The efficacy of the short-term psychotherapy was studied in 90 patients with myocardial infarction using the rating scale and the psychological Mini-Mult test. The hypnosuggestive therapy improved the mental status of the patients by reducing affective disorders. It promoted the formation of adequate psychological reaction to the illness and successful psychological adaptation, exerted a favourable effect on angina pectoris,
cardialgia
and moderate arterial
hypertension
.

Initial manifestations of cerebrovascular pathology (CVP) in the presence of arterial
hypertension
(n-26) and atherosclerosis (n-428) were combined with various cardiovascular disorders ranging from
cardialgia
to coronary heart disease in 67.4% of the patients. Cardial disturbances were expressed as an atypical pain syndrome in 47% and as angina of effort and postinfarction cardiosclerosis in 23.2% of the patients. Electrocardiographic changes were elicited in 52.6-62% of the patients. Bicycle ergometry revealed a decrease in all parameters of tolerance to physical exercise. Disorders of the general and cerebral hemodynamics were more pronounced in atherosclerotic patients; at the first stages of cerebral pathology they predominantly presented the normokinetic type of the hemodynamics (40.6%) while its more marked forms were associated with the hypokinetic type of the circulation (52). The early diagnosis of cardial and hemodynamic disorders is necessary for the presention of acute disturbances of the cerebral circulation.

A study was made of exercise tolerance and indices of central hemodynamics in 151 patients with "isolated" angina and its combination with
cardialgia
, arterial
hypertension
and postinfarctional cardiosclerosis. Daily attacks of angina of effort (III functional class) were noted in all the patients. In patients with "isolated" angina and angina in postinfarctional cardiosclerosis exercise tolerance was the lowest. It was the highest in angina combined with
cardialgia
. The lowest indices of cardiac output were observed in patients with "isolated" angina and angina in postinfarctional cardiosclerosis. The best effect of nitrate therapy was obtained in patients with typical angina.

200 years have gone by since the autonomic disturbance in diabetes mellitus has been described for the first time ever. There is a great deal of information on the close relationship between somatic and visceral symptoms in diabetic polyneuropathy (PNP), so that there should be talked about only of one form of manifestations within the meaning of a distal symmetric somatovisceral PNP. The longer fibres such as the vagal fibres of the viscus, sympathetic fibres of the eye are affected at first and more intensively in the autonomic region same as in the sensory and motor region. Due to the fact that for reasons of fragmentary knowledge pathogenetically substantiated classification of the autonomic disturbances in diabetic PNP is not at hand, such a classification is being made from organotopic and phenomenologic aspects. Frequently, afferent denervation of an organ results in enhancing the effects of an autonomic innervation dysfunction, as for instance in unnoticed hypoglycaemia, in order to modify the symptoms, as for instance in rectal incontinence with unnoticed defecation, or rather to let new symptoms appear, for instance loss of testicular pain. In recent years, appropriate methods of examinations were tested for the clinical routine, permitting to give evidence of autonomic dysfunctions before clinical manifestation. It is still unclear to what extent such subclinical abnormalities are reversible with a more favourable regulation of the metabolic process, for instance with the aid of continuous subcutaneous insulin injections. An impressive symptom of innervation dysfunctions of the cardiovascular system is orthostatic
hypertension
that may, in exceptional cases, even lead to confinement to bed. The most important pathogenic factor seems to be vascular denervation. A pronounced tachycardia at rest, frequently found in diabetics, is the result of the failure of the vagal autonomic system, and, after additional destruction of the sympathetic fibres, it adjusts itself to a lower level that cannot be changed by reflex mechanisms.
Cardialgia
absent in the case of myocardial ischemia is a factor of an increased mortality of long-term diabetics. The correlation between vascular denervation and arteriosclerosis or mediasclerosis, respectively, is being under discussion. Denervation on the gastrointestinal tract has an effect on the motility and excretory functions. The innvervation dysfunctions lead to sialadenosis by changing the composition of saliva. In most cases esophageal dysfunction is not perceived by the patient.(ABSTRACT TRUNCATED AT 400 WORDS)

Twenty-six patients with arterial
hypertension
and chest pain were examined using echocardiography, bicycle ergometry, pharmacological tests with the intravenous administration of dipiridamol and isoproterenol, coronary angiography and left ventriculography. With regard to the painful syndrome the patients were divided into 2 groups: Group 1 with angina, Group 2 with
cardialgia
. The majority of the group 1 patients showed extensive atherosclerosis of the coronary arteries. The results of bicycle ergometry and pharmacological tests in this group were positive in the majority of cases. The group 2 patients displayed minor atherosclerosis of the coronary arteries or intact arteries which correlated with negative results of the bicycle ergometry and pharmacological tests. Echocardiographic and ventriculographic examinations of the functional state of the left ventricle revealed circulation insufficiency in none of the patients.

Detailed clinicofunctional examination of anginal patients with NIDDM using variation pulsometry and electrophysiological cardiac tests in 110 patients (89 females and 21 males aged 36-79 years, mean 64.3 +/- 5.2 years) has revealed that atypical
cardialgia
, arterial
hypertension
, obesity was present in 28.1, 76.4 and 85.4% of anginal patients with NIDDM. Sympathicotonia was registered in autonomic heart regulation of 83% of them. Anginal patients exhibited also suppression of sinus node automatism and sinoatrial conduction. When NIDDM is severe, pacemaker activity of the sinus node intensified suggesting development of cardial diabetic neuropathy.

This study was designed to compare quality of life (QL) in women with arterial
hypertension
(AH) and with normal arterial pressure in the climacteric period. It included 146 postmenopausal women with AH (age median 57 years). The control group comprised 146 women without AH matched by age and duration of menopause. QL was assessed from the results of the SF-36 questionnaire and the severity of climacteric syndrome (CS) based on the modified menopausal index (MMI), pain in the back, joints, heart and headache, and also with the use of the visual analog scale (VAS). The severity of CS in patients with AH, headache, and
cardialgia
was much greater compared with controls. AH was associated with limitations as assessed by physical functioning (P = 0.004), physical role functioning (P = 0.026), role-emotional functioning (P = 0.0001), and psychic health scales P = 0.011). At the same time, AH was unrelated to substantial limitations as apparent from pain (P = 0.715) and social functioning (P = 0.052) scales. Estimates of general health status in women with AH were significantly lower than in its absence (P < 0.001). To summarize, AH considerably deteriorates QL in postmenopausal women.
