerular C3 receptor studies performed on one patient demonstrated diffuse loss of receptor activity correlating with localization of immune deposits. Serum studies showed decreased levels of C3, elevated levels of IgM (greater than 2 SD) (three patients) and circulating immune complexes (two patients). Repeated blood and urine cultures were negative. Immune mechanisms may be involved in the pathogenesis of Talwin addict nephropathy.
